Paracrystals of myosin rod.

To help understand the packing of myosin tails in the backbone of the vertebrate striated muscle thick filament, paracrystals of myosin rod, a proteolytic fragment corresponding to the whole myosin tail, have been examined by electron microscopy and image analysis. Two types of paracrystal were observed. Type I paracrystals were similar to those seen by Moos et al. (1975; J. molec. Biol. 97, 1-9). These showed a 14-nm axial repeat, but yielded no other structural information. Type II paracrystals were long, flexible ribbons with more regularity. When negatively stained they exhibited a weak 43-nm axial striation and appeared to be composed of a layer of narrow filaments. Optical diffraction showed that the paracrystals had a rectangular unit cell of dimensions 43 nm axially and 12.4 nm laterally. Transverse sections indicated a paracrystal depth similar to the lateral dimension of the unit cell. Each unit cell contained two filaments arranged antiparallel and related by a two-fold screw axis perpendicular to the length, and in the plane of the ribbon. The filaments probably consist of parallel rod molecules related by axial displacements of 43 nm and higher multiples of 43 nm. The nature of these paracrystals indicates that the myosin tail alone can form structures like thick filament subfilaments. Their structure, based on distinguishable parallel and antiparallel rod interactions, was sensitive to pH and divalent cations in a similar way to the ionic effects on the structure of thick filaments. This behaviour suggests that some of the interactions present in the paracrystal are the same as those in the thick filament.

Metastatic adenocarcinoma of the cervix presenting as an inflammatory breast lesion.

The occurrence of an inflammatory breast lesion and wide-spread venous thrombosis in a woman with metastatic adenocarcinoma of the endocervix is reported. Examination of biopsy specimens showed the inflammatory breast mass to be histologically consistent with metastasis from the endocervix. Recurrent venous thrombosis requiring large doses of heparin for control of the phlebitis was also a feature of this patient's illness. The disease was associated with elevation of serum CA 125 levels that did not parallel the patient's clinical course.

Woven carbon-fibre patch versus Dacron mesh in the repair of experimental defects in the lumbar fascia of rabbits.

Defects were created in the lumbar fascia of adult New Zealand white rabbits and repaired with either woven carbon-fibre or Dacron mesh. The histological and mechanical properties of the two materials were compared at intervals up to 12 wk post-operatively. Microscopy showed an abundant orderly deposition of collagen highly vascularized with a minimal inflammatory response on the carbon fibre, whereas the Dacron mesh induced the formation of a flimsy collagenous capsule, with a marked inflammatory response. Mechanical testing showed a significant increase in the strength of the carbon-fibre patch after 2 wk (P = .001), the patch becoming much stronger than the Dacron mesh (P = .002) 5 to 12 wk after implantation.

Estimation of plasma area under the curve for etanidazole (SR 2508) in toxicity prediction and dose adjustment.

The hydrophilic 2-nitroimidazole radiosensitizer etanidazole is currently undergoing clinical evaluation. Although considerably less neurotoxic than misonidazole because of its rapid renal clearance and partial exclusion from the nervous system, total dose is limited by peripheral neuropathy. Monitoring plasma etanidazole concentration in patients to determine the area under the curve (AUC0-infinity) has been proposed as a method of predicting patients at risk, and of providing a quantitative basis for dose reduction in such patients. Successful application of this policy requires accurate assessment of AUC0-infinity. We have analyzed plasma data for 18 patients receiving 2 g/m2 etanidazole to determine the errors introduced in the estimation of AUC0-infinity caused by omitting selected time points from the analysis. A 'baseline' AUC0-infinity value was calculated by integration of the rate equation for the 2-compartment model using data points at 0, 15, and 30 min and 1, 2, 4, 8, 12, and 24 hr after the end of infusion. The mean +/- SD area for AUC0-infinity was 502 +/- 152 micrograms ml-1 h (2.35 +/- 0.71 mM.h). Omitting the zero or the 24 hr time point, the average errors were quite small (2.5% in both cases), but errors of up to 16.4 and 7.3%, respectively, were seen for individual patients. Leaving out both the 8 hr and 12 hr points at the same time gave a similar low average error of 2.9%, with a highest error of 7.3%. Omitting all data points after 4 hr, the mean error was 24.7% and 15 of 18 patients had errors in excess of 10%. In addition, failure to correct for infusion time results in an underestimation of AUC0-infinity averaging 4.5% (range 1.9-8.7%). The choice of sampling times for toxicological monitoring will depend upon the accuracy with which the AUC0-infinity must be known. Including all data points between 0 and 24 hr will minimize errors. Considering the general similarity in the errors introduced by omitting the 8 hr and 12 hr points together compared to those seen with exclusion of the single 24 hr point, the choice between these truncated sampling options would be expected to lie in the relative inconvenience caused to patients and medical staff for the particular dose schedule used. The short sampling schedule (0-4 hr) should not be used.

