[The value of the guaiac test in the screening of colorectal tumors].

The sensitivity, specificity and positive predictive value of fecal blood testing are too poor to allow its general use as a screening test for colorectal cancers. Even the predictable qualitative improvement of these tests will be of no avail with regard to non-tumoral sources of bleeding or the non-hemorrhagic tumoral sources of bleeding or the non-hemorrhagic tumours which are frequent in the early stages of colorectal cancer. It is therefore essential to define high-risk groups, to pay particular attention even to minor symptoms, and to perform the necessary endoscopic or radiological diagnostic tests.

[Importance of drug-induced ulceration in endoscopic lesions of the esophagus].

Drug-induced ulcers of the oesophagus represent a rare but probably under-recorded complication. In a series of 5900 endoscopies performed in 32 months, oesophageal ulcers were seen in 4 cases following the intake of doxycycline, and in one case after ingestion of pinaverium bromide and a bulk laxative respectively. Oesophageal ulcers were seen mainly in young patients without underlying oesophageal disease, presenting with chest pain and odynophagia. The most common site of involvement was at the aortic notch in the middle third of the oesophagus. The course was quickly favorable within 5-10 days after the drug was discontinued, but transient complete abstention from oral intake was required in some cases. Ulceration is thought to be secondary to drug stasis and local cytotoxic effects. Oesophageal ulcers can be prevented simply by recommending intake of the drug with sufficient water in the upright position at least two hours before retiring.

[Peptic lesions of the upper digestive tract and drugs. Prospective study].

The association between drugs and diseases of the gastrointestinal tract is well known but has always been evaluated qualitatively and not quantitatively. To study the importance of this association, a prospective study was undertaken at the University of Geneva Hospital in 456 subjects. A statistically significant (p less than 0.05) correlation was found in the following associations: aspirin and multiple gastric ulcers, aspirin and severe erosive gastritis; non-steroidal antiinflammatory drugs and multiple duodenal ulcers, non-steroidal antiinflammatory drugs and mild erosive gastritis. On the other hand, no correlation was found between any of the observed gastrointestinal diseases and prednisone, alcohol or tobacco.

Pseudotumoral enterocolitis and massive eosinophilia.

We report a seven year history of a 23 year old woman born in the Antilles, with pseudotumoral enterocolitis and massive eosinophilia. In 1973 she developed a haemorrhagic colitis with massive peripheral eosinophilia of up to 60000/mm3. Medical treatment, mainly corticosteroids, failed to control the disease. The patient temporarily improved after colectomy and remained in remission for two years. In 1978 the disease recurred in the rectum and small intestine with formation of tumour-like granulomata and massive infiltration by eosinophils, unresponsive to corticosteroids. Intestinal blood and protein loss continued until death seven years after onset. In spite of exhaustive investigation, no parasites, allergens, or other aetiological agents could be found. As only the gut was infiltrated, the hypereosinophilic syndrome could be excluded. The enterocolitis here described does not correspond to an eosinophilic gastroenteritis, nor to other known inflammatory bowel diseases and to our knowledge has not been reported previously.

Significance of duodenal disaccharidases. A comparative study of duodenal and jejunal values.

Endoscopic biopsies at the inferior duodenal flexure from 36 patients were assayed for disaccharidases. Comparative studies in 8 patients were carried out from this location and from the duodenojejunal flexure. 16 patients with lactase values under 20 U/g protein and normal histology also underwent a lactose tolerance test. Evidence is presented that the easily performed duodenal biopsies give the same information as those from the angle of Treitz, at a level about 40% under the latter. Tolerance to lactose was in close relation with the lactase level in 16 patients, the limit being at about 5 U/g protein.

[Endoscopic polypectomy in the early diagnosis of colonic carcinoma].

281 colonic polypectomies, some of which were incomplete, were performed in 202 patients, revealing the presence of 19 (6.8%) non-adenomatous polyps, 218 (77.6%) benign adenomas, 23 (8.2%) adenomas with superficial carcinoma, 12 (4.3%) adenomas with invasive carcinoma and 9 (3.2%) polypoid carcinomas. Malignant adenomas are found in patients of 30 and over, being 3.5 times more frequent when the polyp has villous patterns. Their incidence increases with the diameter of the poly. 21 patients (12.5%) were submitted to further surgery on the basis of endoscopic or histological data. The significance of endoscopic polypectomy is discussed.

Adenoma↗

[Intestinal lactase and milk consumption before and after gastrectomy].

Intestinal lactase assays before and after a Billroth II procedure for chronic duodenal ulcer did not reveal a significant difference. Postoperative lactose intolerance is due to the loss of pyloric function, causing entrance of lactose into the jejunum at a rate exceeding the hydrolytic capacity of the mucosa if the lactase level is low.

[Toxic reactions to gold salts with severe enterocolitis in a patient with rheumatoid arthritis].

A case of severe enterocolitis which occurred on initiation of chrysotherapy for rheumatoid arthritis is reported. The patient, a male aged 50 years, suffered from dramatic diarrhea for several weeks. Biopsy specimens from the stomach, ileum and colon revealed signs of inflammation. Later, general toxicodermia, mouth ulcers, blood eosinophilia and asymptomatic proteinuria were observed. After several months all the side effects disappeared. The toxicodermia was the most persistent.
