Decreased serum interleukin-2 level in patients with chronic headache.

Some cellular immune functions are impaired in cluster headache patients. Interleukin-2 (IL-2) is a polypeptide secreted by antigen or mitogen-actuated T lymphocytes that functions as a growth factor for T cells. To investigate cellular immune functions in patients with chronic headache, we measured the IL-2 concentration of sera in patients with migraine and in patients with tension-type headache. Thirteen subjects suffering from migraine without aura (5 males and 8 females, mean age: 32.8 years) and 46 subjects (20 males and 26 females, mean age: 39.7 years) with tension-type headache (TH) were selected for this study. Forty-three normal healthy volunteers composed the control group (15 males and 28 females, average age 41.6 years). The IL-2 levels of sera were determined using enzyme-linked immunosorbent assay (ELISA) techniques. The IL-2 levels of sera were 3.18 +/- 1.8 U/ml (mean +/- SD) in the healthy controls, 2.29 +/- 2.6 U/ml in the patients with migraine and 1.59 +/- 1.0 U/ml in the patients with TH. The serum level of IL-2 in the patients with migraine was significantly lower than in the controls. The serum level of IL-2 in the patients with TH was significantly lower than in the controls. The central nervous system (CNS) has been considered to be involved in the development of the immune phenomena. In the patients with TH or migraine, reduction in platelet 5-hydroxytryptamine (5-HT) levels and sympathetic hypofunction have been observed. These phenomena might reflect decrease in 5-HT levels in CNS in the patients with TH or migraine. The decreased serum IL-2 level, observed in this study, might reflect a reduction in 5-HT or catecholamine levels in CNS in the patients with migraine or TH.

Serum interleukin-2 levels in patients with dementia of the Alzheimer type.

We examined serum interleukin 2 (IL-2) levels in 127 normal subjects and 22 patients with dementia of the Alzheimer type (DAT) using the enzyme-linked immunosorbent assay technique. There was no significant correlation between serum IL-2 levels and age. However, serum IL-2 levels in elderly normal subjects (older than 70 years of age) were significantly lower than in the younger normal subjects (younger than 30 years of age) (p less than 0.05). There were no significant differences in serum IL-2 levels between the patients with DAT and the age-matched normal controls. These results suggest that the changes of serum IL-2 levels in patients with DAT may be natural in the normal aging process.

Reduced natural killer cell activity in patients with dementia of the Alzheimer type.

We examined the natural killer (NK) cell activity in 50 patients with dementia of the Alzheimer type (DAT) and 37 age-matched normal controls. The NK cell activity in DAT was significantly lower than in the normal controls. The NK cell activity induced by either interferon-alpha (IFN-alpha) or interleukin-2 (IL-2) in DAT was also significantly lower than in the normal controls. There were no significant differences in the level of serum IL-2 and IFN-alpha between the two groups. As regards NK cell subsets, two-color flow cytometric analysis showed no significant differences between the percentages of Leu-11+ Leu-7- cells, Leu-11+ Leu-7+ cells and Leu-11- Leu-7+ cells in the two groups. Our results suggest that NK cells in DAT may have functional abnormalities and may provide important clues to fundamental cellular and molecular aberrations in DAT.

[Two cases of adult Down's syndrome presenting parietal low uptake in 123I-IMP-SPECT].

We performed 123I-IMP-SPECT to patients with Down's syndrome who were supposed to have neuropathological findings of dementia of the Alzheimer's type. In case 1, a 52-year-old man with dementia, bilateral posterior parietal low uptake and right temporal low uptake were noted. Uptake was markedly lower in the right hemisphere. Case 2, a 38-year-old man not exhibiting clinical features of dementia, presented slightly decreased uptake in bilateral posterior parietal areas. We conclude that the patterns of abnormally decreased uptake in these cases of Down's syndrome are similar to those in dementia of Alzheimer's type, and that there is a possibility to detect the predemented stage of Down's syndrome by 123I-IMP-SPECT.

Subacute meningoencephalitis associated with human T-lymphotrophic virus type I (HTLV-I). Report of a case.

We present a case of subacute meningoencephalitis associated with human T-lymphotrophic virus Type I (HTLV-I) infection, for HTLV-I antibody being positive in serum and cerebrospinal fluid. Necropsy findings disclosed leptomeningeal and parenchymal mononuclear cell infiltration with multinucleate giant cells, which are similar to those seen in subacute encephalitis with acquired immunodeficiency syndrome (AIDS).

Dot microfiltration sandwich EIA for the measurement of IGM concentrations in cerebrospinal fluid.

A sensitive and simple sandwich enzyme immunoassay (EIA) using a dot microfiltration apparatus has been developed for the measurement of immunoglobulin M (IgM) concentrations in cerebrospinal fluid (CSF). The lower detection limit was 0.5 micrograms/dl. We applied this method to the testing of 200 CSF samples with normal concentrations of albumin, IgG and total protein, and normal cell counting. The non-parametric 2.5 and 97.5 percentile limits of IgM and the IgM index were 6.1-64.3 micrograms/dl and 0.012-0.063, respectively. We also applied this method to the testing of samples from 11 cases of multiple sclerosis (MS) and 15 cases of Guillain-Barré syndrome (GBS). High values for IgM and the IgM index were observed in 18.2% and 9.1% of MS, respectively and in 66.6% and 20.0% of GBS, respectively.

Kappa/lambda ratios in IgG, IgA and IgM of cerebrospinal fluid and of sera in patients with multiple sclerosis.

