[Anticancer chemotherapy containing cisplatin in patients with lung cancer and kidney function].

To carry out safe anticancer chemotherapy one should consider the kidney function. Insufficiency of that main organ responsible for drugs excretion, caused either by neoplastic disease or by chemotherapy, can diminish the possibility or even make impossible of carrying out a complete treatment cycle. The aim of our work was to evaluate the kidney function in patients with lung cancer during anticancer chemotherapy containing cisplatin. The tubular function was studied by estimation the activity of N-acetyl-beta-D-glucosaminidase in urine and glomerular function was studied by estimation the concentration of creatinine in urine, urea, uric acid and electrolytes in plasma. The observations have recorded that neoplastic process, as well as chemotherapy impaired the tubular function. It has showed that it does exist a necessity of detailed estimation of kidney excretory function before, during and after the end of anticancer therapy. Determination of NAG activity in urine may be helpful for the recognition of the patients at high nephrotoxicity risk, who need special care.

[Evaluation of vascular endothelial growth factor (VEGF) in neoplastic and tuberculosis effusions--preliminary results].

Angiogenesis is an essential process for tumor progression and VEGF is thought to be a critical factor for this process. VEGF is also known as a strong capillary permeability inducer, what can be important for pleural effusion development. The aim of the study was the comparative analysis of VEGF concentration in pleural effusions collected from 31 patients with various type of cancer, 8 patients with tuberculosis, 5 patients with transudates. Additionally VEGF concentrations in 11 serum specimens from patients with neoplastic disease was evaluated. VEGF concentrations was determined using a sandwich enzyme-linked immunoadsorbent assay. The VEGF level in pleural transudates as well as tuberculous effusions was significantly lower than in malignant fluid. The were no differences in VEGF level in malignant pleural fluid of different origin. The correlation between VEGF concentrations in malignant pleural fluid and sera of individual patients was not found. Our results indicate that VEGF might play an important role in accumulation of pleural fluid especially that of malignant origin. VEGF level in pleural fluid differs significantly from cancer to benign group of patients what can be important for differential diagnosis of plural effusions origin.

[Tumor markers TPA and Cyfra 21.1 in patients with non-small cell lung cancer after surgery and chemotherapy].

In 27 patients with operable non-small cell lung cancer (NSCLC) submitted to radical surgery followed by 3 cycles of chemotherapy (cht) serum concentrations of Cyfra 21.1 and TPA were studied. The measurements were performed before and 14 days after surgery, before each cht and every 60th day after cht was completed, for 2 years. Seven patients died during the follow up. There was no significant correlation between preoperative cyfra 21.1 and TPA serum concentrations and stage of diseases or histologic types of NSCLC. Initial concentrations of the two markers had no prognostic meaning. A significant decrease of 2 markers was observed after surgery in the whole group and in patients with therapy success. While adjuvant cht did not influence significantly serum concentrations of the markers, we showed a significant elevation of 2 markers about 4 months before death. It seems that establishing of values of Cyfra 21.1 and TPA in the patient's follow up may be useful in recognition of tumour relapse.

[Soluble interleukin 2 receptor (SIL-2R) in bronchoalveolar lavage fluid (BALF) of patients with sarcoidosis and atopic bronchial asthma].

The aim of the study was assessment of sIL-2R in BALF in asthma and sarcoidosis patients. Forty three patients were enrolled in this study. Twenty eight with sarcoidosis (S) in different stages (I, II, III) and activity of disease: active (AS) or nonactive (NAS) and fifteen with atopic bronchial asthma (AO). BALF sIL-2R levels were measured in both groups. The level of sIL-2R was similar in sarcoidosis patients independently to activity of disease. Significantly lower level of sII-2R in BALF was observed in patients with AO in comparison to S, SII and NAS (p < 0.005). We didn't find significant differences in sIL-2R levels between AO and SI, SIII, AS.

[Increase of skin reactivity to histamine in patients after lung cancer surgery].

In 16 patients with lung cancer submitted to radical surgery skin reactivity was studied. Skin prick test was done in all patients with histamine concentration--10 mg/ml and negative control before and after surgery. The longest wheal diameter was measured after 15 min and 30 min. The wheal size was determined as a difference between histamine wheal diameter and negative control diameter (before surgery--after 15 min. 1.53 +/- 1.43 mm., after 30 min. 2.31 +/- 1.77 mm. vs after surgery--after 15 min 3.44 +/- 1.06; p < 0.001, after 30 min 4.25 +/- 1.20; p < 0.001. In patients with lung cancer the significant increase in histamine wheal size was observed as compared with its size before surgery. The lower expression of histamine wheal receptors (mainly H1 skin receptors) is probably caused by cancer suppression. The expression of these receptors increased after tumor excision.

[Wegener's granulomatosis--a disease of many faces].

We describe a case of Wegener's granulomatosis. The first symptoms included severe headache subsiding only after administration of dexamethasone. Despite a great number of diagnostic tests involving CT and MR of the head, the cause of the headache remained unknown. Because a chest x-ray revealed a nodule in the right lung, the patient was sent to our Clinic. Reevaluation of CT and MR pointed to a massive ethmoid sinusitis. 7-days' course of antibiotics and corticosteroids induced remission of the lung nodule. Several diagnoses were made: neoplasm, bacterial ethmoid sinusitis, trigeminal neuritis, thrombotic cavernous sinusitis and tuberculosis. Results of the ethmoid sinus biopsy together with a high c-ANCA concentration gave the correct diagnosis.

Apoptotic markers p53, Bcl-2 and Bax in primary lung cancer.

BACKGROUND: Apoptosis is the fundamental process necessary for eliminating damaged or unwanted cells. Alterations in the apoptotic pathway appear to be key events in cancer development and progression. The aim of the study was to determine the p53, Bcl-2 and Bax expressions in lung cancer, taking into account histological heterogeneity and the adjacent bronchial resection margin. MATERIALS AND METHODS: Tissue specimens from 60 histopathologically verified lung cancer specimens and 12 bronchial stumps were evaluated. The presence of the studied markers was revealed by immunocytochemistry on paraffin-embedded tissue. RESULTS: The percentage of p53- and Bax-positive lung cancers was comparable (51.6% for both proteins), while Bcl-2 immunoreactivity was observed in fewer (31.6%) cases. There was no p53 accumulation in bronchial stumps, while Bcl-2 and Bax staining formed a repeatable specific pattern in bronchial epithelium. The differences in apoptotic marker expression between non-small cell lung cancer (NSCLC) and small cell lung cancer (SCLC) were revealed, especially regarding p53 and Bax expression (60% vs. 10%, p = 0.005 and 58% vs. 20%, p = 0.04, respectively). Taking into account the histological structure of NSCLC, Bax expression appeared to be more frequent in adenocarcinoma than in squamous cell lung cancers (88% vs. 42%, p = 0.004). No interrelationship between the studied proteins in lung cancer tissue was revealed. CONCLUSION: The expression of p53, Bcl-2 and Bax was altered in lung cancer tissue compared to histologically normal bronchial epithelium. The difference between apoptotic marker expression in NSCLC and SCLC could reflect the different pathogenesis of these two lung pathologies.
