Bathtub drownings: report of seven cases.

The domestic bathtub is an important site for infant drownings. A total population study of drowning and near-drowning accidents involving Honolulu infants has enabled the risks to be specified with greater detail. A series of seven consecutive bathtub immersion accidents is presented. The "at risk" profile comprises highly mobile families of lower socioeconomic status; usually younger siblings in larger families are involved and often the father had immediate care of the infant at the time of the accident. Another case of bathtub immersion as a form of nonaccidental injury is described. In five of the other six cases reported, the drowned child was left attended by an older sibling. Preventive strategies are discussed.

Prophylactic use of cefazolin in monitored obstetric patients undergoing cesarean section.

Prophylactic cefazolin was evaluated in high-risk obstetric patients who had invasive fetal monitoring and subsequent cesarean section. A three-dose regimen of either cefazolin or placebo was administered randomly in a double-blind manner to 93 patients, 48 receiving cefazolin and 45 placebo, with the first dose given when the cord was clamped. In the placebo group, 51% of the patients were treated for endomyometritis compared to 29% in the cefazolin group. The incidence of urinary tract and wound infections was similar in the 2 groups. One patient in the active drug group was diagnosed as having septic pelvic thrombophlebitis and received multiple antibiotics and heparin before she recovered. One patient in the placebo group required triple antibiotic therapy. Operation to control infection was not required in any patients and there were no maternal deaths. The cefazolin patients had fewer degree-hours of morbidity as calculated by the quantitative fever index (P less than 0.002). The perioperative use of cefazolin was efficacious in reducing the incidence of endomyometritis, but did not prevent serious postoperative pelvic infection.

Cellulose acetate electrophoresis of creatine phosphokinase isoenzymes in the diagnosis of myocardial infarction.

A method for rapid separation and visualization of creatine phosphokinase (EC 2.7.3.2 ATP: creatine phosphotransferase) isoenzymes on cellulose acetate has been developed. The technic uses the Beckman microzone electrophoresis system, and is applicable to other cellulose acetate systems. Incubation is with a commercially available substrate solution, and the isoenzyme bands are visualized by fluorescence using a long-wavelength ultraviolet light. An intermediate migrating isoenzyme band (CPK-MB) was detected in sera of 67 of 68 patients with myocardial infarction, and was not present in patients with other disorders that might clinically be confused with myocardial infarction.

Aldactazide-induced consummatory and operant responding to sodium by rats.

The effects of intraperitoneal injections of Aldactazide-A on rats' consumption of water and saline solution (.51M NaCl) were tested in two experiments. During the treatment sessions, the rats showed a substantial increase in saline consumption and in lever pressing to a cue light to obtain the saline, as compared with their negligible intake before the injections and after the injections had been discontinued. The results indicate that both the drinkometer and operantbox measures are sensitive to the natroexigenic effects of Aldactazide.
