[Priapism. Apropos of 10 cases].

The authors report a retrospective study collecting ten cases of priapism over fifteen years. Clinical diagnosis is evident. Idiopathic priapism is most frequent of all the medical treatments available, no one seems to be efficient. Among surgical procedures draining the venous blood from the cavernous bodies, spongiocavernous shunt (Al-Ghorab) has been a simple, fast and effective method in the last five patients. Functional result depends on the age and especially on the precocity of the surgical treatment. Therefore priapism is an andrologic emergency.

Serotonin and dopamine turnover in the female rainbow trout (Oncorhynchus mykiss) brain and pituitary: changes during the annual reproductive cycle.

Brain serotonin (5HT) and dopamine (DA) turnover were studied at various stages of the reproductive cycle of the female rainbow trout by simultaneous determination by HPLC of neurotransmitters and major related metabolites. An increase of 5HT turnover in telencephalon and hypothalamus and a decrease of DA turnover in pituitary and hypothalamus were observed during the periovulatory period. Some changes also occurred during vitellogenesis: decreased 5HT metabolite in telencephalon and preoptic area and increased DA content in preoptic area. These data suggest that physiological fluctuations of biogenic amines could be involved in both ovarian recrudescence and ovulation, with major effects on the hypothalamo-hypophysial complex during the periovulatory period.

Effects of estradiol on brain aminergic turnover of the female rainbow trout (Oncorhynchus mykiss) at the beginning of vitellogenesis.

Brain serotonin and dopamine (DA) turnovers in the female rainbow trout were studied at the beginning of the vitellogenesis and related to blood estradiol (E2) levels; pituitary and plasma gonadotropin (GtH) were also assayed. Ovariectomy did not modify brain aminergic turnover. E2 replacement on ovariectomized fish increased hypothalamic DA turnover (increased DA and increased DA metabolites). E2 stimulated GtH synthesis (positive feedback) but did not enhance GtH release; hypothalamic E2-mediated aminergic inhibition upon release was suspected. Individual relations between blood E2 levels and catecholaminergic neurotransmitters were determined. A linear positive correlation (r = 0.82) was found for the hypothalamus, but not for the pituitary, the preoptic area, or the telencephalon. These data suggest that an activation of hypothalamic tyrosine hydroxylase (the limiting step of catecholamines synthesis) by E2 could develop as vitellogenesis proceeds.

[Isolated retrovesical and retroperitoneal hydatid cysts. 5 case reports].

In the light of five cases of isolated retrovesical and retroperitoneal hydatid cysts, the characteristics of these rare locations are reviewed and the hypotheses of pathogenesis are discussed. An abdominal or abdomino-pelvic mass is the most frequent sign. Ultrasonography and computed tomography are valuable in the diagnosis of these lesions. Partial cysto-pericystectomy via an extraperitoneal approach is the treatment of choice for these hydatid cysts.

[Traumatic rupture of the corpus cavernosum. 25 case reports and literature review].

On the basis of a series of twenty-five cases of traumatic rupture of the corpus cavernosum and a review of literature, the characteristics of this rare affection are reviewed. This injury typically occurs in young adults. The most frequent mechanism is rough manipulation of the erect penis. Urethral injury is associated in 10 per cent of patients. The characteristic history and physical findings usually yield the diagnosis. The preferred form of management is early surgical repair of the tunical defect, by distal circumferential incision. Post-operative complications are rare (8 per cent). The treatment of associated urethral injuries and late complications resulting from untreated rupture is reviewed.

[Psoas bladder hitch procedure. Apropos of 20 cases].

The authors report their experience about 20 cases of psoas bladder hitch performed on 17 women and 3 men. The indications were pelvic ureteral traumatism (10 cases), tuberculous ureteral stenosis (6 cases) and megaureter (4 cases). The technique is described. Recovery was uneventful in the majority of cases, and delayed complications were rare (one case of vesico-ureteral reflux). The results were encouraging, classified in three groups: good results (86 per cent), medium results (7 per cent) and bad results (7 per cent).

[Diagnostic and therapeutic approach to stress urinary incontinence in women].

This work concerns twenty eight cases of urinary stress incontinence treated between 1977 and 1987. The high incidence of previous perineal operations and multiparity are noted. The diagnosis has been established clinically in all cases. Different types of surgery for urinary stress incontinence have been used: endoscopic bladder neck suspension in 17 cases, and retropubic colpopexy in 9 cases. The results are good in respectively 88% and 78%. Correct diagnosis and operative indication are the main elements of treatment of urinary stress incontinence. Each procedure permitting reposition of the bladder neck and proximal urethra in abdominal manometric enclosure amends the urinary stress incontinence. This endoscopic bladder neck suspension procedure is indicated. Otherwise, this procedure is quick, simple, allowing an easy access even though for multi-operated and fat patients. The failure's factors are analysed.

[Volvulus of the sigmoid. Review of 38 cases].

A retrospective study of 38 cases of sigmoid volvulus is reported. The relative frequency of this pathology among young people in Morocco is noted. In 82% of cases diagnosis was without difficulty; and in two cases an associated small bowel volvulus was found. When deciding on the therapeutic approach it is essential to differentiate between cases in which the sigmoid colon is necrotic and those in which the changes are reversible. In the latter, the authors recommend a non surgical reduction (using flatus tube or better: colonoscopy with elective surgery performed at later date. If initial reduction proves impossible, colonic resection with colostomy appears to give the best results.

[Percutaneous fixation of the bladder neck under endoscopic control in the treatment of stress urinary incontinence].

