Localized non-Hodgkin's lymphoma of the adrenal and thyroid glands.

A case of immunoblastic lymphoma, involving only the thyroid and the adrenal glands, is presented. The patient had clinical symptoms and findings of Addison's disease, and computed tomography (CT) demonstrated bilateral adrenal tumoral enlargement. He also had euthyroid diffuse multinodular goiter. The diagnosis of the patient was based on the cytological examination of the aspiration materials from both endocrine glands. The patient received "m-BNCOD" chemotherapy regimen and replacement therapy for Addison's disease. At the end of three courses, a partial response was obtained.

C.T.-guided pain procedures.

C.T.-guided stereotactic destructive pain procedures; percutaneous cordotomy, trigeminal tractotomy-nucleotomy and extralemniscal myelotomy are presented. All procedures were applied in 32 cases without any mispuncture or complication. Advantages of C.T. guidance are direct visualization of target electrode relations, measurements of spinal cord diameters at the lesion site, and demonstration of spinal cord displacement during the procedure.

CT-guided percutaneous cordotomy.

Percutaneous cordotomy is a commonly applied and effective procedure among the ablative pain surgeries. As plain X-ray does not permit visualization of the target relative to the electrode the chances of obtaining good results are decreased and the risk of complications are increased. The use of CT has been found to be useful in cordotomy. The procedure is performed under CT control on the patients who have previously been given 5 ml iohexol into the subarachnoid space. The needle electrode is manipulated by free hand technique. It is possible to measure the diameter of the spinal cord and to detect cord dislocation in the spinal canal. When the electrode system is introduced it is possible to visualize the tip of the electrode which is pushing or puncturing the spinal cord. As the procedure directly visualizes the relation of the electrode to the target it is possible to place the electrode in the lateral spinothalamic tract. Another advantage of the procedure is to enable us to visualize haematomas or other changes that may result from the cordotomy. The application of the technique and clinical results will be presented.

CT-guided trigeminal tractotomy.

Trigeminal tractotomy is an effective procedure in denervating pain areas of 5th, 7th, 9th and 10th nerves. The classical imaging technique is the x-ray method which visualizes the target electrode relation indirectly. The method of CT-guided trigeminal tractotomy demonstrates the target electrode relation directly.

Intrarenal lipoma: report of a case.

We recently had the opportunity to study a case of renal lipoma. Pure intrarenal lipomas are very infrequent among all of the renal tumors and computed tomography is the only means of showing the fatty nature of such masses with its contrast discrimination property.

Asymptomatic-nonfunctional adrenal masses detected by CT.

We identified as an incidental finding "silent adrenal tumors" in 12 patients undergoing abdominal computed tomography (CT) scanning for unrelated problems. Until recently, adrenal masses came to clinical attention either from local symptoms due to massive enlargement or from manifestations of excess hormone production. With CT, small adrenal glands and neoplasms could be also visualized and diagnosed at an earlier preclinical stage. Patients with asymptomatic adrenal masses detected by CT scan must be investigated for the possibility of metastasis from another primary site, secondly screened for hormone production and should be followed-up for malignant degeneration. The investigation did not show any sound criteria for the diagnosis of malignancy except the follow-up of the masses with serial CT scans in short intervals.
