Hypercoagulability after immunotherapy with Corynebacterium parvum in man.

The effect of an immunotherapy with Corynebacterium parvum on the blood coagulation system was investigated in a randomized trial of 18 patients with metastatic breast cancer. All patients received cytostatic therapy. Additionally, C. parvum was given intravenously on day 15 of the cytostatic cycle (group I) or on day 1 (group II) or not at all (group III). Fibrinopeptide A increased within 2 h after intravenous administration of C. parvum in groups I and II and normalized after 24 h (p less than 0.05). Platelet counts decreased continuously in all treatment groups (p less than 0.05). Prothrombin time, fibrinolytic concentration, factor VIII:C and factor VIIIR:Ag were not affected. The fibrinolytic activity showed a slight but not statistically significant increase after intravenous administration of C. parvum. The data suggest that plasma hypercoagulability is induced or enhanced in man even after small intravenous doses of C. parvum.

Lack of effect of cimetidine on action of phenprocoumon.

In patients on oral warfarin, nicoumalone and phenindione an increase in the anticoagulant effect has been described during concomitant treatment with cimetidine. Therefore the effect of cimetidine on the steady state dynamics of phenprocoumon has been investigated in ten outpatients. No change in the anticoagulant effect of phenprocoumon was observed during or after two weeks on cimetidine, as measured by the thrombotest coagulation method, prothrombin time, fibrinopeptide A concentration and plasma phenprocoumon level. The data show that cimetidine does not interact with the metabolism of phenprocoumon in contrast to warfarin. Thus, phenprocoumon maintenance therapy when combined with concomitant cimetidine treatment can be considered not to carry an increased risk of haemorrhagic complications.

Gonadotropin therapy of female infertility. Analysis of results in 416 cases.

This report is based on 416 infertile female patients who were treated for 1,033 cycles with gonadotropins. 28.6% of the patients conceived after hMG/hCG treatment in 79.8% of these pregnancies, healthy children were born. Spontaneous abortion or premature birth occurred in 20.2% of the cases. The twin rate was 28.6%, the triplet rate 5.5%. Most of the abortions occurred in the first trimester (52.2%). No malformations were seen. The pregnancy rate showed striking differences in the various diagnostic groups: hypogonadotropic amenorrhea 44.4%, normogonadotropic amenorrhea 50%, anovulatory cycles 22%, corpus luteum insufficiency 14.8%. The abortion rates for these four groups were as follows: hypogonadotropic amenorrhea 25%, normogonadotropic amenorrhea 14.7%, anovulatory cycles 4.8%, corpus luteum insufficiency 36.3%. A detailed analysis of the treatment cycles is given for the four groups: the number of ampoules of hMG/hCG increased from 21.4 ampoules in patients with corpus luteum insufficiency to 47.7 ampoules in patients with hypogonadotropic amenorrhea. The inactive phase increased from 5.6 days in patients with corpus luteum insufficiency to 8.5 days in patients with hypogonadotropic amenorrhea. Estrogen values around the time of ovulation and in the corpus luteum phase were much lower in patients with spontaneous uterine bleedings. Hyperstimulation syndrome occurred less frequently in these patients. The percentage of pregnancies decreased in patients with corpus luteum insufficiency from 8.1% in the first treatment cycle to 4.8% in the following treatment cycles, whereas it increased from the first to the following cycles in the other diagnostic groups. Patients with anovulatory cycles and corpus luteum insufficiency respond differently to hMG/hCG treatment than patients with normogonadotropic amenorrhea. The inactive and active phase are important parameters for the evaluation of ovulation induction with hMG/hCG, hMG/hCG treatment is of little value in patients with corpus luteum insufficiency.

Urokinase therapy of subclavian-axillary vein thrombosis.