After reviewing the technique of the endoscopic suspension of the vesical neck, the authors report their experience about the use of paravaginal tissue from either side of the bladder neck to treat female stress incontinence. The authors operated upon 20 female patients with clinically demonstrable stress incontinence of mine between January 1985 and June 1989. With periods of follow up of 6 to 53 months, the results are good in 91% of cases. The characteristics of this procedure are emphasised. It is quick, simple, allowing an easy access even though for multi-operated and fat patients, with possibility of simultaneous treatment of an associated cystocele or rectocele. The post-operative morbidity and the tissular lesions are minimal. Thus, the post-operative hospital stay could be reduced and an eventual second intervention easy. The simplicity of this procedure and the good functional results make the authors indicate it in all urinary stress incontinence.

[Fallopian tube bilharziasis caused by Schistosoma intercalatum disclosed by hemoperitoneum].

Schistosomiasis due to Schistosoma intercalatumis common in Gabon and, since 1971, Schistosoma haematobium has been found in the south of the country. Genital involvement is infrequent: 0.8 per cent of gynecologic specimens sent to the pathology laboratory and ten of 622 fallopian tube specimens (1.5 per cent). We report an unusual clinical case with hemoperitoneum as the first manifestation: the schistosomal lesion was developed within the tubal serosa, and caused a breach in an arteriole responsible for a 2500 cc hemoperitoneum. Genital lesions preferentially involve the vulva, vagina, and cervix. Involvement of the tubes and ovaries is less common and seems to have an incidence on fertility only if bacterial superinfections cause adhesions around the tubes and ovaries. Genital schistosomiasis is usually asymptomatic. Ectopic pregnancies and infertility may occur.

[Unusual causes of hemoperitoneum of genital origin].

Extra-uterine pregnancy is the most common emergency in gynaecology and obstetrics in the Hospital Centre of Libreville, and the diagnosis of blood in the peritoneal cavity is synonymous with extra-uterine pregnancy. All the same, when the authors reviewed 429 cases of haemoperitoneum 17 were shown not to be due to ectopic pregnancy. Ruptures of ovarian cysts and uterine perforation from illegal abortions are often found but more rarely, have been rupture in the blind horn of a bicornuate uterus, choriocarcinoma in the tube, tubal bilharzia and thrombopenia due to AIDS. It is essential to carry out histological examination for the differential diagnosis between an ovarian pregnancy and a ruptured cyst as well as choriocarcinoma or bilharzia. If it is not possible to find the site of the bleeding even after looking underneath the mesocolon, a search must be made for coagulation disorders due to a virus such as HIV.

[Neonatal treatment of congenital hip dislocation using an abduction pillow].

Numerous and various devices are being used in the treatment of congenital dislocation of the hip before the age of 1 year. We prefer the abduction pillow of Becker, owing to the simple design and use; there is also an economical factor: everybody can afford it. The abduction pillow can in no way be used for prevention. Only pathological hips with clinical signs confirmed by X-ray or sonography justify this treatment. Because of the good tolerance and efficacity of the pillow, it is the first choice treatment for unstable and luxated hips before the age of 4 months. If hips are not centered, other procedures should be used, in particular the Pavlik harness.

Neuronal pathways involved in the optokinetic head nystagmus of the frog.

The morphology of projection neurons in the basal optic nucleus and the pretectal area and the interconnections of these brain regions were studied with the aid of the cobalt-filling technique. It was found that the nucleus-sends long descending axons to the medullary reticular formation. The two basal optic nuclei are reciprocally interconnected and do not give a direct descending pathway. The pretectal nuclei and the basal optic nucleus are also reciprocally coupled. It is supposed that the described pathways mediate commands for horizontal and vertical nystagmic head movements.

[Penetrating abdominal wounds. Apropos of 330 cases].

Penetrating abdominal wounds are frequent and serious. 330 cases have been treated in the Department of Emergencies and visceral Surgery at Averroes Hospital of Casablanca from 1980 to 1990. The predominance of male sex is noted, and these wounds are always the result of aggression by white arm. All patients have been operated, the white laparotomy rate is of 36%. The classic interventionist attitude is still recommended in spite of this high rate, because our present conditions can't permit us a rigorous watching. According to abdominal lesions, the different interventions are reviewed, and their indications are detailed.

[Presacral cyst of vestigial origin in adults. Problems of diagnosis and therapy].

Concerning one case of cyst developing from embryological remnants in the retro-rectal space, the characteristics of these rare tumors are reviewed. The congenital cysts group is formed by teratomas and developmental cysts. Often they are uncovered when they become septic or when they compress adjoining organs. Computed tomography scan has proved valuable in the diagnosis of these lesions. But the precise diagnosis of a congenital cyst can only be made by pathological examination of the specimen after surgical removal. The possibility of malignant degeneration implies the systematic extirpation of any known pre-sacral cyst or tumor. Kraske's trans-sacral approach gives good access to the retro-rectal space. Sometimes, pelvic spread of these lesions requires the use of the abdominal approach. The prognosis for benign tumors is good, but it is poor for malignant tumors.

[Urethrocele in males].

Concerning one case of male urethral diverticulum, the characteristics of this rare affection are reviewed. Carriage for long time of urethral catheter and urethroplasty are the more frequent etiologies of this affection. The diagnosis is clinic, based on the discovery of tumefaction on the urethral track which pression makes spring up urine through urethral meatus. The urethrography confirms the diagnosis. Surgical approach consists on the resection of the diverticulum and urethrorrhaphy in one or two stages, based on diverticular neck size and sheet quality.
