The treatment of breast hypotrophy has for a long time been considered to be one of the most difficult problems to resolve in our specialty. Even in the best hands and even with the most reliable prostheses in terms of manufacture and origin, the major risk of a fibrous capsule still persists. A a result of this complication, the most beautiful operation is doomed to failure with sometimes severe and always unacceptable morphological and psychological implications. The development of microthane-coated prostheses appears to have radically changed the problem. The incidence of fibrous capsules has fallen, according to most statistics, to 2 to 3% which obviously bears no relation to the rates observed previously (gel: more than 50%, inflatable: 30%). We have not observed any complications inherent to the presence of microthane: difficulty of removal, increased incidence of infection. The only particular feature is possibly an increased difficulty of insertion. Although not a miracle cure, this implant appears to be remarkable and its qualities have resisted the test of time: almost ten years in the USA and four years in our personal experience.
