Lectin binding patterns of alveolar epithelium and subepithelial seromucous glands of the bronchi in sepsis and controls--an approach to characterize the non-specific immunological response of the human lung to sepsis.

A delayed or deficient immunological protection as well as an overstimulation of the mucosal immune system may act as possible additional promoters of sepsis-induced lung injury in patients suffering from a severe septic condition. Lectin-binding patterns in pulmonary tissue samples obtained at autopsy from septic patients and control individuals were studied using 11 carbohydrate-specific lectins (Con A, UEA, GSA I, GSA II, MPA, PNA, Jac, WGA, MAA, LPA, and SNA). There were no differences in the secretory product of serous parts of bronchial glands detectable in the two study groups, whereas lectin binding patterns of alveolar epithelium and mucous parts of subepithelial seromucous glands were different in sepsis cases when compared to controls. Apart from differences in binding sites for alpha-mannose, N-acetyl-neuraminic acid and alpha-(2-6)-galactose (as detected by different expression for Con A, MAA and SNA) in the two study groups, the main finding was that no binding sites for alpha-N-acetyl-galactosamine (as investigated by MPA immunoreactivity) could be detected on alveolar epithelial cells and mucous parts of subepithelial seromucous glands in sepsis cases in contrast to the presence of such binding sites in the control cases. We hypothesize that the finding of an altered secretory product of alveolar epithelial cells and bronchial glands in sepsis may be a result of specific carbohydrate deprivation or consumption, respectively, possibly due to direct bacterial effects or pathogenetic events in response to bacterial toxins during the complex cascade of the host's systemic inflammatory response in sepsis. The altered type of mucus glycoprotein physiologically secreted by alveolar epithelium and mucous parts of subepithelial seromucous glands of the bronchi with subsequent loss of a considerable proportion of protection of the mucosal barrier in sepsis may play an important additional role in the development of sepsis-induced lung injury.

[Diagnostic and therapeutic management of symptomatic cholecystolithiasis and pre-operatively suspected choledocholithiasis].

The goal of this study was to examine the effectiveness of endoscopic retrograde choledocho-pancreatography (ERCP) at the Unfallkrankenhaus Berlin in the setting of pre-operative suspicion of choledocholithiasis. The predictive value of various pathologic findings with regard to choledocholithiasis was to be determined in this patient population.All patients treated operatively for symptomatic cholecystolithiasis between August 2000 and August 2001 were evaluated retrospectively for the following variables: age, gender, operative strategy, intra-operative cholangiography, sonographic findings, occurrence of choledocholithiasis. In 21.4 % of 196 patients who underwent cholecystectomy (n = 42) a pre-operative ERCP was performed. In 19 of these 42 patients, no pathology was found on ERCP. The percentage of therapeutic pre-operative investigations was 47.6 % (n = 20). In 3 patients, a juxtapapillary diverticulum was seen.69.9 % of the operations (n = 137) were performed laparoscopically. 167 patients (85.2 %) underwent intra-operative cholangiography. In 4 patients, the ERCP was performed post-operatively. The indication for 3 of those 4 examinations was an abnormal intra-operative cholangiogram. Complications occurred in 4 of the 46 patients who underwent ERCP (8.7 %). Choledocholithiasis was found in 12.2 % (n = 24) of all 196 patients. The pathologic finding with the highest positive predictive value for the diagnosis of choledocholithiasis was the finding of a dilated common bile duct to more than 8 mm diameter (PPV 75 %). Among laboratory variables investigated in this study, the elevation of serum bilirubin level exhibited the highest positive predictive value (PPV 39.2 %).

Laparoscopic resection of the left pancreas: technique and indication.

Laparoscopic pancreatic resections are rare procedures. A particular position is held by the left resection. In animal trials and 37 operations performed to date, this laparoscopic procedure has been positively assessed. The diseases operated upon were nearly exclusively benign. From November 1998 to July 2001, we performed 5 laparoscopic distal pancreatic resections at our hospital. The indications were: 2 adenocarcinomas; 1 neuroendocrine carcinoma; 1 high malignant T-cell lymphoma, and 1 cystadenoma. Additional procedures in 3 patients were: gastrectomy and resection of the left liver in the case with T-cell lymphoma; resection of a distant metastasis in the liver in the case with advanced pancreatic carcinoma, and a partial adrenalectomy in the case with cystadenoma. We did not note any intra- or postoperative complications in our patients. First of all, benefit is to be found in the early postoperative course. All oncosurgical criteria could be fulfilled.

Spectroscopic identification and direct imaging of interfacial magnetic spins.