Conjunctive search for one and two identical targets.

The assumptions of feature integration theory as a blind, serial, self-terminating search (SSTS) mechanism are extended to displays containing 2 identical targets. The SSTS predicts no differences in negative-response displays, which require an exhaustive search of the display. Quantitative predictions are confirmed for the positive responses, but not for the negatives, suggesting that the SSTS model is incorrect. Two possible explanations for the results in the negative conditions, differential search rates and early quitting in the negatives, are rejected. It is suggested that using any self-terminating search mechanism will lead to difficulty in interpreting the results, including accounts for which the search is parallel over small groups of items. A resource-limited parallel model, which is based on the diffusion model of Ratcliff (1978), appears to fit the data well.

Physique characteristics of Pan American Games lightweight rowers.

The emergence of lightweight rowing as an international sport has made the optimization of physique within the weight restrictions a matter of primary importance in selection and training of the participants. The occasion of the Xth Pan American Games provided opportunity to obtain comprehensive anthropometric data on 20 male and 13 female lightweight rower finalists including most of the medal winners. Anthropometric characteristics, somatotype, and proportionality profiles showed the male rowers to be similar in most aspects to a student control sample, with the exception of short sitting height and large transverse chest breadth. The females, on the other hand, appeared to be very different from the control sample, having a number of characteristics similar to those of Olympic rowers. The female lightweight rowers also uniquely demonstrated two distinct physique prototypes.

Localized fibrous tumor of the serosal cavities. Immunohistochemical, electron-microscopic, and flow-cytometric DNA study.

Localized fibrous tumor of the serosal cavities (localized fibrous mesothelioma) is a generally benign spindle cell neoplasm of uncertain histogenesis. Fourteen histologically similar primary tumors from different mesothelium-lined sites (11 pleural and 1 each in the pericardium, peritoneal cavity, and pouch of Douglas) and 2 recurrences of those tumors (pericardium and pouch of Douglas) were examined histopathologically and by flow cytometry to relate histologic features and DNA ploidy to biologic behavior (follow-up, 48-255 months among 13 patients). All 16 tumors (14 primaries and 2 recurrences) displayed diploid DNA pattern, and none of 13 patients died of disease (1 patient was lost to follow-up). To elucidate the histogenesis, seven primary tumors were examined for vimentin and keratin immunostaining and six primary tumors were assessed by electron microscopy. All cases exhibited spindle-fibroblastic cell proliferation with a prominent hemangiopericytic pattern. All cases so examined had positive results for vimentin and negative results for keratin. Ultrastructurally, the tumor cells showed mesenchymal-fibroblastic features. These results support a mesenchymal origin, most likely from submesothelial fibroblasts. Further, this neoplasm may recur but retain its basic histologic features, diploidy, and benign outcome.

The optic nerve of the viper, Vipera aspis.

A quantitative investigation of the optic nerves of eight adult vipers was carried out, both at the level of resolution of the light microscope by stereological methods, and at the level of resolution of the electron microscope. The majority (87%) of axons are myelinated, and the fibers are organized in fascicles; each bundle of axons is disposed around a central column of glial cells of which the greater proportion (62%) are oligodendrocytes, 33% are astrocytes and the remaining 5% are microglial cells. Estimations of the total number of fibers varied, from 5.37 X 10(4) to 6.96 X 10(4), among the eight specimens. The distribution of diameters of myelinated fibers is well characterized (chi 2 = 16.84, df = 22, 0.9 greater than p greater than 0.5) by a three-parameter lognormal distribution with a mean of 0.735 micron, standard deviation of 0.239 micron, and origin of 0.21 micron. Axon diameter is linearly related (r = 0.60) to the number of electron-dense layers in the myelin sheath. The diameters of unmyelinated fibers are distributed (chi 2 = 1.95, df = 4, 0.9 greater than p greater than 0.5) as a two-parameter lognormal distribution with mean of 0.25 micron and standard deviation of 0.067 micron. A surprisingly large number of myelinated fibers, apparently dispersed at random throughout the nerve, showed anomalies of myelination; two possible explanations, between which it is impossible to discriminate on the basis of the available data, are offered for this state of affairs.

Emergency nursing priorities of the head injured patient.

The first priority in any emergency is always an adequate airway. The nurse is involved in clearing the mouth, inserting an oral airway, assisting with intubation, oxygen therapy and assessing continually the patient's respiratory system. The nurse must also monitor the patient's vital signs, administer intravenous fluids appropriately along with medications, insert foley catheters and note urinary output. Simultaneously, the nurse constantly observes the patient's level of consciousness and neurological status. This information must be documented carefully by the nurse amidst all this apparent chaos. This can be done only when good assessment skills, combined with knowledge of diagnosis, treatment and appropriate nursing intervention, aid the Emergency Nurse in setting priorities in the Emergency care of the head injured patient.