In 18 cases with primary subclavian-axillary vein thrombosis fibrinolytic therapy was performed with urokinase in combination with heparin. The thrombolytic efficacy clearly depended on the thrombus age and the dose of urokinase applied. Under treatment with a median initial maintenance dosage of urokinase of 1,000-2,000 IU/kg/h (loading dose 150,000-250,000 IU urokinase) in combination with heparin (15-17 U kg/h) in mine of 11 patients (82%) with recently developed (8 days or less) thrombosis, a nearly complete deobliteration of the venous system was observed. In the case with thrombosis of more than 10 days no alteration of the venous occlusions could be seen. Relevant side effects did not occur. Our results emphasize urokinase therapy of acute subclavian-axillary vein thrombosis and permit general inferences concerning the efficacy and the dosage requirements of the thrombolytic substance urokinase.

Measurement of fibrinopeptide A in patients treated with phenprocoumon.

Fibrinopeptide A (FPA), which is considered to be a quantitative indicator for the thrombin activity in vivo, was measured in 136 patients treated with phenprocoumon in order to obtain information on the effectiveness of the inhibition of the coagulation system. The results show a decrease of the FPA concentration in relation to the efficacy of the anticoagulant therapy as measured by the thrombotest coagulation method (p less than 0.01). However, elevated FPA was observed even under an effective oral anticoagulation. These data indicate that an increased thrombin activity cannot be completely prevented by oral anticoagulants in every patient. Combined measurement of FPA and the thrombotest coagulation methods might be used to detect patients with an elevated risk of recurrent thromboembolism despite treatment with phenprocoumon.

Different transport pathways of individual precursor proteins in mitochondria.

Transport of mitochondrial precursor proteins into mitochondria of Neurospora crassa was studied in a cell-free reconstituted system. Precursors were synthesized in a reticulocyte lysate programmed with Neurospora mRNA and transported into isolated mitochondria in the absence of protein synthesis. Uptake of the following precursors was investigated: apocytochrome c, ADP/ATP carrier and subunit 9 of the oligomycin-sensitive ATPase. Addition of high concentrations of unlabelled chemically prepared apocytochrome c (1-10 microM) inhibited the appearance in the mitochondrial of labelled cytochrome c synthesized in vitro because the unlabelled protein dilutes the labelled one and because the translocation system has a limited capacity [apparent V is 1-3 pmol X min-1 X (mg mitochondrial protein)-1]. Concentrations of added apocytochrome c exceeding the concentrations of precursor proteins synthesized in vitro by a factor of about 10(4) did not inhibit the transfer of ADP/ATP carrier or ATPase subunit 9 into mitochondria. Carbonylcyanide m-chlorophenylhydrazone, an uncoupler of oxidate phosphorylation, inhibited transfer in vitro of ADP/ATP carrier and of ATPase subunit 9, but not of cytochrome c. These findings suggest that cytochrome c and the other two proteins have different import pathways into mitochondria. It can be inferred from the data presented that different 'receptors' on the mitochondria. It can be inferred from the data presented that different 'receptors' on the mitochondrial surface mediate the specific recognition of precursor proteins by mitochondria by mitochondria as a first step in the transport process.

[Fibrinolytic treatment of severe thrombosis of the basilar artery (author's transl)].

Thrombosis of the basilar artery occurred in a 27-year-old woman after physical exercise (bowling). She became deeply unconscious without change so that four days after onset urokinase was administered. The neurological status than markedly improved, with partial regression of the paresis and the cranial nerve deficits, and she became responsive again. Angiography after four-day urokinase administration demonstrated complete recanalisation of the basilar artery. Subsequent rehabilitative measures led to almost complete disappearance of all symptoms. Later neurological tests revealed merely very discrete left hemiparesis, mainly of the arm.

Induction of drug metabolizing enzymes by sulfinpyrazone.

