Effect of sodium cholate on the phase transition temperature of dipalmitoyl phosphatidylcholine.

On treatment with sodium cholate, the phase transition temperature (Tc) of multilamellar liposomes derived from dipalmitoyl phosphatidylcholine (DPPC) progressively decreased with increasing cholate: lipid-P molar ratio. A molar ratio of 4.5 caused a one degree depression in Tc. The unilamellar cholate vesicles of DPPC exhibited the same Tc as that of the multilamellar ones. The formation of unilamellar vesicles from the multilamellar ones by cholate treatment was investigated by electron microscopy.

Primary pelvic neuroblastoma with central nervous system metastases.

Secondary intracranial neuroblastoma is an extremely rare site of metastasis from primary extracranial disease. Direct parenchymal involvement of the neuraxis without disease involving the overlying bone, dura, or venous sinuses is even rarer. We report a case of pelvic neuroblastoma with cerebral and cerebellar metastasis and communicating hydrocephalus, probably caused by diffuse leptomeningeal involvement. Clumps of neuroblastoma cells were seen in the cerebrospinal fluid (CSF). The CSF pathway was the probable route for neuraxis dissemination.

Purification and characterization of a 31 kDa mycobacterial excretory-secretory antigenic protein with a diagnostic potential in pulmonary tuberculosis.

Proteins secreted into the culture medium by Mycobacterium tuberculosis are shown to be a source of antigens of immunodiagnostic importance. In our earlier study, we had reported a 31-37 kDa seroreactive gel-eluted antigenic fraction (ESAS-7), isolated from culture filtrate proteins of Mycobacterium tuberculosis H37Ra. In this report, we describe further purification of excretory-secretory ESAS-7 antigen fraction by fast protein liquid chromatography (FPLC) on Resource 'S' cation-exchange column and isolation of a more active and purified protein antigen fraction ESAS-7F. ESAS-7F antigen was characterized as a 31 kDa molecular weight glycoprotein containing a metallo-serine protease activity. N-terminal sequence analysis showed the first five amino acids as NTGQS (Asp-Thr-Gly-Glu-Ser). The present study helped in the isolation of a well characterized 31 kDa mycobacterial glycoprotein antigen with protease activity and diagnostic potential in detection of tuberculosis infection.

Chemomodulatory action of Brassica compestris (var sarason) on hepatic carcinogen metabolizing enzymes, antioxidant profiles and lipid peroxidation.

The effect of two different doses (400 and 800 mg/kg body wt/day for 15 days) of a 95% ethanolic extract of the seeds of Brassica compestris (var sarason) was examined on carcinogen metabolizing phase-I and phase-II enzymes,antioxidant enzymes and glutathione content and lipid peroxidation in the liver of Swiss albino mice. Positive control mice were treated with butylated hydroxyanisole (BHA). Significant elevation in the levels of cytochrome p450 (p<0,.05), cytochrome b5 (p < 0.05) glutathione s-transferase (p<0.01), DT-diaphorase (p<0.05), superoxide dismutase (p<0.01), catalase (p < 0.001) and reduced glutathione (p<0.001) was noted in the group treated with 800 mg/kg body wt. of Brassica extract in comparison with the negative control group. Brassica compestris acted as a bifunctional inducer since it induced both phase - I and phase - H enzyme systems. Since phase-I and phase-II enzymes are considered to be reliable markers for evaluating the chemoprevention efficacy of particular test materials,these findings are suggestive of potential chemopreventive roles for Brassica seed extract.

Community Psychiatry Clinics at Sundarban: a clinical and cultural experience.

A series of Community Psychiatric Clinics were conducted in different blocks of Sundarban region of West Bengal. One of the primary objectives of this was to collect clinical epidemiological data on psychiatric morbidity in the region. A total of 26 clinics were conducted in Sagar, Kakdwip, Canning and Gosaba block of the Sundarban region during the period from end 1998 to end 2000. A total of 451 psychiatric cases with diagnostic categories (male 239, female 212) and 215 non-psychiatric cases (male 107 and female 108) were seen in these clinics. Diagnostic Interview Schedules (SCID) and Clinical rating scales like Hamilton Depression Rating Scale and Brief Psychiatric Rating Scales were used to ascertain clinical diagnosis quantitatively. Special emphasis was given on common psychiatric disorders.

Realistic expectations for treatment success in Alzheimer's disease.

Alzheimer's disease (AD) is a progressive degenerative disease that warrants active management to delay or slow progression of its symptoms. The symptoms of AD encompass behavior and daily function as well as cognition, so clinicians should take a global view in the assessment of treatment success. Because there is currently no cure for AD, one cannot expect an initial cognitive improvement observed in the first few months of therapy to be sustained indefinitely. However, one should expect that the patient who is treated early and persistently with medication for AD will show less evidence of behavioral, functional, and cognitive deterioration over a period of time than one would expect in the absence of pharmacotherapy. Thus, treatment success includes not only short-term improvement of symptoms but also less decline over the long term. Determination of treatment success therefore also requires awareness of the typical progression of untreated AD. In this article we review the natural history of AD and evidence for the effectiveness of the treatments indicated for AD: donepezil, galantamine, rivastigmine, and memantine.

Serum IgG anti-native type II collagen antibodies in rheumatoid arthritis: association with HLA DR4 and lack of clinical correlation.