Using x-ray absorption spectromicroscopy we have imaged the uncompensated spins induced at the surface of antiferromagnetic (AFM) NiO(100) by deposition of ferromagnetic (FM) Co. These spins align parallel to the AFM spins in NiO(100) and align the FM spins in Co. The uncompensated interfacial spins arise from an ultrathin CoNiOx layer that is formed upon Co deposition through reduction of the NiO surface. The interfacial Ni spins are discussed in terms of the "uncompensated spins" at AFM/FM interfaces long held responsible for coercivity increases and exchange bias. We find a direct correlation between their number and the size of the coercivity.

Spin reorientation at the antiferromagnetic NiO(001) surface in response to an adjacent ferromagnet.

Polarization dependent x-ray photoemission electron microscopy was used to investigate the spin structure near the surface of an antiferromagnetic NiO(001) single crystal in response to the deposition of a thin ferromagnetic Co film. For the cleaved NiO surface we observe only a subset of bulklike antiferromagnetic domains which is attributed to minimization of dipolar energies. Upon Co deposition a spin reorientation near the NiO interface occurs, with the antiferromagnetic spins rotating in plane, parallel to the spins of the Co layer. Our results demonstrate that the spin configuration in an antiferromagnet near its interface with a ferromagnet may significantly deviate from that in the bulk antiferromagnet.

Studies of the magnetic structure at the ferromagnet-antiferromagnet interface.

Antiferromagnetic layers are a scientifically challenging component in magnetoelectronic devices, such as magnetic sensors in hard-disk heads, or magnetic random-access memory (RAM) elements. In this paper, it is shown that photoelectron emission microscopy (PEEM) is capable of determining the magnetic structure at the interface of ferromagnets and antiferromagnets with high spatial resolution (down to 20 nm). Dichroism effects at the L edges of the magnetic 3d transition metals, using circularly or linearly polarized soft X-rays from a synchrotron source, give rise to a magnetic image contrast. Images, acquired with the PEEM2 experiment at the Advanced Light Source, show magnetic contrast for antiferromagnetic LaFeO3, microscopically resolving the magnetic domain structure in an antiferromagnetically ordered thin film for the first time. Magnetic coupling between LaFeO3 and an adjacent Co layer results in a complete correlation of their magnetic domain structures. From field-dependent measurements, a unidirectional anisotropy resulting in a local exchange bias of up to 30 Oe in single domains could be deduced. The elemental specificity and the quantitative magnetic sensitivity render PEEM a perfect tool to study magnetic coupling effects in multilayered thin-film samples.

[[Surgical intra-articular interventions in arthrosis].

The reconstruction of weight-bearing joint surfaces has long been a challenge for the orthopedic surgeon. The statistical rise in life expectancy, the spread of high-risk sports and leisure-time activities combined with high demands for painfree joint function even in older age as a factor for contemporary quality of life are the demographic factors of the bone and joint decade announced by the WHO. Osteoarthritis represents a multifactorial, irreversible, often heterogeneous joint disease with joint destruction as the inevitable consequence. Conventional and innovative intra-articular operative methods for the treatment of osteoarthritis are presented and discussed. Tissue engineering stands for vitalizing tissue modification by modification of cellular and humoral mechanisms. Thus, new therapy options for curing degenerative joint diseases will arise in the future. Autologous chondrocyte transplantation (ACT) represents one of the first clinically established methods. The paradigm of treating osteoarthritis by the orthopedic surgeon will be multimodal in the future. Innovative techniques in the biotechnological sector provide a promising horizon.

Comparative evaluation of pulmonary lactoferrin and lysozyme immunoreactivity for the postmortem diagnosis of death due to sepsis.

To determine whether lactoferrin (LF) and lysozyme (LZ) can be used as immunohistochemical postmortem markers of sepsis, pulmonary tissue sections from autopsy cases of sepsis-related fatalities (n = 13) and control cases of non-septic fatalities (n = 14) were evaluated for differences in leucocytic immunoreactivity. LF and LZ were investigated in paraffin sections using the AEC technique. The immunohistochemical expression of both markers was scored, evaluating the quantity of immunopositive cells and the intensity of the intracellular immunoreactivity. There was a statistically significant association between an enhanced expression of LF on pulmonary leucocytes in sepsis-related fatalities in contrast to non-sepsis cases (P < 0.001), whereas no such difference could be observed for LZ immunoreactivity between the two study groups. Pneumonic tissue alterations had no significant influence on LF and LZ immunoreactivity, thus suggesting differences between the degranulation of these non-specific antibacterial agents in local and systemic inflammatory processes. While the variability of LZ immunoreactivity, possibly reflecting a non-specific release from lysosomes according to the length of the postmortem interval, limits its application to the postmortem diagnosis of sepsis, the immunohistochemical detection of an enhanced expression of LF can contribute to the postmortem discrimination between sepsis and non-septic fatalities.

