[HLA typing in classical and African Kaposi's disease].

HLA A, B, C, typing have been done in 39 patients with clinically and histologically documented classical Kaposi's sarcoma. Thirty three were also typed for HLA DR antigens. Twenty seven were males, 12 were females and three ethnic groups were represented: european caucasoids 41%, north african caucasoids 38.5% and negroids 20.5%. The only statistically significant abnormality is an increase of HLA DR5 frequency (60.6 vs 26. p less than 0.001 et RR = 4.2). Such an increase has been evidenced also in AIDS patients, with or without Kaposi's sarcoma and then is not discriminant between all this different types of the disease.

[Cowden's disease or the multiple hamartoma syndrome].

Cowden's disease, also called multiple hamartoma syndrome, is a clinical entity characterized by hamartomatous tumours of endodermal, mesodermal and ectodermal origin. Although extremely rare, the disease must be known to all internists. A case of Cowden's disease in a 36-year old male patient is reported. The authors insist on the high incidence of digestive disorders and the risk of malignant degeneration of mammary and thyroid tumours. They also describe the cutaneous and mucosal lesions characteristic of the disease.

[Prognosis and treatment of scleroderma].

The prognosis and treatment of scleroderma depend on the clinicopathological form of the disease: superficial, localised scleroderma is only a minor cosmetic problem; circumscribed, deep forms of scleroderma may cause troublesome symptoms which justify steroid therapy. Circumscribed and diffuse scleroderma are similar to systemic scleroderma for which prognostic criteria have been identified. There is a poor prognosis in men, in extensive skin involvement and in intestinal, cardiac or renal involvement. Limited skin disease and anti-centromere antibodies are associated with a good prognosis. The absence of an animal model and the uncertainty as to the underlying cause, explain the therapeutic problems. The authors discuss the respective value of a number of "specific" drugs, including D-penicillamine which seems to be the most hopeful line of treatment. Continuous follow-up of this chronic condition remains essential for the diagnosis and symptomatic management of visceral complications.

Acute Disease↗

[Reappearance of soft chancre: comments on the current epidemic in Paris (author's transl)].

A total of 678 cases of soft chancre were treated in one hospital between 1973 and 1979. The majority of the patients were males (97 p. cent), and most of them came originally from Maghreb or Black Africa. Contamination was from prostitution (61 p. cent) or chance acquaintances (29 p. cent). The genital ulceration was often clinically atypical, frequently syphiloid; pain was present in only 59 p. cent of cases, and adenopathy, noted in 63 p. cent of patients, had the appearance of an inflammatory bubo in only 23 p. cent. The present high frequency of soft chancres, and their misleading clinical characteristics, makes it sometimes difficult to establish the clinical differential diagnosis from syphilitic chancre, but confirmation of the latter is supplied by direct examination and culture to search for Ducreys bacillus, this being indispensable for establishing the correct diagnosis. Treatment with sulfamethoxazole trimethopin or streptomycin prevents complications and cures the affection in one or two weeks.

[Cowden's disease : multiple hamartomata syndrome (author's transl)].

The third case of Cowden's disease to be discovered in France is reported. The authors emphasize the means for the diagnosis of multiple hamartomata, the typical symptoms and signs in the case reported, and that the family history included a daughter aged 8 years with macrocephaly, and slight mental retardation, who had already been operated upon twice for tonsillectomy.

Facial Neoplasms↗

[Kaposi's disease (author's transl)].

A case of Kaposi's disease is reported with a lesion in the buccal mucosa, and the history, classification, signs and symptoms, and treatment of this disease are described.

Aged↗

[Ulcerovegetating syphilid of the buccal mucosa (author's transl)].

The authors report a case of ulcerovegetating lesions of the buccal mucosa in a patient with confirmed cutaneous and biological secondary syphilis, and discuss the diagnostic features of ulcerovegetating mucous syphilids which correspond to the lesion originally described by Milian, and to the diagram illustrating the work by Grinspan quoted in the references.

Adjuvant chemotherapy in the management of primary malignant melanoma.

In a prospective randomized study, the effect of chemotherapy (either systemic or combined intraarterial and systemic) was studied in 117 patients undergoing a curative resection of Clark's level III, IV or V malignant melanoma. Systemic chemotherapy was started one month after surgery one week courses with an I.V. injection of Vinblastin 6 mg/m2, Thiotepa 6 mg/m2, Rufocromomycine 60 microgram/m2, Methotrexate 15 mg/m2 on day one with procarbazine 30 mg/m2 orally daily were given every other week for three months and later every four weeks. Intraarterial chemotherapy of DTIC 80 mg/kg day for ten days was given 28 days prior to surgery. 65 patients with limb malignant melanoma were treated either by surgery only (27 patients), or by systemic chemotherapy (23 patients) or by preoperative intraarterial chemotherapy and systemic chemotherapy (15 patients): 52 patients with non limb malignant melanoma were treated either by surgery only (28 patients) or by systemic chemotherapy (24 patients). We drew curves of disease free survival following surgery and studied the levelling off of the curves, 24 months after surgery 65% of the patients treated by surgery alone were alive and free of disease whereas 81% of the patients treated by surgery and chemotherapy were alive and free of disease (p less than 0.05) suggesting a possible benefit of adjuvant chemotherapy. Intraarterial preoperative chemotherapy has not proved of additional benefit to date.
