[Primary adenocarcinoma of the bladder. Apropos of 10 cases. Retrospective study].

We report ten clinicopathological cases of primary adenocarcinoma of the urinary bladder. Histologic features were often reminiscent of those seen in digestive tumors: Lieberkühnian adenocarcinoma (4 cases), mucinous adenocarcinoma (3 cases) and signet ring cell adenocarcinoma with linitis plastica (1 case). Prognosis was poor in these tumors that were all invasive and had spread beyond the muscular layer: four patients died and six were critically ill when they were lost to follow-up. We emphasize the role played by glandular metaplasia of the vesical urothelium in the histogenesis of these tumors.

[Genitourinary malacoplakia. Apropos of 5 cases. A review of the literature].

The authors report 5 new cases of malakoplakia: 2 with kidney, 1 with prostatic and 2 with testicular involvement. The symptoms are not clinically specific, but pseudo-tumor forms are predominant. Four cases were treated by removal of the organ. Owing to a better understanding of its etiopathogenic mechanism, this disease can be treated by long-term antibiotic, cholinergic agonists and vitamin C therapy.

[Lymph node tuberculosis].

Ganglionic tuberculosis with a cervicofacial location is still frequent in Tunisia. The main problem posed is bacteriological. Over the last nine years, we have treated 110 patients. There is a clear male predominance. The preferential location is the jugulocarotid and upper spinal chain. The notions of contamination and a secondary location, particularly in the lungs, are rarely encountered. Extended cellulolymphadenectomy is the basis of surgical treatment, backed up by polychemotherapy over a period of 6 months.

[2 cases of mycotic infection caused by Cryptococcus neoformans].

The authors present two cases of Cryptococcus neoformans disease in two young persons. Its appears, in the first case, a diffuse ganglionnary and cutaneous lesion with an evolutive meningo-encephalic localization, and in the second case it appears only a meningitic lesion. These two observations are characterized by the apparition of the infection in a immunodepression patient and by an unfavorable evolution in spite of the treatment.

[Na/K transport in red blood cells from severely burned patients (author's transl)].

The working of the Na/K pump in the red cells of 6 patients with extensive burns was analyzed using radioactive substances with the aim of comparing their red cells with those of 20 normal subjects. In the red cells of patients with extensive burns was found that the intracellular sodium concentration was clearly increased, that the rate constant of ouabain-sensitive efflux diminished, and that the total sodium efflux was augmented by the increase of the ouabain-insensitive part. It is likely that the intra and extra-cellular transportation defects produce an accumulation of sodium inside the cell which succeeds in stimulating the activity of the pump. Although this pump is impaired, the high sodium concentration obtains a new steady state characterized by apparently normal ouabain-sensitive sodium efflux and by an increased ouabain insensitive efflux. The pathogenesis of these defects of cellular homeostasis which are linked to the presence of various complex mechanisms (shock, calorie balance, fluid and electrolyte imbalances, the circulation of "toxic substances" etc), in cases of extensive burns, has not been completely elucidated.
