We report on 11 patients with "very proximal" lesions out of a total of 300 consecutive patients who underwent percutaneous transluminal coronary angioplasty (PTCA). Eight patients had native left anterior descending (LAD) lesions and three had LAD saphenous vein grafts lesions. Lesions were considered to be "very proximal" when one half or more of the balloon was inflated in the left main coronary artery for the native LAD lesions or the aorta for the LAD coronary artery saphenous vein bypass graft (CABG) lesions. There was a mean reduction in stenosis from 88.3% (range 75-99) to 13.8% (range 0-60) and a mean reduction in transtenotic gradient from 47.2 mmHg (range 20-80) to 8.3 mmHg (range 0-20). The initial success rate was 90.9% (10 out of 11 patients) with a partial success in the other patient. No complications occurred in any of the patients. Two patients had restenosis (18.2%) at 3 months and 6 months, respectively, post-PTCA. It is concluded that "very proximal" lesions can be successfully dilated with a high initial success rate and low complication rate. Nevertheless, these lesions may present problems with guiding catheter stability and, because of the potential risk of circumflex (CX) occlusion, this vessel may have to be protected with a second guidewire.

The reduced nutritional state of cancer patients can be influenced by a purposeful feeding, which decreases the rate of complicating factors and diminishes side effects resulting from the often aggressive cancer therapy. Multimorbidity, a change in the need for nutritive substances in advanced age and senile functional changes in numerous organs make it a necessity to adapt elderly cancer patients trophotherapy to these conditions.
The balanced artificial nutrition of the aged has to notice involutional-induced changes in the function of organs and organ systems. The energy requirement decreased with age. By means of a normo- and hypocaloric parenteral nutrition it is possible to overcome starvation and a increased energy requirement in trauma can be satisfied. The artificial nutrition can reduce the risk of surgical treatment of the aged.
With advancing age functional and morphological changes take place within the intestinal tract. The absorption rate (constant k12), the time of appearance of the relative maximum of the D-xylose concentrations in the blood serum after oral administration (time tm), and especially the extent of the relative absorption in the small intestine until the time tm RR (tm) determined by graphic-mathematical evaluation of the blood levels kinetics of D-xylose after oral and intravenous administration, are useful parameters for differentiation between involutively and pathological changes in intestinal absorption. Absorption disorders in old age are due to morbidity and not, sui generis, to involution.
On the basis of experimental and theoretical examinations the senile changes in man's absorbent system and renal excretory function were analysed. It was found that intestinal absorption and summary elimination are delayed with advancing years. Practical consequences with regard to dosage and dosage intervals can be derived for geriatric pharmacotherapy. By the example of selected groups of drugs the necessity for revised dosage instructions and application intervals in old age is explained.
Founded upon experimental and theoretical investigation at age-dependence of the absorping system in human beings is analysed. Starting from a compartment model the absorption, velocity of absorption and effectivity of absorption are defined in the physical sense. The experimental investigations results in a general decrease of the velocity of all the processes in the absorping system with increasing age. This is the reason for a temporal changed kinetic behaviour of aged systems. By comparison between young and aged systems with reference to their kinetic properties we find the following results: With increasing age we observe a decrease of the velocity of absorption and of the effectivity of absorption, but with regard to the quantity of intestinal absorption not a difference between young and aged is pointed out.

Studies show that mouthwashes containing alcohol soften the surface of composite resin restorations. The present study determined weight change over time in heat-treated composite resin soaked in alcohol and nonalcohol-containing mouthwashes. The results indicate that samples soaked in mouthwashes containing alcohol gained significantly more weight than samples soaked in nonalcoholic mouthwashes.
