[The valves of the ureter. Apropos of 2 cases].

The authors report two cases of congenital ureteral valves of the upper third of the ureter complicated by renal stone in two children and diagnosed by I.V.P. The two patients underwent a resection of the pathological segment and a pelvic-ureteral anastomosis successfully. The review of the literature data regarding ureteral valves allowed to emphasize the uncommon of the bilateral form complicated by renal stone and the better preoperative diagnosis by ureteroscopy and antegrade pyelogram.

[Malignant fibrous histiocytoma with giant cells of the skin. Anatomoclinical and immunohistochemical study of a case].

The authors report a case of malignant giant cell soft tissue tumor which appeared as a skin tumor of the thigh in a Tunisian women aged 17 years. Immunohistochemical study showed that the giant cells recognized KP1 antibody, while spindle interstitial cells reacted with anti-factor XIIIa antibody. The patient is alive and well eighteen months after large surgical removal. Two similar cases have been reported in the literature. Differential diagnosis, histogenesis and prognosis of this rare tumor are discussed.

[Carcinosarcoma of the uterus. Clinical case and review of the literature].

The authors report a case of carcinosarcoma which occurred in the uterus of a 69 year old woman. The survey of literature shows that it is a scarce tumor (1-3% of the uterine cancers). Its definition and its classification are still debated. Its final diagnosis is anatomopathological. Its treatment is generally radio-chirurgical and its prognosis is bad.

Age Factors

Monitoring of elastase in plasma of burned patients in relation to other inflammation parameters.

Twenty burned patients divided in three groups according to the severity of the lesions were investigated at 1- or 2-day intervals for up to 5 weeks after injury. Plasma elastase levels were elevated during the first day after injury and were correlated with the area of the burns. However, plasma elastase was rapidly bound and inactivated by protease inhibitors. Leucocyte counts, fever and the concentration of alpha-1-proteinase inhibitor were not correlated with the extent of the burn. The rise of plasma elastase was not accompanied by consumption of the elastase inhibitory capacity (EIC) of plasma, which increased to a plateau around day 5. The EIC values were in accord with the rise of alpha-1-proteinase inhibitor, the major anti-elastase agent in plasma. Studies of blister fluid in eight patients showed that the elastase content was higher than that of corresponding plasma, while the concentration of alpha 1-proteinase inhibitor and the EIC were comparable with those of plasma. Measurements of the levels of tumour necrosis factor released by stimulated macrophages in five patients with major burns showed no significant increase compared with controls.

[Nephroblastoma in the adult. Apropos of a case].

Adult nephroblastoma is a rare tumor. The authors report a new case observed in a 35 year old woman in whom the diagnosis was made by histopathology. The authors study the particular features of this tumor. Its prognosis appeared to be poorer than that of renal adenocarcinoma.

[Spontaneous subcapsular hematoma of the kidney].

A case of spontaneous subcapsular hematoma of the kidney is reported. CT scan findings and etiologies are examined in the light of previously published data. An etiology-specific therapeutic strategy for subcapsular hematoma of the kidney is proposed.

[Embryonal carcinoma of the submandibular gland. Apropos of a case].

The authors report a case of infantile embryonal carcinoma of the submandibular gland in a nine months old infant. The diagnosis was based on typical histopathological features, and positive reaction of tumor cells with alpha-foetoprotein. The patient was treated by wide surgical excision and chemotherapy. The clinical course was favourable two years later with normalization of serum alpha-foetoprotein levels and with no abnormality on the thoraco-abdomino-facial CT scan.

[Signet ring cell carcinoma of the bladder. Apropos of a case].

Signet ring cell adenocarcinoma of the bladder is a very rare tumor. It presents similar to the linitis plastica type of gastric adenocarcinoma. The authors report a new case and analyze the medical literature in order to specify the diagnostic problems and the histogenesis of this tumor.

Adenocarcinoma

[Primary adenocarcinoma of the bladder. Apropos of 10 cases. Retrospective study].

We report ten clinicopathological cases of primary adenocarcinoma of the urinary bladder. Histologic features were often reminiscent of those seen in digestive tumors: Lieberkühnian adenocarcinoma (4 cases), mucinous adenocarcinoma (3 cases) and signet ring cell adenocarcinoma with linitis plastica (1 case). Prognosis was poor in these tumors that were all invasive and had spread beyond the muscular layer: four patients died and six were critically ill when they were lost to follow-up. We emphasize the role played by glandular metaplasia of the vesical urothelium in the histogenesis of these tumors.

Adenocarcinoma

[Genitourinary malacoplakia. Apropos of 5 cases. A review of the literature].

The authors report 5 new cases of malakoplakia: 2 with kidney, 1 with prostatic and 2 with testicular involvement. The symptoms are not clinically specific, but pseudo-tumor forms are predominant. Four cases were treated by removal of the organ. Owing to a better understanding of its etiopathogenic mechanism, this disease can be treated by long-term antibiotic, cholinergic agonists and vitamin C therapy.