Serum anti-type II collagen antibodies were measured in 85 rheumatoid arthritis patients before and after treatment with disease modifying drugs. These patients were also serotyped for HLA class II antigens. High anti-type II collagen antibodies (anti-CII) were detected in 35% and 29% of patients at onset and completion of study, respectively. Eighty percent of patients with high anti-CII initially had HLA DR4 (p less than 0.05). There was no correlation of anti-CII levels with severity of disease or response to treatment.

Physiological role of dehydroascorbic acid.

Dehydroascorbic acid is present in insignificant amounts in plant and animal tissue but appears in considerable amounts under various physiological and pathological conditions. It is found increased: in blood of patients suffering from infectious diseases; in blood and tissues of thyrotoxic patients; in blood after injection of thyroxin, corticotropin and cortisone. In all the above conditions there is concomitant decrease in L-ascorbic acid and glutathione values of blood and tissues. Dehydroascorbic acid, however, disappears after continued administration of a high dose of ascorbic acid. The accumulation of dehydroascorbic acid seems to be an indication of ascorbic acid deficiency. The extreme sensitivity of the ascorbate system to physiological changes is suggestive of a major biochemical role for this redox system. Accumulated evidences indicate that dehydroascorbic acid possible control cell division.

Reliability of measurements of total, protein-bound, and unbound estradiol in serum.

Estradiol (E2) circulates in the blood in three states: unbound (U-E2), bound to sex-hormone binding globulin (SHBG-E2), and bound to albumin. There is evidence to support the concept that only U-E2 and albumin-bound E2, are bioavailable (i.e., rapidly extracted by tissues). A case-control study nested within a large cohort of women, in which we are examining the effect of estrogens on breast cancer risk, offered the opportunity to assess the reliability of measurements of E2, the percentage of SHBG-E2, and the percentage of U-E2 based on multiple annual serum specimens. Long-term (1-2 year) reliability, as estimated by the intraclass correlation coefficient, was assessed in a subgroup of 71 premenopausal and 77 postmenopausal controls for whom two or three serum specimens were assayed. In postmenopausal women the intraclass correlation coefficient for a single measurement of total E2 was only 0.51. As for the percentage of SHBG-E2, intraclass correlation coefficients were 0.83 and 0.94, and for U-E2, 0.72 and 0.77 in the premenopausal and postmenopausal groups, respectively. These data suggest that, whereas single determinations of total E2 are insufficient to reliably estimate a woman's true mean level, a single measurement of the percentage of SHBG-E2 or U-E2 is adequate to assess bioavailability of E2 in an epidemiological study, irrespective of day of the menstrual cycle.

Cell proliferation in the dorsal and lateral lobes of the rat prostate during postnatal development.

We have shown previously that cell proliferation occurs throughout the ducts of the rat ventral prostate during its postnatal development. Our objectives herein were to identify and quantify proliferating cells in the dorsal and lateral lobes, to quantify DNA synthetic activity in the distal and proximal segments of these lobes, and to compare the results obtained to the results of our previous studies of the ventral lobe. [3H]-Thymidine, administered in vivo, was incorporated into both the distal and proximal segments of day 10-60 rat dorsal and lateral prostates. Quantitative analyses of autoradiographs revealed that the percentages of labeled cells in the distal and proximal segments of the dorsal and lateral lobes were not significantly different at days 45 and 60, and they were only slightly (though significantly) different at days 10 and 20. Biochemical analyses of [3H]-thymidine incorporation similarly revealed no significant differences in the distal and proximal segments at days 45 and 60, and only small differences at days 10 and 20. The results of analyses of mitotic figures were consistent with the results of autoradiographic analyses, showing clearly that cell proliferation occurs throughout the rat dorsal and lateral prostates. These results show for the first time that cell proliferation occurs throughout the dorsal and lateral lobes, strikingly reminiscent of the proliferation pattern that we reported previously for the ventral lobe.

Fast-tracking of myocardial infarction by paramedics.

OBJECTIVE: To study the effectiveness of a fast-track method of admitting patients with myocardial infarction directly to the coronary care unit (CCU). STUDY DESIGN: Ambulance paramedic staff were trained and provided with a Life Pak XI Monitor/Defibrillator which can obtain a 12-lead electrocardiogram. When a diagnosis of acute myocardial infarction was made by the paramedics, the CCU was informed and the patient was directly transferred to the CCU, bypassing the accident and emergency (A&E) department. The appropriateness of admission to the CCU was assessed against set criteria. The time from call for help to the administration of thrombolytic therapy (thrombolysis time) in patients directly admitted to the CCU was compared with that in another group of patients with definite myocardial infarction who were admitted through the A&E department over the same period of time. RESULTS: Twenty-five patients were fast-tracked to the CCU. Diagnosis of myocardial infarction was confirmed on admission in 14. Thirteen were treated with thrombolysis as there were no contra-indications; of the other 11 patients, seven were diagnosed as angina, one had complete heart block, one had haemodynamically significant atrial fibrillation and two had non-cardiac chest pain. The average time from call for help to thrombolysis in this group was 82 +/- 32 minutes. This was significantly shorter (p < 0.02) than in the patients who were admitted through A&E, in whom the average time from call for help to thrombolysis was 112 +/- 35 minutes. Twenty-one of 25 fast-tracked patients fulfilled the criteria for CCU admission. CONCLUSION: The majority of fast-trackings are appropriate and will result in quicker administration of thrombolysis in hospitals where the facility for thrombolysis does not exist in the A&E department.
