Capillaries within human skeletal muscle fibers.

Internalized capillaries, i.e. capillaries within muscle fibers, represent a rare myopathological feature. This was systematically studied in 923 muscle biopsy specimens and found in 24, chiefly in the gastrocnemius muscle, more rarely in the biceps and quadriceps muscles affecting males more often than females and most frequently associated with juvenile spinal muscular atrophy or Becker's muscular dystrophy. Internalized capillaries, often multiple, ran along the long axis of the muscle fiber within an "internalized" extracellular space and were almost exclusively seen in type I myofibers. Internalization seems to start at the site of fiber splitting while penetration through the intact sarcolemma and invasion into transverse tubules were never observed. The presence of internalized capillaries within type I myofibers and increased density of intramuscular capillaries/fiber though not per muscle fiber area suggested hypoxia to play a possible if not crucial role in the formation of internalized capillaries. Our findings do not distinguish between active proliferation of capillaries into myofibers at the site of myofibers and internalization by fusion of vicinal myofibers as the morphogenetic principles suggest that both of these mechanisms may occur.

Drug-induced versus drug-triggered pemphigus.

112 cases of induced pemphigus were reviewed. The causative drugs were divided into three groups according to their chemical structure, and the clinical, prognostic and laboratory characteristics were analyzed and compared. Our literature screening indicates that the biological progress and clinical course of the disease depend upon the type of the inducing/triggering drug in the majority of patients. Patients with penicillamine-induced pemphigus or with pemphigus induced by SH drugs (drugs containing a sulfhydryl radical) showed spontaneous recovery in 39.4 and 52.6%, respectively, once the drug was discontinued. Patients whose pemphigus was induced by other drugs showed spontaneous recovery in only 15% of the cases. These results indicate that in patients with pemphigus induced by penicillamine (or SH drugs), the drug plays a major role in the pathogenesis of the disease, as compared to patients with pemphigus induced by other drugs. It seems that penicillamine (and SH drugs) actually induces pemphigus in most of the cases, whereas other drugs only trigger the disease in patients with a previous predisposition.

A prospective, randomized trial comparing combination half-dose tissue-type plasminogen activator and streptokinase with full-dose tissue-type plasminogen activator. Kentucky Acute Myocardial Infarction Trial (KAMIT) Group.

BACKGROUND: The potential benefits of combination thrombolytic agents in the treatment of myocardial infarction remain uncertain. In a small pilot study, we demonstrated that combining half-dose tissue-type plasminogen activator (t-PA) with streptokinase (SK) achieved a high rate of infarct vessel patency and a low rate of reocclusion at half the cost of full-dose t-PA. METHODS AND RESULTS: We designed a prospective trial in which 216 patients were randomized within 6 hours of myocardial infarction to receive either the combination of half-dose (50 mg) t-PA with streptokinase (1.5 MU) during 1 hour or to the conventional dose of t-PA (100 mg) during 3 hours. Acute patency was determined by angiography at 90 minutes, and angioplasty was reserved for failed thrombolysis. Heparin and aspirin regimens were maintained until follow-up catheterization at day 7. Acute patency was significantly greater after t-PA/SK (79%) than with t-PA alone (64%, p less than 0.05). After angioplasty for failed thrombolysis, acute patency increased to 96% in both groups. Marked depletion of serum fibrinogen levels occurred after t-PA/SK compared with t-PA alone at 4 hours (37 +/- 36 versus 199 +/- 66 mg/dl, p less than 0.0001) and persisted 24 hours after therapy (153 +/- 66 versus 252 +/- 75 mg/dl, p less than 0.0001). Reocclusion (3% versus 10%, p = 0.06), reinfarction (0% versus 4%, p less than 0.05), and need for emergency bypass surgery (1% versus 6%, p = 0.05) tended to be less in the t-PA/SK group. Greater myocardial salvage was apparent in the t-PA/SK group as assessed by infarct zone function at day 7 (-1.9 SD/chord versus -2.3 SD/chord after t-PA alone, p less than 0.05). In-hospital mortality (6% versus 4%) and serious bleeding (12% versus 11%) were similar between the two groups. CONCLUSIONS: These results suggest that a less expensive regimen of half-dose t-PA with SK yields superior 90-minute patency and left ventricular function and a trend toward reduced reocclusion compared with the conventional dose of t-PA.

Intact cells on the laser handpiece--a non-contact contamination.

