Prognosis in acute renal failure of septic origin: a multivariate analysis.
The goal of the present study was to identify variables associated with the outcome of patients with acute renal failure (ARF) of septic origin, using multivariate analysis. The records of 168 patients were reviewed retrospectively and a crude mortality of 74% was found. Both univariate as well as multivariate analysis demonstrated an association between mortality and variables which depended on patient related factors. These included age over 60 years and several underlying diseases such as pneumonia, peritonitis, and organ dysfunction. Only one variable (late oliguria) related to the ARF itself. Thus, outcome seems related to underlying disease more than to severity of ARF.

[Post-traumatic tics].
Secondary tics are those in which an aetiology justifying them can be found, as compared to idiopathic tics, which make up the majority, and the Gilles de la Tourette syndrome (SGT), which is, at the moment, of unknown origin. Of the possible aetiologies described as causing tics, craneo-encephalic trauma has been mentioned on very few occasions. We present a case of post-traumatic tics (verbal and neck) in a young man of 24, and review the published cases which can be considered to be of post-traumatic tics. We have found six cases of tics secondary to traumas, all craneo-encephalic, like ours (the one under study). The time interval between the blow and the appearance of the tic or tics varied between 2 weeks and 3 months. The absence of significant lesions seen in the complementary investigations make it impossible for us to discover the site of the lesion caused by the trauma. However, the presence in some cases of other tics before the trauma, and of family histories of tics, supports the idea of a genetic basis or predisposition to suffer this disorder.
Cardiac arrest in a 31-year-old man because of the Arnold-Chiari malformation.
We describe a 31-year-old man who suffered from loss of consciousness and cardiac arrest with ventricular fibrillation after a brisk head movement. Clinical investigation was inconclusive apart from the magnetic, resonance imaging that showed the Arnold-Chiari malformation. It must be taken into account in the diagnosis, as a specific treatment is possible.

Reflex cardiovascular response to acute myocardial ischaemia.
The usual cardiovascular response to acute myocardial ischaemia is either pressor, characterised by tachycardia and hypertension or depressor, manifested by bradycardia, hypotension and systemic vasodilatation. We studied the incidence of ectopic arrhythmias, the changes in heart rate, in left ventricular pressure and dP/dt and the changes in plasma level of free fatty acids. Acute anterior ischaemia increased the heart rate, left ventricular dP/dtmax and plasma level of free fatty acids and developed frequent ventricular ectopic arrhythmias. Previous injection of propranolol prevented the elevation of plasma free fatty acids. Acute inferior ischaemia produced a slight decrease in heart rate, no change in plasma level of free fatty acids and a significant decrease in left ventricular dP/dtmax. The experimental model allowed us to study the response to acute myocardial ischaemia in conscious dogs; the results obtained point to a preferential location of the pressor and depressor receptors respectively to the anterior and inferior surfaces of the left ventricle.