Purification of the cardiac Na+-Ca2+ exchange protein.

We have used fractionation procedures to enrich solubilized cardiac sarcolemma in the Na+-Ca2+ exchange protein. Sarcolemma is extracted with an alkaline medium to remove peripheral proteins and is then solubilized with decylmaltoside. Next, the exchanger is applied to DEAE-Sepharose and eluted with high salt. The DEAE fraction is applied to WGA-agarose, and a small fraction of protein, enriched in the exchanger, can be eluted by changing the detergent to Triton X-100. This fraction is reconstituted into asolectin proteoliposomes for measurement of Na+-Ca2+ exchange activity and gel electrophoresis. The purified fraction has a Na+-Ca2+ exchange activity of 600 nmol Ca2+/mg of protein per s at 10 microM Ca2+ and a purification factor of about 30 as compared with control reconstituted sarcolemmal vesicles. Ca2+-Ca2+ exchange and Na+-Ca2+ exchange activities were both present in the same final reconstituted vesicles indicating that the same protein is responsible for both transport activities. SDS-PAGE reveals two prominent protein bands at 70 and 120 kDa. After mild chymotrypsin treatment (1 microgram/ml), there is no loss of exchange activity, but the 120 kDa band disappears and the 70 kDa band becomes more dense. This suggests that the 70 kDa band is due to an active proteolytic fragment of the 120 kDa protein. Under non-reducing gel conditions, only a single protein band is seen with an apparent molecular weight of 160 kDa. Antibodies to the purified exchanger preparation are able to immunoprecipitate exchange activity and confirm that the 70 kDa protein derives from the 120 kDa protein. We propose that both the 70 and 120 kDa proteins are associated with the Na+-Ca2+ exchanger.

A longitudinal study of the effects of retinal ablation on the organization of the retinal target lamina of the optic tectum in the teleost, Rutilus rutilus.

The optic tecta of 55 Rutilus rutilus were examined at intervals varying from 2 days to 4 years after unilateral retinal ablation. Qualitative ultrastructural examination of the retinal target lamina of the optic tectum (stratum fibrosum et griseum superficiale, SFGS) revealed that an initial period of degeneration and glial reaction, each of which could take one of a variety of forms and which lasted for 1-3 months after ablation, was followed by the temporary formation of heterologous synapses which persisted for a further 1-12 months. This in turn was followed by the degeneration of these synapses during the second year after ablation. Quantitative analysis at the level of the light microscope revealed a shrinkage of the SFGS throughout the level of the light microscope revealed a shrinkage of the SFGS throughout the first 14 postoperative months with no further reduction taking place thereafter. Analysis at the ultrastructural level revealed that this shrinkage was due to the disappearance, and not to the reduction in size, of pre- and postsynaptic profiles accompanied by glial reaction. No signs of collateral sprouting were seen throughout the survival period. Thus, partial deafferentation of the SFGS leads in the long run to a marked impoverishment of its neuronal network, without any apparent compensation.

Relationship of the body mass index with skinfolds, girths, and bone breadths in Canadian men and women aged 20-70 years.

The relationship of Quetelet index (w/h2), or body mass index (BMI), with the sum of skinfolds at five sites, two skinfold-corrected limb girths, and two bone breadths were studied in a cross-sectional sample of 12,282 men and 6,593 women aged 20-70 years. The correlations of the BMI with skinfolds (0.50), bone breadths (0.51), and girths (0.58) were too low for individual prediction. Contingency tables of the BMI and sum of skinfold categories further indict its use for the purpose of assessing adiposity status or monitoring change in individuals.

The multi-dose clinical tolerance and pharmacokinetics of the combined radiosensitizers, Ro 03-8799 (pimonidazole) and SR 2508 (etanidazole).

The hypoxic cell radiosensitizers Ro 03-8799 (pimonidazole) and SR 2508 (etanidazole) have differing physico-chemical properties and clinical toxicities. The former is basic, lipophilic and produces an acute but transient central nervous system syndrome; the latter is neutral, hydrophilic and causes cumulative peripheral neuropathy. We therefore investigated the possibility of combining these agents to achieve additive radiosensitization with no enhancement of toxicity, as demonstrated in a rodent tumor model. Following a single dose study which showed a lack of interaction with respect to both toxicity and pharmacokinetics, twenty-one patients have now completed simultaneous drug administration on an escalating, multiple dose schedule. There has been no adverse acute interaction up to 0.75 g/m2 Ro 03-8799 with 2 g/m2 SR 2508 for 15 doses. At this dose-level, however, all patients experienced peripheral neuropathy. There was no adverse pharmacokinetic interaction, or perturbation of plasma pharmacokinetics between initial and final infusions. Tumor concentrations were determined in 48 biopsy samples 0-60 min after administration. Mean values normalized to a dose of 0.75 g/m2 Ro 03-8799 plus 2 g/m2 SR 2508 were 33 micrograms/g Ro 03-8799 and 74 micrograms/g SR 2508. These would be expected to produce a single-dose sensitizer enhancement ratio of around 1.5. The combination is predicted to be around 6.8 times more active than misonidazole, and superior to any single agent tested to date. The current schedules are reaching the limits of clinical tolerance, and an attempt is now being made to define the optimal regimen for use in a randomized clinical trial of the combination.

