[Contribution of the argon laser in iridotomy using the Yag laser].

Yag laser iridotomy was performed immediately after argon laser application in 59 eyes with chronic angle closure glaucoma, occludable angle and secondary glaucoma. Postoperative intraocular pressure was elevated temporarily more than 5 mmHg on 86.4% of patients. Iris haemorrhages occurred in 17% of cases when laser Yag iridotomy was carried out with a power of 4 to 6 mJ. Prior argon laser seemed to prevent iris haemorrhages.

Experimental formation of 100 nm periodic fibrils in the mouse corneal stroma and trabecular meshwork.

The so-called long-spacing collagen that exhibits approximately 100 nm periodicity has been observed in the interstitial connective tissue of various organs under normal and pathological conditions. Although we, as well as many other investigators, have reported an increasing amount of these structures with age in the human trabecular meshwork, the pathological significance and the mechanism of its formation are still unknown. We incubated mouse ocular tissues in culture medium containing 20 mM ATP and prepared them for electron microscopic observation according to the method of Bruns et al (J Cell Biol 103:393, 1986). After the incubation, abundant 100 nm periodic fibrils were observed in the corneal stroma and the region of the trabecular meshwork, both of which show no structure of 100 nm periodicity under normal conditions. For the experimental formation of 100 nm periodic fibrils, ATP, acidic condition and temperature around 37 degrees C are necessary. The 100 nm periodic fibrils observed in our experiment were very similar to long-spacing collagen, in that the dark transverse bands have 100 nm intervals and very fine filaments of 6-7 nm diameter axially connect the bands. Long-spacing collagen is not usually observed in the human cornea, even in aged persons. The results of our study suggest that the occurrence of long-spacing collagen is related to special conditions developing in the trabecular meshwork with age but not in the corneal stroma. Experimental studies of 100 nm periodic fibril formation in mice offer a useful model for the age-related increase of long-spacing collagen in the trabecular meshwork of the human eye.

[Analysis of motor response with respect to the time course in cyclofusion].

Torsional eye movements during cyclofusional responses are recorded objectively and analyzed with respect to the time course of the eye movement until the subject perceives a fused image. Eight subjects whose age ranged from 24 to 35 years old were studied. Torsional disparity was presented stepwise by a synoptiscope. The Number of subjects whose motor response was stepped was less than that of the subjects whose motor response was not stepped. The time course of responses varied in individual. Moreover, the number of the subjects whose motor response showed cyclovergence was less than that of subjects whose motor response showed cycloversion. Fusional cyclovergence is reasonable to bring the disparate images within Panum's area efficiently. However our analysis shows that stepped response and cyclovergence is less in number and the motor response is variable in each individual. Therefore, we do not accept the explanation that the cyclofusional motor response simply brings the disparate images within Panum's area. We speculate that cyclofusional motor response is carried out by a feed-back loop through the cyclofusional sensory input to fuse the disparate images.

[Studies of ocular fundus and visual functions in Kearns-Sayre syndrome--with special reference to the new stage classification].

This paper describes the fundus appearance, visual function and electrophysiological findings in five cases of Kearns-Sayre syndrome. In three of them, retinopathy was not seen before the onset of this syndrome. The characteristic "salt and pepper retinopathy" progressed to peripapillary loss of the RPE and choriocapillaris over a period of some years. The ERG, normal in the beginning, became extinct in these cases. They were also at first subnormal for the scotopic as well as for the photopic activity. We distinguished five stages of ocular manifestations in Kearns-Sayre syndrome. Stage 0: The fundus appearance and the visual function are normal. Stage I: "Salt and pepper retinopathy" occurs in the entire retina but the visual function and the ERG are still normal. Stage II: Abnormal visual function and ERG occur with retinopathy. Stage III: A chorioretinal atrophy progresses around the disc and nasal retina and the ERG becomes extinct. Stage IV: The retinopathy demonstrates the appearance of choroidal sclerosis.

[Study of dynamic counterrolling of the eye--quantitative analysis of effects by visual and vestibular systems].

