Familial case of permanent form of junctional reciprocating tachycardia: possible role of the HLA system.

The permanent form of junctional reciprocating tachycardia (PJRT) is a very rare arrhythmia with the following clinical and electrocardiographic findings: (1) it occurs predominantly in infants and children; (2) it is almost incessant and refractory to pharmacological therapy; (3) the onset is commonly related to a critical shortening of the P-P cycle length without P-R prolongation; (4) during tachycardia the ECG shows an R-P longer than P-R interval, with a negative P wave in leads II, III, aVF. Recently, the anatomic and electrophysiological characteristics underlying PJRT have been identified: there is an accessory pathway of working myocardium with decremental properties, located in the posterior pyramidal space. A case of familial PJRT is reported: the arrhythmia has been documented in a 72-year-old female and in her 16-year-old grandson. Several triggering tachycardia mechanisms have been observed. Tachycardia was almost incessant and the heart rates were 115 and 135 beats/min, respectively. Typing according to the HLA system, performed in all members of the family, demonstrated the Bw41 antigen in both our patients as well as in the boy's paternal uncle. This is the first documented familial case of PJRT, but the possible significance and correlation with the Bw41 antigen should be further investigated.

Effects of betamethasone on theophylline disposition in man. Are saliva values predictive of serum theophylline levels?

The pharmacokinetic interaction between betamethasone (5 mg/day for 5 days) and theophylline (3.35 mg/kg orally before and after betamethasone treatment) was studied in 13 female patients evaluating saliva and serum theophylline concentrations by means of an enzyme multiplied immunoassay technique. Betamethasone treatment did not significantly change saliva and serum theophylline levels. As regards the potential usefulness of saliva theophylline levels in predicting the concomitant serum concentrations of the bronchodilator, in spite of the significant correlation found in our study (r = 0.758; P less than 0.001), we think that, using individual saliva values, there is a poor predictive value for serum theophylline concentrations.

Improvement of mucociliary transport in smokers by mucolytics.

The purpose of the present study was to compare the effects of two mucolytic drugs with different mechanism of action on mucociliary transport (MCT). N-acetylcysteine (NAC-600 mg/day) and ambroxol (AMB-90 mg/day) were administered according to a double-blind cross-over scheme to 12 heavy smokers suffering from hypersecretory bronchitis and homogeneous reduction of the MCT. Placebo of both treatments was administered during an interval of ten days between the administrations of NAC and AMB. The entire treatment period was 30 days. The data were analyzed according to ANOVA for the two-period cross-over clinical trial. The results indicate that: NAC and AMB, administered both before and after placebo, produce a significant increase in MCT, NAC showed a slightly greater efficacy than AMB, but the differences are not statistically significant. The overall efficacy of NAC and AMB is consistently greater than that of placebo. The sequence of administration of the drugs does not influence their effect.