Kappa/lambda (kappa/lambda) ratios of each immunoglobulin, i.e. IgG, IgA and IgM in cerebrospinal fluid (CSF) and in sera of patients with multiple sclerosis (MS) were analysed by sandwich enzyme linked immunosorbent assay. In CSF kappa/lambda ratios of IgG of MS patients were significantly (p less than 0.05) higher than those of normal controls, whereas the values of IgA and IgM did not differ significantly from those of normal controls. In sera kappa/lambda ratios of IgG, IgA and IgM did not differ significantly from those of normal controls. Our results suggest that in MS patients abnormal kappa/lambda ratios are also restricted to IgG components in CSF, as oligoclonal IgG bands are.

Transient global amnesia and falcotentorial meningioma--a case report.

A 59-year-old woman suffered from a single episode of transient global amnesia (TGA). A computerized tomographic scan and angiographic examinations revealed a falcotentorial meningioma, supplied by the posterior cerebral arteries. We suggest that TGA can be attributed to the vascular insufficiency in this case.

Oligoclonal bands staining for IgG, IgA, IgM, and kappa and lambda light chains in cerebrospinal fluid of patients with multiple sclerosis.

We investigated the oligoclonal bands (OB) in CSF of patients with multiple sclerosis (MS), Guillain-Barré syndrome (GBS) and aseptic meningitis (AS) using polyacryamide or agarose isoelectric focusing following electroblotting and immunostaining for IgG, IgA, IgM and light chains. OB staining for IgG was detected in 80% of patients with MS, 25% with GBS and 13% with AS. OB staining for IgA and IgM was not detected in any disease. In MS, OB were stained only for the kappa or lambda light chains, which was compatible with OB staining for IgG. These results suggest that in MS, OB are monoclonal because they can be stained for gamma heavy chains and for only one light chains.

Pure sensory stroke due to pontine haemorrhage.

Two cases of pure sensory stroke due to pontine haemorrhage are reported. Haematomas were located in the dorsolateral tegmentum of the pons, detected by computed tomographic scan. One showed the cheiro-oral syndrome on the left side. The other showed sensory dysaesthesia on the right side of the body, followed by dysaesthesia of cheiro-oral distribution. These cases indicate that the pons can also be one of the lesion sites of pure sensory stroke.

Electrophysiological studies of myoclonic jerks provoked by photic stimulation.

A patient with a type of myoclonus produced by somatosensory and photic stimuli was studied electrophysiologically. Jerk-locked averaging showed cortical spikes in association with the spontaneous myoclonus. High amplitude somatosensory evoked potentials and high amplitude visual evoked potentials were observed. Somatosensory and photic stimuli produced 2 or 3 successive electromyographic (EMG) discharges. The time interval from the onset of the preceding EMG discharge to that of the following EMG discharge produced by photic stimulation corresponded to the latency of a long-loop reflex. It also corresponded to the time interval from the onset of the preceding EMG discharge to that of the following EMG discharge produced by somatosensory stimulation. Not only somatosensory but also photic stimuli might excite the loop of a long-loop reflex and cause myoclonic jerks.

Conglutinin microtiter plate ELISA system for detecting circulating immune complexes.

A new technique for a conglutinin ELISA using a microtiter plate has been developed for measuring circulating immune complexes. The sensitivity of our system ranges between 0.2 and 3.2 micrograms equivalents of aggregated human IgG. We have applied this system to 21 normal healthy controls and 80 neurological patients and have found high percentages of positivity in multiple sclerosis and amyotrophic lateral sclerosis.

C1q microtiter plate ELISA system for detecting circulating immune complexes and its application of testing the sera in neurological diseases.

We have developed a C1q microtiter plate ELISA system for detecting circulating immune complexes. The sensitivity of our system ranges between 0.5 and 32 micrograms equivalents of aggregated human IgG per ml. We have applied this system to 21 normal healthy controls and 80 neurological patients. Average level of circulating immune complexes in normal healthy controls is 4.67 +/- 2.67 micrograms per ml. We choose the upper limit of normality as 10.01 micrograms per ml (M + 2SD) and define the cases above this limit as positive cases. We have found high percentages of positivity in multiple sclerosis and amyotrophic lateral sclerosis.

Peripheral nerve involvement in HTLV-I-associated myelopathy: report of a case.

A patient came to our clinic suffering from mild sensory loss, exaggeration in knee reflexes, bilateral extensor plantar responses and elevation of the antibody level to the human T-lymphotropic virus type I (HTLV-I). Electrophysiological and pathological studies showed axonal and demyelinating neuropathy. The condition appeared to be a slowly progressive myeloneuropathy. HTLV-I may play an important role in the pathogenesis of myeloneuropathy.

Lymphocyte proliferation and subpopulations in dementia of the Alzheimer type.

Lymphocyte proliferation in dementia of the Alzheimer type (DAT) was evaluated by the level of mitogen response using phytohemagglutinin, pokeweed mitogen, concanavalin A and staphylococcal protein A. Lymphocyte subpopulations in the peripheral blood were also investigated using a monoclonal antibody with a fluorescence-activated cell sorter. The mitogen response in DAT was not significantly different from that in the normal controls. Lymphocyte subpopulations (Pan T, helper T, suppressor T, B cell, HLA-DR positive cell and helper/suppressor ratio) in DAT were not significantly different from those in the normal controls. These results suggest that the lymphocyte proliferative response to mitogens and the subpopulations in DAT may be natural in the normal aging process.