A previous interaction study of sulfinpyrazone (Anturano) suggested that it induced microsomal drug metabolizing enzymes in the liver. To verify this finding the effect of sulfinpyrazone 800 mg per day for four weeks was investigated in ten healthy volunteers. Both the therapeutic actions of sulfinpyrazone, the uricosuric and the antiaggregating effects, were demonstrated (p less than 0.05). The influence on the microsomal drug metabolizing system in the liver was demonstrated by an increase in serum-gamma-glutamyl transpeptidase from 15.1 to 23.3 U/l (p greater than 0.05), a significant increase in the urinary excretion of d-glucaric acid (29.6 to 77.9 microMol/24 h, p less than 0.05) and an increase in antipyrine clearance from 50.3 ml/min to 83.9 ml/min (p less than 0.05). The possibility of enhancement of drug metabolism during treatment with sulfinpyrazone in combination with other drugs should be kept in mind.

[Social integrations related to institutionalized and noninstitutionalized forms of care of elderly people].

This is a report about a research project analyzing costs and effectiveness of institutionalized and ambulatory care of older people. For this analysis economic factors as well as social factors are considered. On the sociological part multiple objective conditions (e.g. state health, financial situation and family relations) were correlated with subjective factors as feelings of well-being and various personal attitudes. To this purpose approximately 900 elderly persons (3 samples: 1. inhabitants of homes of the aged, 2. ambulant served (meals on wheels, home help and community nurses), 3. random sample of persons living at their own home. The 3 samples were taken in an urban, semiurban and rural area. Some remarkable differences between the 3 samples could be shown referring to the general physical and mental status and rate of impairment, in the state of care, familiar situation, social integration and subjective attitude (e.g. demonstrating that the inhabitants of homes for the aged were less socially integrated and less satisfied.

[Ancient tattooing from today's point of view].

Both literary and arachaeological evidence indicates that, up to now, ancient tattoos can be traced with certainty in painting only among Thracians. A comparison with modern tattoos reveals differences of motivation and motifs, whereas localization, technique, and removal show similarities. The illustrations demonstrate some tattoos typical for Thracians on Greek vases.

Nitrogenase XII. Mössbauer studies of the MoFe protein from Clostridium pasteurianum W5.

We have studied the molybdenum-protein (MoFe protein) from Clostridium pasteurianum with Mössbauer spectroscopy in the temperature range from 1.5 to 200 K in magnetic fields up to 55 kG. Except for some small differences in the hyperfine parameters the results for the C. pasteurianum protein are essentially the same as those published previously for the protein from Azotobacter vinelandii, i.e. (30 +/- 2) Fe atoms partition into two identical cofactor centers M (each center most likely containing six Fe atoms and one Mo atom), four P-clusters (each center containing four Fe atoms), and one iron environment labeled S (about two Fe atoms per holoenzyme). We have analyzed the spectra of the cofactor centers in three distinct oxidation states, Formula: (see test). The diamagnetic (electronic spin S = 0) state MOX is attained by oxidation of the native, EPR-active (S = 3/2) state MN. The reduced state MR is observed in steady state under nitrogen fixing conditions; high-field Mössbauer studies show that the cofactor centers are paramagnetic (integer electronic spin S greater than or equal to 1) in the state MR. We have evaluated the complex high-field spectra resulting from the P-clusters in the oxidized state POX. The analysis shows that one iron site is characterized by a positive hyperfine coupling constant A0 while the other three sites have A0 less than 0. A slightly modified set of parameters also fits the high-field data of the MoFe protein from A. vinelandii. Finally, we will present a discussion summarizing our principle results obtained to date for the proteins from A. vinelandii and C. pasteurianum.

[Pharmacotherapeutical action on arteriosclerotic vascular diseases with benzarone (author's transl)].

We report the results of a pilot study to evaluate the therapeutical efficiency of Benzarone in a time span of four weeks in 11 male patients with intermittent claudication. Numerous experimental data indicate that there was a statistically significant increase of painless walking distance and a statistically significant decrease of plasma uric acid, cholesterol and triglycerides. Coagulation analyses furthermore show an increase of the factor VIII activity and the plasminogen levels. For the first time, positive results were shown with Benzarone in patients with intermittent claudication.
