Bipolar hip arthroplasty in ankylosing spondylitis.

Nineteen severely affected hips in 12 young patients with ankylosing spondylitis were treated with bipolar hip arthroplasty followed by a comprehensive postoperative rehabilitation programme. Additional operative procedures of adductor tenotomy, knee flexion release, supracondylar femoral osteotomy for genu valgum and tendo Achillis lengthening for equinus contracture were required in 13 limbs. After a mean follow-up of 45.8 months, results were good in all cases with respect to relief from pain, and there was significant improvement in posture and function. All the patients resumed employment. The average gain in sum total range of hip movements was 194 deg, and the mean preoperative Harris hip score of 27.8 improved to 82.6 after bipolar arthroplasty. Ectopic ossification did not develop in any case. Bipolar hip arthroplasty appears to be ideally suited for young patients with ankylosing spondylitis.

A radiological sign in pulled elbows.

A new radiological sign of distal shift of the radius compared to the ulna is described in the condition of pulled elbow. This measurement has been termed the proximal radial length and was altered in 21 out of 25 cases of pulled elbow diagnosed clinically. The normal relationship was restored after manipulation.

Bipolar hemiarthroplasty in femoral neck fractures.

Thirty-two elderly patients with a femoral neck fracture treated by bipolar hemiarthroplasty and 36 patients (matched for age) with an Austin-Moore hemiarthroplasty were followed-up and compared. Bipolar replacement resulted in a higher percentage of satisfactory results, less postoperative pain, greater range of movement, more rapid return to unassisted activity, fewer unsatisfactory results and no acetabular erosion. The device functioned as bipolar in all the cases studied for inner-bearing motion.

Percutaneous pinning for radial neck fractures.

Eight patients with angulated radial neck fractures were successfully treated by percutaneous reduction and pinning. The operative technique is simple and is not associated with any important complications. This technique may however not be suitable for radial neck fractures with major translocation.

Classification and treatment of volar Barton fractures.

Results of treatment in 78 patients with volar Barton's fractures have been analysed. On radiological assessment these fractures were divided into three distinct categories and this classification is related to the results of closed manipulation. Results are best when this fracture is treated with open reduction and internal fixation. In non-operatively treated cases results are much better if the articular step is less than 2 mm.

Rotatory dislocation of the second carpometacarpal joint: case report.

Isolated dorsal dislocation and rotatory dislocation of the second carpometacarpal joint is rare. A case of rotatory dislocation of this joint is presented, where reduction was blocked by entrapment of the extensor carpi radialis longus tendon in the joint. A roentgenographic technique for evaluating dislocation of carpometacarpal joints is also described.

Ipsilateral radial head dislocation with radial shaft fracture: case report.

An extremely rare injury, traumatic radial head dislocation with concomitant fracture of the radial shaft, is reported. The dislocated radial head could not be reduced by closed technique because of capsular interposition. Reduction of the radial shaft fracture was possible only after the dislocation of the radial head was reduced.

Gastric teratoma in infants--a report of two cases.

Gastric teratoma is an exceeding rare lesion seen most often in male infants. Two cases of gastric teratoma, the first in a three and half month male, and the second in a one year old male child are reported. In the first case, the tumour was more solid, while in the second case it was a solid-cystic mass. These two cases represent the additions to the few, limited, known gastric teratomas reported in the world literature.

High valgus tibial osteotomy for osteoarthritis of the knee.

We report a statistical analysis of the results of 65 tibial osteotomies for primary osteoarthritis of the knee followed up at 3 and 5 years. Preoperative walking ability and the severity of pain are the factors with which the long term prognosis can best be correlated. The preoperative range of motion and mild subluxation become important at 5 years, but these factors were not important at 3 years. Mild instability did not have any influence. The severity of the osteoarthritis in radiographs and the postoperative femoro-tibial angle did not have any important association with the result. The results are therefore best predicted by preoperative pain and activity. The degree of correction did not affect the result.

Isolated fracture-dislocations of the first tarsometatarsal joint.

Eight cases of isolated fracture-dislocation of the first tarsometatarsal joint are presented. This is an uncommon injury and a distinct entity from other tarsometatarsal joint injuries. Reduction of both the fracture and the dislocation is easy but is unstable. Kirschner wire and screw fixation adequately stabilizes the reduction and gives good results. Results of conservatively treated cases are usually poor, since redisplacement in plaster is quite common.
