Pharmacokinetics of bredinin in renal transplant patients.

A pharmacokinetic study of bredinin, a new immunosuppressive agent, was carried out in 28 renal transplant patients. Serum bredinin concentration-time curves were analyzed using a one-compartment open model with a first order absorption process. The peak serum bredinin level appeared 2.4 h after oral administration of bredinin 50-200 mg. The calculated mean peak serum level was 0.852 micrograms/ml/mg/kg, when the dose was adjusted to the body weight of the patient. In the dosage range used of 0.85-4.46 mg/kg, a linear relationship was observed between the dose and the peak serum bredinin level. The elimination rate of bredinin from serum was dependent on kidney function, and the elimination rate constant was well correlated with the endogenous creatinine clearance. No circadian rhythm was apparent in the elimination rate constant. The absorption rate of bredinin from the gastrointestinal (GI) tract was affected by GI diseases. The need for dosage adjustment based on the renal function of the transplant patient is suggested.

[Long-term result and its evaluation after aorto-coronary bypass grafting].

Since a first successful A-C bypass using saphenous vein graft was performed in 1970, eighty five patients with severe coronary heart disease were operated on in our clinic. Up to date, A-C bypass was carried out in sixty four cases, and aorto-coronary sinus bypass (Ao-Cs bypass) as an arterialization of the coronary venous system was done in one case in the group of angina pectoris. Three cases among sixty four patients who underwent A-C bypass and a case of Ao-Cs bypass died of low cardiac output syndrome postoperatively. Other two cases died of refractory arrhythmia and cardiomyopathy 6 years after surgery. The remaining fifty nine cases are doing well without any complications during follow up from ranging 2 months to 13 years (mean 3 years 4 months). Recently, no operative death was found during past 8 years with development of surgical technique and myocardial protection. Ventricular function (cardiac index, LVedp, Max LV dp/dt, EF, LVEDVI , double products at rest and during treadmill exercise test) disclosed good results especially in complete revascularization group. Graft patency had a close correlation between a diameter of the coronary artery at the anastomosis site and bypass graft flow after completion of bypass. Subsequently, a good patency rate (84%-86%) was obtained in cases with the coronary artery more than 1.5 mm in diameter. It could be concluded that A-C bypass was useful procedure to improve ventricular function as well as clinical symptoms and longevity of the life.

Comparison of efficacy of a new butyrophenone derivative, timiperone and perphenazine in schizophrenia by a multicentre controlled study.

The efficacy of timiperone in schizophrenia as compared with perphenazine was assessed in a double-blind fashion in 205 patients throughout a 12-week treatment period. More than half of the subjects were chronic schizophrenics. Global improvement in the timiperone treatment group was superior to that in the perphenazine group in view of a lower rate of aggravation. With regard to overall safety and general usefulness rating, there were no significant differences between the two drug treatments. However, timiperone showed less frequency of excitability, irritability, anxiety and anorexia than perphenazine on occasions during the 12 week treatment period. From these results it may be concluded that the efficacy of timiperone could be superior or equivalent to that of perphenazine which has been confirmed to be of use as an antipsychotic.