Material adhering to the handpiece of the laser instrument was collected and analysed. Examination of semithin sections of the material by light microscopy revealed small clusters of clearly defined whole cells among the abundant debris. Electron microscopy of the cells revealed the presence of cell membranes, cytoplasm and organelles such as mitochondria. This could mean that infected cells or virus particles could adhere to the laser instrument or the hands of the operator as well. The medical implication of this finding requires serious consideration in regard to risks of infection from this procedure, to both patient and clinical personnel.

Noninvasive procedures in dermatology. Potential infective risks and need for sterility.

Although in the area of incisional surgery the level of sterile techniques has improved in recent years and approaches that of the surgical standards in other specialties, one cannot say the same with regard to some of the nonsurgical or semisurgical procedures. Here, unfortunately, there are even no standards to aim for and no guidelines to adopt. Since in the field of nonsurgical cosmetic skin care dermatologists have a unique position, we alone must shoulder the responsibility for setting the standards and determining the appropriate conditions for carrying out these procedures. Unfortunately, it appears that in certain of the dermatologic procedures not involving actual skin incision, the level of sterility has not improved for decades. In such dermatologic procedures there are marked discrepancies between the care taken to maintain a high degree of sterility of instruments that penetrate the skin, compared with the lack of attention to sterility of instruments that do not directly penetrate the skin and the area surrounding the procedure site. It seems that the guiding principle we must strive for is to prevent the transmission of infection from one patient to another. The emphasis should be not on sterility per se but on the prevention of transmission of disease from patient to patient.

[Estrus and ovulation synchronization in Merino meat sheep. 1. Effect of Gonavet "Berlin Chemie" after estrus synchronization with prostaglandin F2 alpha in Merino meat sheep].

Oestrus synchronisation by means of PGF2 alpha analogues was followed by injection of Gonavet "Berlin-Chemie" which triggered off an LH peak, 2 to 3 hours from injection. Injection of Gonavet "Berlin-Chemie", 44 hours after PGF2 alpha application, caused synchronisation of all LH peaks. The interval between injection of Gonavet "Berlin-Chemie" and onset of ovulation amounted to 22 hours. The length of ovulation was not accurately determinable. Ovulation was successfully induced to all sheep by application of Gonavet "Berlin-Chemie", 44 or 48 hours after PGF2 alpha injection. Ovulation rates were 1.75 or 1.54. Luteolytic action on sheep of Cloprostenol "Jenapharm", a PGF2 alpha analogue, proved to be just as good as that of Oestrophan (SPOFA).

[Estrus and ovulation synchronization in Merino meat sheep. 2. Fertility outcome after estrus and ovulation synchronization with Cloprostenol "Jenapharm" and Gonovet "Berlin-Chemie" and term-oriented artificial insemination].

49 of 102 sheep of one production herd, which had returned to oestrus once or twice within the 1985 breeding season, were treated twice in a nine-day interval with the PGF2 alpha analogue Cloprostenol "Jenapharm". 53 animals received an additional dose of 10 micrograms of Gonavet "Berlin-Chemie". A lambing rate of merely 15% was achieved by subsequent term-oriented artificial insemination. 31 sheep still in oestrus one day after term-oriented artificial insemination were served, bringing the lambing rate from synchronisation of oestrus and ovulation to 26% and the lambing result to 154%. Serving of the remaining sheep in 2 subsequent oestric cycles, on balance, yielded lambing rates and results of the remaining sheep in 2 subsequent oestric cycles, on balance, yielded lambing rates and results which were higher with significance than those recorded from untreated sheep in the same herd.

[Effect of collateral circulation on the progression of left ventricular dilatation after myocardial infarct].

