ASPAN and the organizational EBP vision: synergistic opportunities for perianesthesia nurses.

Health care organizations and the American Society of PeriAnesthesia Nurses (ASPAN) both have set a goal to promote quality by providing evidence-based nursing care. This shared goal provides a synergistic opportunity for ASPAN members to align with the organizational vision by incorporating evidence-based standards into practice and by answering important perianesthesia clinical questions. The purpose of this article is to discuss the forces that foster the health care organization's goals to implement evidence-based practice within their setting and to consider strategies for how ASPAN and ASPAN members can combine efforts with the health care organizational vision.

Evidence-based practice: a practical approach to implementation.

Organizations often do not have processes in place to support nurses through a systematic approach for developing and evaluating nursing interventions, protocols, critical pathways, and policies that are derived from scientific evidence. The development of a framework to guide inquiry will have a positive impact on patients. This process may foster a higher level of professional engagement by nurses that may, in the long-term, help improve nurse retention and recruitment. The authors discuss a nursing evidence-based practice model and guidelines that were developed by a team of hospital and academic nurse leaders and is practical and easy to use. This model has been successfully implemented across the department of nursing as a strategic initiative. Results of the implementation have shown that staff nurses can effectively use this model with the help of knowledgeable mentors.

Barriers to research use in the PACU.

To advance the growth of perianesthesia research and promote research-guided patient care, PACU nurses must use research findings in practice. The purpose of this survey was to explore the perceived barriers to research use in a specific PACU setting so that strategies could be planned to improve research use. The greatest perceived barriers related to attributes of the organization. This is an area that nurse leaders can affect by building the infrastructure needed for nurses to develop skill and knowledge related to research use.

Clinical evaluation of the ASPAN Pain and Comfort Clinical Guideline.

The ASPAN Pain and Comfort Clinical Guideline was created because of the urgent need for a standardized evidence-based approach to the management of patients' pain and comfort in all perianesthesia settings. The purpose of the research presented here was to test the content of the ASPAN Pain and Comfort Clinical Guideline, which included the domains of assessment, intervention, and outcomes. Each domain was rated on clarity, usability, and feasibility using a Likert scale, which ranged from 1 (strongly disagree) to 4 (strongly agree). A survey design was used with a convenience sample of 215 perianesthesia nurses. The results of the study demonstrated a mean score of clarity, usability, and feasibility in all perianesthesia settings ranging from 3.54 (SD) to 3.80 (SD). There were no differences in the clarity, usability, or feasibility of the guideline between perianesthesia settings. The results of this study support that the ASPAN Pain and Comfort Clinical Guideline has practical utility for perianesthesia nurses in all settings. Use of this guideline in perianesthesia settings will standardize pain and comfort management and has the potential to positively impact pain and comfort in perianesthesia patients.
