Gastric function in Crohn's disease.

Gastric acid secretion before and after stimulation with pentagastrin assessed in 11 patients with Crohn's disease. Age and sex-matched patients with ulcerative colitis and healthy controls were used for comparison. Basal acid output was found to be significantly elevated in patients with Crohn's disease who had had surgery (P less than 0.01). This elevation was related to ileal resection rather than ileal disease. There was no difference in the maximal or peak acid outputs between any of the groups.

Diagnosis of Crohn's disease.

A study has been made of diagnostic errors and delays in 140 patients with Crohn's disease. Results were correlated with the site of the disease. Only 13% of patients with small-bowel disease and 21% of the group as a whole were correctly diagnosed after initial investigation. A large number of misdiagnoses were made, and these were often multiple. Of patients with small-bowel disease, 19% were first thought to be suffering from a psychiatric disorder.

Liver disease in patients with diabetes mellitus.

Liver function tests were assessed in 60 unselected out-patient diabetics stabilized on insulin or oral hypoglycaemic agents. Routine liver function tests, particularly plasma concentrations of gamma-glutamyl transpeptidase and alkaline phosphatase were elevated occasionally but rarely to more than twice the upper limit of normal. There was no correlation between measures of diabetic control and results of liver function tests. Twelve (20%) patients had evidence of gall stones, a prevalence above the expected from the community. Fourteen (23%) patients had an abnormally bright liver ultrasound echo pattern, probably indicative of fatty infiltration of the liver. This echo pattern was associated with only a minimal rise in plasma alanine amino transferase and alkaline phosphatase concentrations. It is concluded that functionally significant liver disease is uncommon amongst stabilized diabetic patients.

Virus isolations from mosquitoes in southern Ontario, 1976 and 1977.

Following the 1975 epidemic of St. Louis encephalitis (SLE) in Ontario, programs were instituted to monitor virus activity in mosquito populations during 1976 and 1977. Mosquitoes were trapped with CDC light traps and CO2 cone traps, pooled by species, and tested for virus by intracerebral inoculation of suckling mice. In 1976, 51 175 mosquitoes were tested. SLE virus was isolated from two mixed pools of Culex pipiens--C. restuans mosquitoes. Five isolations of California serogroup viruses were made. Three of these were trivittatus virus, which has not been demonstrated previously in Canada, and the other two were snowshoe have virus. Other viruses isolated in 1976 were a virus antigenically identical to the virus of infectious bursal disease of chickens and 34 Flanders viruses. In 1977, 34 428 mosquitoes were tested. Flanders virus was isolated most frequently, from pools of mixed C. pipiens--C. restuans mosquitoes. The only other isolate was a Bunyamwera group virus, Cache Valley virus. This virus has not been reported previously in Ontario.

The accuracy of ultrasound in the detection of fatty infiltration of the liver.

The association of high-amplitude echoes returned from the liver in diffuse fatty infiltration has now become well recognized. We report our experience on the accuracy with which ultrasound will detect a fatty liver. A review was made of the liver ultrasound images in patients known to have either histologically normal livers or livers with fatty infiltration. Only two of 40 patients with normal liver biopsies had a brightly reflective echo pattern, but 12 of 20 patients with fatty infiltration showed this pattern. Of the patients with histologically moderate or severe fatty infiltration, nine out of ten had a brightly reflective echo pattern.

Hepatic necrosis in rats following halothane administration: protective effect of diethyldithiocarbamate.

Foci of necrosis have been observed in the liver of 50% of enzyme-induced rats treated with halothane. The frequency of liver necrosis was significantly reduced by treatment with the free radical scavenger diethyldithiocarbamate. The severity of the lesion was not increased by repeated exposures to the anesthetic, and, although the frequency of liver necroses may be greater, the differences were not statistically significant.

Type B hepatitis: progression to chronic hepatitis.

The ability to test for HBV associated antigens has provided evidence that viral hepatitis is a common cause of chronic liver disease world-wide. Fewer than 10% of cases of acute icteric HBV become chronic, but chronicity is more common after anicteric infections. The prognosis for chronic persistent hepatitis B is usually good whereas many cases of chronic active hepatitis B will progress to cirrhosis although the time scale may be long. Factors determing the severity of the liver disease and the progression to chronicity are unclear, but the immune response of the host to the infection seems to play a major role.

Adult age and the rate of an internal clock.

Two experiments were conducted to determine whether young and old adults differ in the rate of a hypothetical internal clock. Clock rate was measured as the slope of the function relating actual duration to perceived duration. No age differences were apparent when subjects were asked to judge the duration of a flash of light in Exp. I, or to judge the duration of a dark interval between two light flashes in Exp. II. It was concluded that there is no evidence to support the hypothesis that perceptual and motor speed differences associated with increased age can be attributable to a slower rate of internal time.

Serum lysozyme levels in malignant histiocytosis of the intestine.

Serum lysozyme levels were significantly raised in a group of eight patients with malabsorption associated with gastrointestinal lymphomas of a type recently characterised as malignant histiocytosis of the intestine. In four of the cases, levels were markedly raised. In contrast there was no significant difference between groups of patients with uncomplicated adult coeliac disease and healthy controls. The estimation of serum lysozyme is a simple test to perform and may be valuable in the diagnosis of malignant histiocytosis of the intestine, in particular differentiating it from uncomplicated adult coeliac disease.

Antibodies to maize in patients with Crohn's disease, ulcerative colitis and coeliac disease.

The incidence of antibodies to maize using an immunofluorescent technique has been found to be 14% in controls, 33% in Crohn's disease, 50% in ulcerative colitis and 44% in coeliac disease. This result indicates that humoral immunity to maize is probably unimportant in the pathogenesis of Crohn's disease. The similar incidence of antibodies in the inflammatory bowel disease and coeliac groups suggests absorption of dietary antigen secondary to an increased mucosal permeability.