Additivity of radiosensitization by the combination of SR 2508 (etanidazole) and Ro 03-8799 (pimonidazole) in a murine tumor system.

The nitroimidazole radiosensitizers SR 2508 and Ro 03-8799 have different dose-limiting toxicities in man and hence can be used in combination. Such therapy will be beneficial only if their radiosensitizing properties are additive, which this study sought to determine using clinically relevant radiosensitizer concentrations in the EMT6 tumor in the flanks of BALB/c mice. 240 mg/kg of each drug gave tumor concentrations (+/- 2 se) 55 min after i.v. administration of the combination of 50.4 +/- 10.6 micrograms/g (236 +/- 50 nmol/g) for SR 2508 and 39.7 +/- 9.0 micrograms/g (137 +/- 31 nmol/g) for Ro 03-8799. The radiosensitization by both agents administered both singly and in combination at 240 mg/kg and singly at 480 mg/kg was measured, giving sensitizers 30 min before 20 Gy of 250 kV X rays. Tumor response was assayed by clonogenic cell survival. SER values (with 95% confidence limits) were 1.28 (1.20-1.37) for 240 mg/kg SR 2508, 1.20 (1.10-1.30) for 240 mg/kg Ro 03-8799, 1.46 (1.33-1.59) for 240 mg/kg of both drugs in combination, 1.46 (1.38-1.55) for 480 mg/kg SR 2508 and 1.46 (1.31-1.62) for 480 mg/kg Ro 03-8799. These data confirm the additivity of radiosensitization by the two drugs administered in combination.

Hypoxic cell radiosensitizers in the treatment of high grade gliomas: a new direction using combined Ro 03-8799 (pimonidazole) and SR 2508 (etanidazole).

The hypoxic cell radiosensitizers Ro 03-8799 (pimonidazole) and SR 2508 (etanidazole) have been evaluated for their simultaneous penetration into human brain tumors and surrounding normal tissue. Thirteen patients received a dose of 1 g of each agent, infused over a 10 minute period during neurosurgery. Samples of glioma (20), brain (10) and cerebrospinal fluid (1) were obtained at a mean time (+/- SD) of 31 +/- 18 min from the end of infusion. A 24 hr plasma time course was measured in six patients. Nitroimidazole concentrations were determined by HPLC. For a mean dose of 0.55 g/m2 of each agent, the mean tumor concentrations (+/- SD) were 17.0 +/- 12.0 micrograms/g for Ro 03-8799 and 13.5 +/- 10.9 micrograms/g for SR 2508. The tumor/plasma ratios were 279 +/- 230% and 47 +/- 34% respectively. For adjacent 'normal' brain tissue, the radiosensitizer concentrations were 29.9 +/- 13.1 micrograms/g for Ro 03-8799, and 4.0 +/- 1.7 micrograms/g for SR 2508, and the brain/plasma ratios were 430 +/- 29% and 14 +/- 8% respectively. There was a significant trend towards increasing accumulation of both agents with time, in both tumor and normal brain. Concentrations in cerebrospinal fluid were very low. Plasma pharmacokinetics for Ro 03-8799 were similar to previous experience, but for SR 2508 the terminal half-life was greater in this series by a factor of 1.3. The results confirm that Ro 03-8799 is distributed widely in the central nervous system, and demonstrate that SR 2508 can achieve high tumor concentrations when the blood-brain barrier is compromised. The concentrations achieved with the combination are indicative of a significant advantage over metronidazole, misonidazole, or either agent alone, and normalized to the therapeutic dose of 0.75 g/m2 plus 2.0 g/m2 SR 2508 are consistent with those giving additive sensitization in an in vivo mouse tumor model.

[Spatio-temporal evolution of optic tract regeneration in Rutilus rutilus].

Regeneration of the different components of the optic tract of Rutilus takes place at a variable rate and follows a relatively precise pattern. The first optic centres to be reinnervated belong to the lateral thalamo-pretectal group (5 weeks at 14 degrees C after section of the optic nerve), followed by the anterior optic tectum and lateral geniculate nucleus (8 weeks after section), the central regions of the tectum, the suprachiasmatic nucleus and the nucleus of the basal optic root (10-15 weeks after section), and finally the medial thalamo-pretectal nuclei and the caudal regions of the optic tectum (16-25 weeks after section).
