Local induction of a tumor necrosis factor (TNF)-like cytotoxic factor in murine tissues with tumorous and nontumorous inflammation after systemic administration of antitumor polysaccharides.

Local induction of a cytotoxic factor (CF), which was reported by us to be a tumor necrosis factor (TNF)-like molecule, in murine tumor tissues by i.v. administration of antitumor polysaccharides was studied. The CF was measured by cytolysis assay against L929 fibroblasts in vitro. The antitumor polysaccharides mannoglucan polyalcohol (MGA), lentinan, carboxymethyl-(1----3)-beta-D-linear glucan DP540 (CM-TAK) and yeast mannan induced the CF in MH134 hepatoma tissues inoculated intradermally, with MGA inducing the highest level of the CF. MGA induced the CF in MM46 mammary carcinoma, Ehrlich carcinoma, and MH134 hepatoma, the growth of which were all inhibited by MGA, but not in Lewis lung carcinoma and EL-4 lymphoma, which are therapeutically resistant to MGA. MGA induced the CF in solid MH134 hepatoma tissues inoculated subcutaneously or intramuscularly as well as intradermally, but not in ascitic fluids with intraperitoneal MH134 hepatoma on which MGA is ineffective. These findings suggest that CF induction is correlated with the antitumor activity of polysaccharides. CF induction in tumor tissues was detectable 6 h after i.d. inoculation of MH134 hepatoma. Even in nontumorous inflammatory skin tissues produced by injection of TAK, the CF was induced by MGA. Thus, the early inflammatory reaction with accumulation of host cells and MGA treatment act cooperatively in local induction of the CF.

Plasma cells in the bronchoalveolar lavage fluid of a patient with eosinophilic pneumonia. Morphologic proof of local production of antibodies.

We describe a case of eosinophilic pneumonia in which plasma cells appeared in bronchoalveolar lavage fluid (BALF). A 49-year-old man presented with wheezing, lung infiltrates, peripheral eosinophilia, and extremely high IgE levels in serum and BALF. A differential count of BALF revealed 56.6 percent lymphocytes and 1.3 percent plasma cells. The appearance of plasma cells suggests local maturation of B cells and represents a morphologic proof of local production of immunoglobulins. The increased number of lymphocytes suggests their role in B cell differentiation via the lymphokine network.

A case of Graves' disease associated with an autonomously functioning thyroid nodule (AFTN) (Marine-Lenhalt syndrome) which spontaneously became a cold nodule.

We report a 31-year-old female with Graves' disease associated with an autonomously functioning thyroid nodule (AFTN) (Marine-Lenhalt syndrome) in which the AFTN spontaneously became a cold nodule. Initially the patient was thyrotoxic and had diffuse goiter with an elevated radioiodine uptake. She became euthyroid following six months of antithyroid drug therapy, and in addition to diffuse goiter, the solitary hot nodule was palpable in the left lobe. Fourteen months later, hyperthyroidism recurred and the thyroid scan revealed diffuse radioiodine uptake with a cold area in the nodular region. The resected nodule showed extensive degeneration and the histological diagnosis was follicular adenoma with Graves' disease. We discussed the significance of recognizing the syndrome and also compared the frequency of spontaneous degeneration in AFTN and in solitary cold nodules.

[The effect of vagotomy and gastrectomy on the lower esophageal sphincter motility].

The motor activity of the canine lower esophageal sphincter (LES) was studied under conscious states by means of the strain gage transducer, and gastroesophageal reflux was investigated after injection of the contrast medium through the silastic tube inserted in gastric fundus by x-ray. The motor activity and function of LES were observed in the dogs with selective proximal vagotomy (SPV), truncal vagotomy (TV) and TV + gastrectomy. Results were summarized as follows; 1. In the postprandial period, so called receptive relaxation was observed in gastric body, and tonic contraction was observed in LES. On the other hand, the occurrence of the LES contraction was consistent with gastric periodic motor activity in the interdigestive state. It was found that such a contractile pattern of LES and gastric body prevented the reflux of gastric juice from stomach to esophagus in the digestive and interdigestive state. 2. In the postprandial period, the tonic contraction of LES was inhibited by the venous injection of atropine sulfate. Mechanisms of digestive contractile activity of LES were mainly regulated by cholinergic nerve. 3. After SPV and TV, the motility of LES and gastric body was disturbed. Namely, receptive relaxation disappeared, and tonic contraction was observed in gastric body, but the relaxation of the LES was found after ingestion. These findings suggested that discordant LES contractiles with the contraction of the gastric body after vagotomy produced a gastroesophageal reflux. 4. After TV + gastrectomy, the synchronous motor activity of the LES, remnant stomach and duodenum disappeared. Namely, it was thought that the reflux esophagitis occurred after gastric surgery with vagotomy and lymphnodes dissection was caused by the duodenal juice reflux to remnant stomach and esophagus.

