Nasal dermoid with intracranial extension.

Nasal dermoids are rare congenital anomalies, which usually present in early childhood as a midline nasal mass that requires surgical management. It is important to rule out any intracranial extension before subjecting the patient to surgery. We describe a case of nasal dermoid with intracranial extension. The embryology of the formation of the nasal dermoid, cyst and sinus, with the role of CT scan and MRI in diagnosing intracranial extension, and its management are discussed.

The first successful simultaneous pancreas-kidney transplant in India.

Insulin-dependent diabetes mellitus is associated with renal failure, diabetic retinopathy, neuropathy and vasculopathy. We report the first successful simultaneous pancreas-kidney transplant in India in a young diabetic with renal failure. The dual transplant has cured his diabetes and renal failure and has had a beneficial effect on his neuropathy, retinopathy and quality of life. Obstacles to dual transplant in India include a lack of suitable recipients and a cadaver donor programme that is still in its infancy.

Micro-arteriovenous malformations of the brain: superselective angiography in diagnosis and treatment.

PURPOSE: We assessed the usefulness of superselective angiography in patients with micro-arteriovenous malformations. PATIENTS AND METHODS: Five patients had superselective angiography for either diagnosis or treatment of brain arteriovenous malformations having a nidus of less than 1 cm. All patients presented with an intracerebral hematoma. RESULTS: In one patient superselective angiography confirmed the presence of a shunt that was suspected due to visualization of an early vein on the follow-up angiogram; in another patient superselective angiography was helpful in defining the topography of the malformation; in the remaining three patients, superselective angiography was used for embolization of the malformation. We were unsuccessful in achieving a complete angiographic cure in these 3; however, superselective angiography revealed an aneurysm in one that was obliterated by the liquid adhesive embolic agent. CONCLUSION: With the recent advances in microcatheters, superselective angiography has a promising role in defining the topography and angioarchitecture of micro-arteriovenous malformations. Micro-arteriovenous malformations may not be angiographically evident at the time of initial hemorrhage due to the hematoma. With future technical advances it may be possible to completely obliterate these malformations by embolization alone.

Ultrasonography of carcinoma of the gallbladder: an analysis of 80 cases.

Eighty patients with primary carcinoma of the gallbladder were studied by ultrasonography. Two broad groups were defined. In group I (72.5%), the gallbladder lumen was identified along with a mass lesion, and in group II (27.5%), a large mass totally replacing the gallbladder was seen. Group I was further categorized into three subtypes according to the morphology of the lesion: type 1--a mass almost filling the lumen; type 2--a polypoidal mass projecting into the lumen; and type 3--an infiltrating mass. Associated findings of infiltration of the liver, presence of calculi, biliary tree obstruction, liver metastasis, and lymphadenopathy further supported the diagnosis. Ultrasonography was found to be highly reliable in the diagnosis of carcinoma of the gallbladder. In addition, it provided guidance for biopsy/aspiration of nonpalpable tumors. Its shortcomings appear to be its inability to assess peritoneal and bowel involvement, as well as that of the omentum and posterior rectus sheath.

Neuroendocrine tumor (paraganglioma) of the cauda equina: MR and pathologic findings.

The MR and pathologic features of a case with neuroendocrine tumor (paraganglioma) of the cauda equina are presented. MR showed the tumor to be hyperintense on postcontrast examination and also showed serpiginous flow voids suggesting vessels capping the tumor. A neuroendocrine tumor should be considered in the differential diagnosis of tumors in this location.

Unusual location of a shunt tube in the basal subarachnoid space.

The authors observed flow-void in a shunt tube in the basal cisterns of a 28-year-old man who had previously undergone surgery for a craniopharyngioma. They describe the appearance of a subarachnoid shunt catheter in the basal cisterns to caution against misinterpreting it as an aberrant vessel.

The quality of life in successful live-related renal allograft recipients in India.

BACKGROUND: The high cost of maintenance of haemodialysis makes most patients in India and elsewhere opt for a renal transplant. The degree of rehabilitation can best be assessed by evaluating the quality of life in successful recipients. METHODS: We studied vocational rehabilitation, social relations, sexual and married life, psychological status and life satisfaction in 51 successful live-related renal allograft recipients using Schwab's depressive scale, Bigot's life satisfaction index and the Kamofsky physical scale. RESULTS: Eight-four per cent of our patients had returned to their original jobs. Ninety-eight per cent of patients had a Kamofsky scale of 90-100 and 81% were leading a normal married life. Ninety-four per cent of them led an active social life. CONCLUSION: Successful live-related renal transplantation is associated with a good quality of life and should be the treatment of choice for patients with end-stage renal disease.