Dynamic counterrolling of the eye is induced when the head is tilted to one side: it includes cyclorotatory nystagmus, which is affected by visual and vestibular systems. Both of these two ocular movements were measured quantitatively through a video recording system, and the role of visual and vestibular systems in visual stabilization was analyzed. The relationship between the optokinetic and vestibulo-ocular systems was variable in relation to the velocity of head tilting. The optokinetic system contributed more in slow head tilting. As the velocity of head tilting increased, the suppression by the optokinetic system was decreased and eventually the vestibulo-ocular system became more dominant. This suggests that the role of visual and vestibular systems in head tilting is similar to that in head rotation. Visual and vestibular systems seem to work together mutually with some individual variation in stabilizing vision with sensory control in the higher level of the visual cortex.

Three cases with light-induced retinopathy.

Three patients presented a characteristic retinopathy presumably induced by light arising from operating microscope during extracapsular cataract extraction combined with intraocular lens insertion. The operating microscope was equipped with video-recorder. Light sources were tungsten bulb. All cases were asymptomatic and an inspection at the post-operative control examination led to the detection of this type of retinopathy.

Form vision deprivation amblyopia: further observations.

Nine cases of esotropia occurring in deprivation amblyopia, where exotropia rather than esotropia is usually found, showed a refractive error of hypermetropia. This fact suggested that an accommodative factor is largely responsible for the development of esotropia. A- or V-pattern strabismus was encountered in a higher incidence in deprivation amblyopia than in ordinary strabismus. Pattern-reversal VEP showed more prominent abnormality than flash VEP did. Studies of the sensitive period of the visual system revealed that the sensitivity is likely to be low for a month or two after birth and increases with a peak around the 18th month of age, decreasing thereafter with a waning slope to the end of the 8th year of life.

Cavernous sinus subarachnoid diverticulum and sixth nerve palsy.

Metrizamide CT cisternography in a young woman with a chronic sixth nerve palsy demonstrated a cavernous sinus subarachnoid diverticulum originating from the cerebellopontine cistern. The possible etiologic role of this diverticulum in her palsy is discussed.

[Treatment of severe uveitis with cyclosporin A].

The results of a treatment with ciclosporin A in 4 patients with severe uveitis (2 cases with Behçet's disease, 2 cases Vogt-Koyanagi-Harada disease) are presented. Ciclosporin A produced a rapid regression of exudates in the ocular fundus and of the inflammatory signs at the anterior segment in the 4 cases. However, at the same time, side effects including the deterioration of the hepatorenal function developed. Interruption of the treatment with ciclosporin A led to the development of inflammation at the anterior and posterior segments in cases 2 and 4.

[T-lymphocyte subsets in 20 cases of uveitis of various type].

Lymphocyte T subsets are determined by system OKT in 20 patients with various types of uveitis (Behçet disease 8, Vogt-Koyanagi-Harada disease 5, anterior uveitis 4, Kirisawa type uveitis 1, uveitis associated with Psoriasis vulgaris 1, undetermined type uveitis 1). Augmentation of OKT 8+ (p less than 0.05) associated with reduction of OKT 4+/OKT 8+ was found as compared with normal individuals. Diminution of OKT 4+/OKT 8+ (p less than 0.05) was noted in 5 patients with Behçet disease in association with augmentation of OKT 8+ (p less than 0.005). In Vogt-Koyanagi-Harada disease there is augmentation of OKT 8+ (p less than 0.05) which is, however, not so marked as in Behçet disease. On the contrary anterior uveitis was characterized by a remarkable rise in OKT 4+/OKT 8+.

Stimulus deprivation amblyopia. Simultaneous recording of local macular electroretinogram and visual evoked response.

Local macular electroretinogram (ERG) and visual evoked response (VER) were recorded simultaneously in ten adult patients with stimulus deprivation amblyopia. The fundus was monitored by an infrared television fundus camera. The location of the stimulus spot and eccentric fixating point were also monitored during recording. Responses in amblyopic eyes were compared with those in the normal fellow eyes in terms of the size of the stimulus spot, location of the eccentric fixating point in response to the stimulus spot, and visual acuity. The amplitude of VER in the amblyopic eye showed enhancement when the stimulus spot centering around the fovea involved the eccentric fixating point. The amplitude and peak time of the ERG in the amblyopic eye was essentially normal regardless of the degree of eccentricity of the fixating point and visual acuity.