[Laparoscopic surgery of rectal carcinoma. Radical oncology and late results].

The rectal cancer is one of the big challenges for laparoscopic surgery. From August 1993 to October 1999, at our hospital 108 rectal cancers including all UICC stages were laparoscopically operated on, 83 cases of them with curative intention. 77% of the carcinomas were situated in the middle or lower rectal third. A rectal amputation was performed in 13% of the cases, a continuity resection in 87%. All valid histopathological quality criteria were fulfilled: en bloc resection, total mesorectal excision, distal tumor free resection edge, and truncal dissection of the lower mesenteric artery. 25 lymph nodes per specimen were histopathologically described on average. All preparations of curative operations were assessed as R0 resections. In a median follow up of 30 months we saw locoregional recurrences in 1.2% and a recurrence-free survival in 90%. In a subpopulation with 4 years follow up, the rate of local recurrences was 2%, and the recurrence-free survival 85.7%. The value of multimodal therapy concepts in total mesorectal excision cannot be assessed at present yet. Laparoscopic rectal surgery is a very effective treatment option for rectal cancer, but it should be practiced only in specialized centres.

[Laparoscopic left pancreas resection in tumors. Initial clinical experiences].

Laparoscopic pancreatic resections are rare procedures with left resections considered as a special subject. In animal trials and in 26 operations performed so far, this laparoscopic procedure was assessed positively. The operations were carried out mainly for benign diseases. From November 1998 to July 2000, five laparoscopic left pancreatic resections were performed in our hospital for the following diseases: adenocarcinoma (2), neuroendocrine carcinoma, highly malignant T-cell lymphoma, and cyst adenoma. In 3 patients the pancreatic resection was completed by other procedures: the case of T-cell lymphoma by gastrectomy and left side hemihepatectomy, the case of advanced pancreatic carcinoma by resection of a liver metastasis, and the case of cyst adenoma by a partial adrenalectomy. There was no patient suffering from intra- or postoperative complications. The benefit is mainly noticed in the early postoperative period. All required oncosurgical criterias were fulfilled.

Internal current mark in a case of suicide by electrocution.

Fatalities caused by electrocution often lack specific morphologic evidence. Investigation of the death scene along with technical inspection of the electrical apparatus may help in clarifying the cause and manner of death. Cutaneous current marks may be the only sign of low voltage associated fatalities. The authors report a case of an electrician who committed suicide by electrocution using a time switch after oral ingestion of diazepam. Electrodes (coins) were fixed with adhesive tape at the height of the heart to the front and back of the left side of the chest. Autopsy revealed a blackish linear mark on the pleura parietalis of the inner side of the thoracic cavity, connecting the cutaneous current marks. Current-related and heat-related changes, such as hypercontraction bands of the intercostal muscles and coagulative changes of the perineurium of peripheral nerves, were found at histologic examination. Taking into consideration that the body had been under the influence of low-voltage current for 7 days, the morphologic alteration on the pleura parietalis was in accordance with an internal current mark, indicating the main route of current flow through the body. To the authors' knowledge, such a current-related phenomenon has not been reported to occur on an inner body surface before.

[Technique of rectum resection and excision with special reference to TME].

Rectal cancer poses an immense challenge within the field of laparoscopic surgery. Sufficient enlargement on a monitor and improved instruments enable an optimal preparation and the total mesorectal excision. The standardized technique is the key to success. By December 2000, 138 rectal carcinomas were operated upon in laparoscopic technique at our hospital. The operating steps are adapted to the criteria of the "open" surgery. A rectal extirpation was performed in 13%, a continuity resection in 87%. The rate of local recurrence was 3.8% after a follow-up of 40 months, the survival rate in curatively operated patients was 90%.

