[Monthly dynamics of the epidemic process in viral hepatitis B].

Monthly fluctuations in the number of registered cases of acute viral hepatitis B and HBsAg carriership have been studied. The study has revealed that, similarly to other infectious diseases, viral hepatitis is characterized by monthly fluctuations in the intensity of the epidemic process. Such fluctuations are characteristic of all known clinical forms of this infection; they are determined by the specific pathogenetic features of the process and by the ways of the transfer of the virus. The vernal rise of the infection is explained by activation of the manifest and asymptomatic chronic variants of the infectious process and, as the consequence of a rise in the number of asymptomatic cases, by a higher incidence rate of post-transfusion hepatitis infection. The autumnal rise of the infection results from the action of natural factors contributing to the transfer of the virus. Both rises are interrelated and interdependent. The seasonal fluctuations of the epidemic process should be taken into consideration when planning and implementing prophylactic and epidemic-control measures.

[Effect of elemental nutrition and fasting on the resistance of the liver to ischemia].

The efficiency of the element nutrition with glucose and alvezin and fasting as means of the antiischemic defense was studied in 50 experiments in perfusion of the isolated liver on rabbits previously subjected to 18-20 minute heat ischemia. The parenteral administration of glucose and graded fasting were shown to be most effective for the antiischemic defense of the liver.

[Optimal anion composition of the perfusate for preservation of the liver].

The search for optimal concentrations of fumarate, malate, acetate, lactate, asparginate and sulphate in the perfusate was performed in 187 experiments with 2 h perfusion of the isolated rabbit liver at different temperature regimens of conservation. Nonlinear relation between the optimal anion composition of the perfusate and temperature of the liver conservation was found. The critical point in changes of the optimal concentrations of anions in the perfusate was shown to be 10-12 degrees C.

[Optimal cationic composition of perfusates in relation to temperature during preservation of the liver].

Search for the optimal concentrations of potassium, sodium, magnesium, calcium and H+ under different temperature conditions of preservation was performed in 193 experiments with the 2 h perfusion of the isolated rabbit liver. A nonlinear relation was established between the optimal cationic content of the perfusate and the liver preservation temperature. The sharpest changes to the optimal concentration of cations in the perfusate were found to start when the liver preservation temperature had dropped lower than 10-12 degrees C.

[Effects of complex therapy with emoxipin on the course of diabetic retinopathy].

The efficacy of 1% emoxipin solution (eye drops) in the nonproliferative stage of diabetic retinopathy was studied. The inhibition of lipid peroxidation and an enhancement of antioxidizing guard were observed both after the therapy by the above preparation and 3 months later. It was concluded that emoxipin was effective for leveling the biochemical violations in the "lipid peroxidation--antioxidants" system of lachrymal fluid in patients with the nonproliferative stage of diabetic retinopathy. The prescription of emoxipin eye drops can enhance the efficacy of therapy at the early stages of the disease.

[Increase in the rate of anaerobic glycolysis as an index of hypoxia in hemorrhagic shock].

A rate of anaerobic glycolysis and the activity of several enzymes involved were studied in brain, heart, liver and small intestine of rabbits in hemorrhagic shock. In hemorrhagic shock at 45 mm of m. c. the rate of glycolysis was increased in liver tissue and intestine, at 35 mm of m.c.--in liver tissue, intestine and brain, at 26 mm of m.c.--in all the tissues studied. The enzymatic activity did not alter markedly. The possible mechanism of the increased rate of anaerobic glycolysis is discussed.

[Effectiveness of a series of pharmacological substances in liver ischemia].

Antiischemic activity of alpha-tocopherol, prednisolone, T-407 and T-408 on functions of liver tissue was studied after 90 min contraction of hepatoduodenal ligament in 61 animals. All the preparations studied exhibited the antiischemic protection of liver tissue; the most effective drugs proved to be T-408 and prednisolone.

[Bladder neck sclerosis in children].

Nowadays there are contradictory opinions of the prevalence of urinary cervicovesical sclerosis in children and therefore the high incidence of the disease is explained by some investigators as hyperdiagnosis. This article presents the results of examination performed in 15 children with urinary cervicovesical sclerosis who were referred to hospital for other diagnoses. Morphological examination, including electron microscopy, failed to demonstrate the congenital origin of the disease. The authors suspect that the development of cervicovesical sclerosis in children was associated with some secondary reasons. Clinical signs of the disease are characteristic of intravesical obstruction confirmed by uroflow, profilo- and cystomanometries. The findings failed to confirm the dysfunction of the detrusor. In general, cystoscopic examination documented a heterogeneous, more commonly mosaic picture which was not infrequent in the presence of chronic inflammation. Trabecularism of the mucosa was documented in all examinees, false diverticula in four of them. When the irrigation liquid passed the neck of the urinary bladder the latter opened but the stricture was not continuous. X-ray examination revealed the rigidity, compression and high position of lower cervical segments. Cervicovesical sclerosis was treated by transurethral [correction of transureteral] electroresection with a subsequent multiple urodynamic study which proved the correctness in the choice of the treatment policy and its beneficial impact. In case of a "dry urinary bladder', the authors first made operative corrections for the recovery of the urine passage, followed by transurethral [correction of transureteral] electroresection of the neck of the urinary bladder.
