Endonasal microendoscopic pansinusoperation in chronic sinusitis. II. Results and complications.

PURPOSE: We evaluated the long-term results and complications of endonasal pansinusoperation in chronic polypoid sinusitis. PATIENTS AND METHODS: In a retrospective study, 170 patients were followed-up for 20 months to 10 years after bilateral endonasal microendoscopic pansinus surgery or ethmoidectomy. The follow-up consisted of a standardized questionnaire and clinical examination with the flexible endoscope. RESULTS: We found that 85.6% of the ethmoid cell systems, 69.4% of maxillary sinuses, and 37.5% of frontal sinuses could be visualized endoscopically. The ethmoid mucosa was normal in 56% and thickened in 19%. Recurrent polyps were found in 25%. The evaluation--as per the graduation of results defined by us as a combination of examination findings and subjective assessment of the operative result--resulted in an operative success of 92%. Two studies dealing with the frequency of complications showed injury to the dura in 2.3% to 2.55% and periorbital injury without permanent sequelae in 1.4% to 3.4%. Because of two cases of bleeding from the internal carotid artery, the problems of vascular complications in particular will be thoroughly discussed. CONCLUSION: More than 90% of patients with chronic polypoid sinusitis gain long-term satisfying results after endonasal ethmoidectomy with microscope and endoscope. For minimizing the risk of injury of the optic nerve or the internal carotid artery preoperative, computed tomography is necessary. A special training program is recommended.

Air medical transport utilization review in North Carolina.

OBJECTIVE: To perform a review of the collective experience of all hospital-based helicopter ambulances in the state of North Carolina for compliance with utilization review criteria. DESIGN: Flight records of the six members of the North Carolina Aeromedical Affiliation for the months of November and December 1989 were compared with utilization review criteria by an independent reviewer. A secondary review was performed by a staff member for each service. Scene responses and patients flown to a hospital other than the sponsor were evaluated. SETTING: All six hospital-based helicopter services in North Carolina. TYPE OF PARTICIPANTS: All available flight records for November and December 1989. INTERVENTIONS: None. MEASUREMENTS AND MAIN RESULTS: Of 756 transports, 747 flight records were available for review. Initial review demonstrated compliance with the criteria for 713 (95.4%) patients; secondary review showed compliance for 18 of 34 flights not meeting initial review, for an overall compliance rate of 97.9%. Compliance rates for scene responses and transports taken to a hospital other than the sponsoring facility were 96.6% and 94.1%, respectively. CONCLUSIONS: Review of all flights over a period of two-months by all six hospital-based helicopter services in North Carolina using utilization review criteria demonstrated a very high rate of compliance with the established criteria.

Packing in endonasal surgery.

Nasal packing is used to control bleeding in epistaxis and after endonasal surgery, for internal stabilization, and to prevent synechiae or restenosis, particularly after surgery. Generally accepted standards regarding the materials that should be used for packing, how long the packing should be left in place, or the indications for nasal packing are lacking. In view of the present lack of standardization and the many different packing materials used, we review the currently available materials and outline their respective properties, indications, and risks.

[Standardizing terminology in pediatric oncology--the basic data set].

BACKGROUND: In the context of more than 20 therapy optimizing clinical trials in pediatric oncology an extensive documentation with a big number of case report forms was developed in the last 20 to 25 years. Across these trials same information is partially captured in different terminological ways, by which documentation about patients in the clinics is made more difficult. METHOD: Terminology of therapy optimizing clinical trials of German Society for Pediatric Oncology and Hematology (GPOH) is standardized by a central "standards committee". RESULT: As a first result the basic data set of GPOH could be revised and made available in internet via http://www.dospo.uni-hd.de. CONCLUSION: A basis of a unique documentation language in pediatric oncology is available for German speaking regions.

Short-term in vitro culture and molecular analysis of the microsporidian, Enterocytozoon bieneusi.

The microsporidium, Enterocytozoon bieneusi, causes a severe, debilitating, chronic diarrhea in patients with the acquired immunodeficiency syndrome. Specific diagnosis of intestinal microsporidiosis, especially due to Enterocytozoon, is difficult and there is no known therapy that can completely eradicate this parasite. Preliminary studies indicate that a short term (about 6 months) in vitro culture of this parasite yielding low numbers of spores, may be established by inoculating human lung fibroblasts and/or monkey kidney cell cultures with duodenal aspirates and or biopsy from infected patients. The cultures may subsequently be used for the isolation and molecular analysis of parasite DNA.

Use of crestal bone for augmentation of extremely knife-edged alveolar ridges prior to implant placement: report of 3 cases.

A technique is presented for interforaminal lateral augmentation of mandibles with adequate bone height, but extremely knife-edged mandibular alveolar ridges (Class IV of Cawood and Howell's classification of residual ridges), in which the crestal portion of the knife-edged ridge is used as grafting material. Following an osteotomy and rotation of the grafts by 180 degrees, the grafts were fixed to the residual ridge below the osteotomy line by means of miniscrews. All grafts showed only mild resorption after a healing period of 3 months, and it was possible to place 4 implants in the now sufficiently wide host region.

Atypical cognitive disorders in a man with developmental surface dyslexia.

The neuropsychological profile of a man with a developmental surface dyslexia is presented here. This case study is of interest because J.C. exhibited a pattern of cognitive disorders rarely documented in previous data. Results showed that JC's difficulties in reading comprehension were closely related to complex memory disorders and were also associated with cognitive slowness. The present observations do not support the visual memory failure hypothesis. The data rather suggest that the core difficulty primarily lies with the nonautomatization of grapheme-phoneme correspondence rules, which in turn dramatically contributed to lexicon weaknesses. The hypothesis of a timing mechanism in reading disorders is discussed.

Culture, electron microscopy, and immunoblot studies on a microsporidian parasite isolated from the urine of a patient with AIDS.

Microsporidian spores isolated from a urine sample of an HIV-positive patient were inoculated onto monolayers of six different cell cultures. The parasites (CDC:0291:V213) grew profusely in two of the cultures (HLF and E6) and extruded spores into the culture medium. The spores were Gram-positive, 2.25- to 2.8-microns long, 1.25- to 1.8-microns broad, and smooth-walled. Some of the spores had already extruded their polar tubes, which were either straight or slightly coiled. Infected host cells contained parasitophorous vacuoles filled with developing stages of the parasite, including mature spores. Each spore was surrounded by a thin, electron-dense exospore; a thick electron-lucent endospore; and a thin cell membrane. Cross-sections of six coils of the polar tube were seen inside the spore. Proteins extracted from spores of our isolate and those from Encephalitozoon cuniculi were separated on gradient sodium dodecyl sulfate-polyacrylamide gels and either silver-stained or transferred to nitrocellulose membranes. As many as 35 bands, ranging in molecular mass from 10,000 to 200,000, were visualized in the silver-stained gel. When reacted with the serum of our patient, strips cut from the membrane showed a number of bands ranging in molecular weight from 25,000 to 200,000. However, unique differences between the profiles of the two parasites were seen both in the immunoblot and the silver-stained protein profiles. Based on these findings, we conclude that our isolate belongs to the genus Encephalitozoon, but more studies are needed to identify our isolate to the species level.