48 patients (aged 50.5 +/- 8.5 years) with proven acute first myocardial infarction underwent coronary angiography 1.6 weeks to 4 years (mean 13.3 weeks) after infarction. No reperfusion interventions were performed, no patient received a longterm therapy with diuretics, glycosides, beta-blockers or ACE-inhibitors. In accordance with the angiographic results, patients were divided into two groups: 25 patients demonstrated a proximal occlusion of one major coronary artery with retrograde contrast filling by non-compromised collaterals; in 23 patients no collateral filling of the distal vessel segment was visible. The contralateral coronary arteries and corresponding LV wall segments were normal. Age, sex distribution, localization of the infarction and infarct related vessel and intraventricular pressures (LVSP = 121.2 +/- 13.5 vs. 122.1 +/- 18.9 mm Hg; LVEDP = 13.1 +/- 5.2 vs. 11.9 +/- 4.1 mm Hg) were not significantly different between both groups. Between both groups patients were compared in whom angiography was performed less than 4, 4-6, and greater than 6 weeks after infarction. In addition, occlusion of the LAD was compared with LCX- and RCA-obstructions both with and without collaterals. Patients with collaterals revealed no significant time dependent changes of the endsystolic (48.1 +/- 11.3; 56.7 +/- 20.5; 43.8 +/- 16.2 ml/m2), enddiastolic (114.9 +/- 30.6; 121.3 +/- 10.8; 99.0 +/- 22.5 ml/m2) volumes and ejection fraction (57.7 +/- 6.5; 53.9 +/- 13.5; 56.4 +/- 9.9%). In contrast in patients without collaterals ESVI increased significantly (44.2 +/- 16.2; 50.8 +/- 17.5; 68.3 +/- 30.9 ml/m2) by an exponential function (y = 28.11 e0.13x; r2 = 0.99; p less than 0.0005).(ABSTRACT TRUNCATED AT 250 WORDS)

[Use of preventive intra-operative antibiotic administration in appendectomy. A pharmacokinetic study].

Studies on Cefazedone-kinetics in serum and subcutaneous fat tissue showed an early and lasting tissue concentration high above minimal inhibitation concentrations so that intraoperative single-shot administration of antibiotics are proven to be effective. Due to these pharmacokinetic patterns the results of an earlier prospective trial is confirmed that single-shot antibiotics during surgery are useful to reduce postoperative wound sepsis rate after appendectomy.

Atopic dermatitis provoked by AL721 in a patient with acquired immunodeficiency syndrome.

A 40-year-old patient with acquired immunodeficiency syndrome (AIDS) had an atopic dermatitis-like skin eruption and elevated serum IgE levels after therapy with AL721. This agent is a mixture of egg yolk lipids used for the treatment of AIDS. The relationship between the drug and the eruption has been based on circumstantial evidence and confirmed by the positive results of skin tests. Although an almost unlimited amount of materials have been reported to be responsible for atopic manifestations, cases of drug-induced atopic dermatitis are rare. AL721, however, can be regarded as a food product; therefore, the appearance of the eruption following intake of this agent should not be considered exceptional.

Visual control of straight flight in Drosophila melanogaster.

The optomotor system of Drosophila is investigated in a flight simulator in which the fly's yaw torque controls the angular velocity of the panorama (striped drum, negative feedback). Flies in the flight simulator maintain a stable orientation even in a homogeneously textured panorama without landmarks. During 'straight' flight, torque is not zero. It consists of small pulses mostly alternating in polarity. The course is controlled by the duration (and possibly amplitude) of the pulses. The system operates under reafference control. By comparing the pulses with the visual input the system continuously measures and adjusts the efficacy of the torque output. The comparison, however, is not between angular velocity and yaw torque but, instead, between visual acceleration and pretorque, the first time derivative of torque. For comparison, the system first computes a cross-correlation. If the correlation coefficient is above a certain threshold the system calculates the external gain and adjusts its internal gain so as to keep the total gain constant. With the correlation coefficient below threshold, however, the system keeps the internal gain low despite the infinitely small external gain. We propose that for a reafferent optomotor system the coupling coefficient and the correlation coefficient of pretorque and visual acceleration are more relevant than the distinction between exafference and reafference.

Systemic cold-induced urticaria--clinical and laboratory characterization.

Six patients are described with a history of cold-related urticaria in whom standard tests (water-immersion and ice-cube) did not induce symptoms. Only total-body cold exposure induced generalized urticaria. Systemic cold urticaria should, therefore, be included in the differential diagnosis of cold-dependent allergic disorders.

Recent neurochemical and pharmacological aspects of pathogenesis and therapy of affective psychoses.

Regarding the neurochemical pathogenesis of affective disorders a strict discrimination between pathogenetic, phenomenological and epiphenomenological neurochemistry has to be made. The possible functional role of an opioid-deficiency in the pathogenesis of endogenous depression and the therapeutic effect of recently developed opioid compounds are considered. Furthermore, the clinical use and the theoretical background of mood stabilizers in affective disorders and especially the mode of action of valproate is represented. As to the basic neurobiological factors regulating mood in normals and in patients with affective disorders an "unidimensional" model is confronted with a "bidimensional" scheme in which two different "gating-zones" are proposed. Interestingly, a neurochemical/psychopharmacological consideration of such a model reveals a new synopsis about the mode of action of psychoactive compounds modifying mood in patients with affective disorders.
