Neonatal lupus erythematosus, multiple thromboses, and monoarthritis in a family with Ro antibody.

We describe a family afflicted with striking clinical and serologic autoimmune features. The mother and maternal uncle of a patient with neonatal lupus had rheumatic disease manifestations. All three had Ro antibodies (SS-A) in their sera, as well as La antibody (SS-B). The 17-year-old mother developed postpartum inflammatory monoarthritis of the right knee and had a positive lupus band test. The uncle at the age of 26 developed a fulminant disease most consistent with systemic lupus erythematosus (SLE); initial manifestations were myocardial infarction, deep vein thrombosis, and the nephrotic syndrome. Although it is known that mothers of neonatal lupus infants can develop SLE postpartum, the development of severe disease in the maternal uncle suggests the relevance of identifying seropositive relatives of individuals with neonatal lupus.

Topical zinc sulfate augmentation of human delayed type skin test response.

The ability of topical zinc sulfate to augment the cutaneous delayed hypersensitivity response to Candida antigen was evaluated in 47 adults (15 controls and 32 hospitalized patients). On each adult subject, intradermal standard Candida extract was administered to each forearm followed immediately by topical application of 10% zinc sulfate in Aquaphor ointment on one arm and Aquaphor alone on the other arm. The reaction size was assessed in a single blinded manner. Of the 24 subjects who reacted positively to Candida antigen, a significantly larger (p less than 0.01) number of individuals showed an augmented cutaneous delayed hypersensitivity response in the arm on which the topical zinc sulfate had been applied. However, when patients were stratified by plasma zinc concentrations, only the normal plasma zinc patient group demonstrated a statistical augmentation. Possible mechanisms and selectivity of this observed effect are discussed. These findings suggest a role for topical zinc application to augment cutaneous immune responsiveness.

Technicium as a radiolabel in the assessment of in vivo human Fc clearance.

99m-Tc was used as a radiolabel to study in vivo human clearance of IgG sensitized erythrocytes. Stannous chloride pretreatment at different doses and varying amounts of 99m-Tc were studied with respect to radiolabelling efficiency and elution. Mean in vivo clearance rates in systemic lupus erythematosus patients were significantly prolonged compared to controls. Using as little as 200 microCi of 99m-Tc per patient, in vivo human immune clearance can be determined with a minimal radiation biohazard. With this method, serial immune determinations can be performed within very short time frames, allowing for monitoring effects of immunomodulatory therapy as well as variations in disease activity.

Suppressor function of T lymphocytes in the acquired immunodeficiency syndrome as assessed by allogeneic mixed lymphocyte culture.

Nine patients with the acquired immunodeficiency syndrome (AIDS) were studied for suppressor cell activity utilizing an allogeneic mixed lymphocyte culture technique. The patients studied had opportunistic infections and a high incidence of intravenous drug abuse. T lymphocytes of the AIDS patients showed a consistently greater ability to suppress immunoglobulin M (IgM) synthesis by pokeweed mitogen stimulated allogeneic lymphocytes compared to controls (p less than 0.03). In 4 patients, T helper function was studied and compared to controls. Diminished helper cell function was consistently observed (p less than 0.01). All AIDS patients had leu 3a/leu 2a ratios of less than 0.9. However, a statistical correlation between the helper/suppressor cell ratio and suppressor activity was not demonstrated. Functional tests of T lymphocytes may be of value in assessing immunocompetence in patients with altered lymphocyte subset ratios.

Serum immunoglobulin levels and tissue antibodies in psoriasis.

Serum samples from psoriatic patients were subjected to quantitation of serum IgA, IgG and IgM levels and screening of tissue autoantibodies including ANA, GPA, MTA and SMA. Serum IgA was found to be significantly elevated and incidences of ANA and GPA were also increased. Their significance and possible mechanism were discussed.

Squamous cell carcinoma of the urachus.

The third documented case of primary squamous cell carcinoma of the urachus in the English literature is reported. The interesting coincidental occurrence of the dermoid cyst of the right ovary and a complete longitudinal septum of the vagina and uterine cervix is described.

Patterns of nasal eosinophilia in allergy clinic patients as determined by swab and curette sampling.

The presence of eosinophils in nasal secretions often characterizes inflammatory disease in the nose and paranasal sinuses. This study analyzed differential associations between various clinical parameters and the presence of nasal eosinophilia in mucosal specimens obtained by two sampling methods (swab and curette-probe). Nasal mucosal secretions were obtained in patients attending an adult allergy clinic using both sampling methods for each patient. The presence of eosinophilia as determined by both methods had significant associations with 1) nasal mucosal pallor, 2) younger age, and 3) the presence of basophilic cells. To examine diagnostic characteristics of the two sampling methods of determining eosinophilia as a characteristic of allergic rhinitis, receiver operating characteristic (ROC) curves were evaluated for patients categorized as having allergic rhinitis on the basis of elevated aero-allergen specific IgE, and rhinitis based on either a) significant nasal mucosal appearance abnormalities, or b) a referring physician's diagnosis of rhinitis. The curette-probe determined nasal eosinophil quantitations had greater areas under the ROC curves when rhinitis was defined on the basis of mucosal appearance. On the other hand, the swab determined eosinophil quantitations had greater ROC curve areas when rhinitis was defined on the basis of referring diagnosis. In summary, nasal eosinophilia determined by curette or swab sampling has differences in strengths of their clinical associations.