[One-month subacute intravenous toxicity study of cefodizime sodium in rats].

One-month subacute intravenous toxicity study of cefodizime sodium (THR-221) in rats was carried out with dose levels of 2000, 1000 and 500 mg/kg/day. Sixteen males and 16 females were used per group (including the control group). THR-221 caused neither death nor change in general conditions at any dose level throughout the study, except that decreased spontaneous activity appeared only transiently in a part of the animals given 2000 mg/kg/day. Increases in water intake were observed in all compound groups, and transient decreases in food consumption were seen at an early stage of the administration period. However, the compound did not affect the body weight at any dose level. In urinalysis, the urine sediments in all THR-221 groups contained an increased number of epithelial cells as compared with the controls. At autopsy, dilation of the cecum was observed in all THR-221 groups, and in a part of the rats with this change, red spots or reddening of the serous membrane of the organ also appeared. Light microscopy revealed brown granules in the epithelium of renal tubules in all compound groups (with dose-dependent incidences) and congestion or hemorrhage in the cecum in some compound-treated animals. Electron microscopy on the kidney showed small bodies (considered to be lysosomes) in the tubular epithelium in all compound groups. No other changes related to THR-221 were observed. From the present results that no marked toxic signs were seen at any dose level, the toxicologically non-effective dose of THR-221 for rats of both sexes is considered to be more than 2000 mg/kg/day.

[Spinal intramedullary tumor with exophytic growth].

We experienced 22 cases of spinal intramedullary tumor, in which 5 cases were accompanied with exophytic growth. We discussed their clinical and radiological features, and therapeutic problems. Concerning the locations of tumors with exophytic growth, the most common site was conus medullaris, accounting for 60% (3 cases). Histopathological findings were astrocytoma in two cases, and in one case, mixed glioma, ependymoma and hemangioblastoma. In neurological observation, the most common initial symptom was back pain and lumbago, suggesting root pain. No neurological features distinguishable from those of extramedullary tumors were presented. In radiological examination, myelography and CT myelography were very helpful for diagnosis. Myelography and CT myelography showed extramedullary mass, shift and deformity of spinal cord that was not serious as compared with the size of extramedullary mass, and showed the portion where the spinal cord was swollen. Good outcomes were obtained in a case with total removal, and two cases with subtotal and partial removal that were managed with additional irradiation and chemotherapy. However recurrence and intracranial seeding made prognosis poor in two cases where total removal was impossible. We thought that postoperative careful follow-up was necessary not only to detect recurrence but also to detect intracranial seeding in the cases of spinal intramedullary tumor with exophytic growth.

Synergistic enhancement of the antitumor activity of recombinant human TNF-alpha by recombinant human IFN-gamma.

The effect of recombinant human interferon-gamma (rHu-IFN-gamma) on the antitumor activity of recombinant human tumor necrosis factor (rHu-TNF-alpha) was examined in vitro and in vivo. rHu-IFN-gamma enhanced both cytostatic and cytocidal activity of rHu-TNF-alpha against most rHu-TNF-alpha-sensitive tumor cells in vitro. However, there was no correlation between the degree of enhancement by rHu-IFN-gamma and that of the susceptibility of tumor cells to rHu-TNF-alpha. The enhancing effect of rHu-IFN-gamma was most marked when tumor cells were treated with rHu-IFN-gamma either for 1 day before treatment with rHu-TNF-alpha or for the first day of the exposure to rHu-TNF-alpha. A marked enhancing effect of rHu-IFN-gamma was also observed in the in vivo antitumor activity of rHu-TNF-alpha against HMV-2 melanoma. A combined treatment with rHu-TNF-alpha and rHu-IFN-gamma in a patient with papillary adenocarcinomas was shown to be much more effective than treatment with rHu-TNF-alpha alone. These results suggest that combined treatment with both agents will have better results in clinical trials.

[A case of rectal carcinoma that disappeared following preoperative chemotherapy with an FT-207 suppository--confirmed by a thorough histologic examination of the resected specimen].