Direct observation of the alignment of ferromagnetic spins by antiferromagnetic spins

The arrangement of spins at interfaces in a layered magnetic material often has an important effect on the properties of the material. One example of this is the directional coupling between the spins in an antiferromagnet and those in an adjacent ferromagnet, an effect first discovered in 1956 and referred to as exchange bias. Because of its technological importance for the development of advanced devices such as magnetic read heads and magnetic memory cells, this phenomenon has received much attention. Despite extensive studies, however, exchange bias is still poorly understood, largely due to the lack of techniques capable of providing detailed information about the arrangement of magnetic moments near interfaces. Here we present polarization-dependent X-ray magnetic dichroism spectro-microscopy that reveals the micromagnetic structure on both sides of a ferromagnetic-antiferromagnetic interface. Images of thin ferromagnetic Co films grown on antiferromagnetic LaFeO3 show a direct link between the arrangement of spins in each material. Remanent hysteresis loops, recorded for individual ferromagnetic domains, show a local exchange bias. Our results imply that the alignment of the ferromagnetic spins is determined, domain by domain, by the spin directions in the underlying antiferromagnetic layer.

Oscillatory decay of magnetization induced by domain-wall stray fields

The demagnetization of a hard ferromagnetic layer via the fringing fields of domain walls created by reversing the moment of a neighboring soft ferromagnetic layer is explored experimentally. An unusual oscillatory decay of the magnetic moment of the hard layer is observed using structures in which the demagnetization occurs after a few hundred cycles. This surprising observation is confirmed on a microscopic scale by detailed imaging of the magnetization of the hard layer using high resolution photoemission electron microscopy and by micromagnetic simulations.

Observation of antiferromagnetic domains in epitaxial thin films

Antiferromagnetic domains in an epitaxial thin film, LaFeO(3) on SrTiO(3)(100), were observed using a high-spatial-resolution photoelectron emission microscope with contrast generated by the large x-ray magnetic linear dichroism effect at the multiplet-split L edge of Fe. The antiferromagnetic domains are linked to 90 degrees twinned crystallographic regions in the film. The Neel temperature of the thin film is reduced by 70 kelvin relative to the bulk material, and this reduction is attributed to epitaxial strain. These studies open the door for a microscopic understanding of the magnetic coupling across antiferromagnetic-ferromagnetic interfaces.

Cytokine profile of HIV-positive Kaposi's sarcoma derived cells in vitro.

Kaposi's sarcoma (KS) is the most frequent neoplastic complication observed in HIV-infected patients. Cutaneous KS is the most common manifestation but visceral and lymph node involvement may occur. HIV-infection does not only lead to a decrease of certain cell types (CD4 T-lymphocytes), but also modifies the function of non-infected cells such as B-lymphocytes and NK-cells by upregulating cytokine release of IL-1, IL-6, GM-CSF, IFN-gamma and TNF-alpha. These multifunctional mediators show both autocrine and paracrine proliferative effects on normal endothelial cells and AIDS-related KS-cells. Using ELISA-, RIA- and IRMA-techniques we analysed the influence of seven cytokines (IL-1beta, IL-6, TNF-alpha, GM-CSF, IFN-alpha, IFN-beta, IFN-gamma) and the soluble IL-2 receptor (sIL-2R) on the growth of eight different KS-derived cell lines compared with eight fibroblast cell lines, established from skin biopsies of HIV-positive individuals. Furthermore, we analysed the dose-dependent effect of the above mentioned cytokines on KS-derived cells in vitro. The KS-derived cell culture medium demonstrated significantly higher concentrations than the fibroblast cell lines in view of the following cytokines: sIL-2R, IL-1beta, IL-6, TNF-alpha, GM-CSF, IFN-gamma (p<0.05). The most pronounced differences between KS-cells and fibroblasts were observed for IL-1beta and IFN-gamma. The antiproliferative effect of IFN-beta and IFN-gamma began at a concentration of 20 and 50 IU/ml, respectively, whereas for IFN-alpha an antiproliferative effect was observed at a concentration of 100 U/ml. Furthermore we observed a proliferative effect in low concentrations (2-5 IU/ml) of IFN-gamma in our in vitro model

[Injection therapy in lumbar syndromes].

Injection therapy as part of orthopaedic pain treatment is of major importance in sciatica. It helps to differentiate unclear complaints and allows a break through of the vicious circle of pain and muscle spasm. Pharmacological principles and possible complications have to be observed. The different techniques need experience and manual training. The paper describes the various injection techniques in detail.

[Primary hypertrophic pyloric stenosis. A are form and stomach outlet stenosis in the adult].

A 38-year-old white female with primary hypertrophic pyloric stenosis is presented. The patient was admitted to our service with a history of upper digestive tract pain and postprandial vomiting since her 17th year of life. Diagnosis of benign pyloric stenosis was made preoperatively and the patient was successfully treated by Finney pyloroplasty. Primary hypertrophic pyloric stenosis in adults is a rare condition of unknown etiology. Only about 200 cases of primary hypertrophic pyloric stenosis in adults have been reported in the literature.