Presented is the case of a patient, a 56-year-old female, who had complained of bloody stool and constipation. A barium enema and endoscopic examination revealed a tumor (Type 2) with a crater surrounded by a thick embankment, extending from the anterior wall to the left wall of the lower rectum. Biopsy specimens of the tumor disclosed a well-differentiated adenocarcinoma. A FT-207 suppository (1500 mg/day) was administered preoperatively for 50 days (total dose 75 g). On February 16, 1987, the patient underwent an abdominoperineal excision. The resected specimen took on the appearance of a chronic ulcer with an irregular depression, measuring 2.0 x 3.0 cm in size, in the lower rectum. The histology of the lesion also indicated a chronic ulcer, the base of which consisted of fibrosis covered with regenerative mucosa. No cancer cells or nests were demonstrated even serial tissue sections. As far as the rectal carcinoma is concerned, there has been no reported case of its disappearance by preoperative chemotherapy. The above results suggest that preoperative adjuvant chemotherapy can be quite effective against an advanced rectal carcinoma.

[Radiological diagnosis of chronic spinal cord compressive lesion at thoraco-lumbar junction].

Radiological findings in five cases with chronic spinal cord compressive lesion at thoraco-lumbar junction were reported. Three cases had spondylosis and two cases had ossification of yellow ligament (OYL). The levels of the lesions were T12/L1 in three cases and T11/12 in two cases. Two out of three spondylotic patients had also OYL at the same level. The five cases consisted of three men and two women. The ages ranged from 42 to 60 years old with a mean age of 53 years old. Neurologically, every patient showed flaccid paresis and sensory disturbance of the legs. Two cases had sensory disturbance of stocking type. The intervals from the onset of the symptoms to the final diagnosis were 6 months, 7 years, 8 years, 11 years and 12 years. Myelography showed anterior spinal cord compression by bony spur in spondylotic patients, and posterior compression by OYL in other cases. Myelography in flexion posture disclosed the cord compression by bony spur more clearly in two out of three spondylotic patients. Delayed CT-myelography showed intramedullary filling of contrast material in two cases, which indicated degenerative change or microcavitation due to long term compression of the spinal cord. MRI was taken in three spondylotic patients and could directly show compression of the spinal cord. Difficulty in detecting abnormality at thoraco-lumbar junction on plain roentgenogram, and similarity of the symptoms to peripheral nerve disease often lead to a delay in diagnosis. The significance of dynamic myelography and delayed CT-myelography when dealing with such a lesion was discussed here.(ABSTRACT TRUNCATED AT 250 WORDS)

[Spinal epidural tumors without destructive bone change: clinical and radiological studies].

CT in 61 patients having spinal epidural tumors were reviewed. Of these patients twelve had an epidural mass lacking of destructive bone change. Histological examination of these 12 cases revealed that usual carcinomatous tumors are relatively less common but sarcomatous tumors and malignant hematologic neoplasms are more likely to form an epidural mass without bone destruction. The tumors preponderance thoracic spine and extended usually more than three spinal levels along spinal axis. In all but one tumor masses were found in the intervertebral foramen and the paraspinal region in addition to the spinal canal. Contrast enhanced CT was particularly effective to delineate tumor location and extension. Ring enhancement of the dural sac which constricted by epidural masses was considered pathognomonic of epidural tumors.

[Lymphatic flow of the remnant stomach].

Aiming at establishing an appropriate lymph node dissection for carcinoma of the remnant stomach and of the lower esophageal carcinoma in the status post-gastrectomy, lymphatic flow was investigated clinically as well as experimentally. Nineteen cases of carcinoma of the remnant stomach and 8 cases of esophageal carcinoma after partial gastrectomy were studied. Lymph node metastasis of the remnant stomach carcinoma were more frequently seen at perigastrium, splenic hilum, and along splenic artery. Those further extended to para-aortic and diaphragmatic nodes. Three cases of lower esophageal carcinoma after gastrectomy had massive nodal involvement at perigastrium, as well as intra-thoracic lymph nodes. Experimentally 5-Fu emulsion was injected submucosally under endoscope in 25 dogs and subserosally in 6 rabbits. 5-Fu contents in lymph nodes were measured 30 minutes after injection. The most prominent difference in lymphatic flow from the remnant stomach was increase in ascending flow into intrathoracic lymph nodes through para-aorta. This increment was seen irrespective of Billroth I or II anastomosis. On the contrary, descending lymphatic flow from the lower esophagus into the intra-abdominal lymph nodes was not disturbed by gastrectomy. Cardiac lymph node dissection in rabbits accelerated ascending flow. Those results would indicate the necessity of complete block of ascending flow in cases of the remnant stomach carcinoma and of intra-abdominal lymph node dissection in those of the lower esophageal carcinoma after gastrectomy.
