Pimozide versus chlorpromazine in chronic schizophrenia: a 52 week double-blind study of maintenance therapy.

This study compared the effectiveness and safety of pimozide and chlorpromazine in the maintenance therapy of chronic schizophrenic outpatients. Forty-three patients were assigned in double-blind fashion to one of the drugs for the 52 week study. Both medication groups showed a general pattern of improvement over the course of the study, but there was no significant difference between the two groups. The results demonstrated the effectiveness and safety of pimozide in once daily administration. Previous research results showing the special utility of pimozide for the improvement of emotional withdrawal and social competence in schizophrenia were not replicated.

Symptomatic methemoglobinemia with a commonly used topical anesthetic, cetacaine.

Severe cyanosis resulting from acquired methemoglobinemia after application of a topical anesthetic, Cetacaine spray, occurred in a 37-year-old patient following bronchoscopy for postoperative atelectasis. Response to methylene blue therapy was dramatic and complete. Attention is drawn to a dangerous adverse effect of this commonly used topical anesthetic agent.

Cyclophosphamide-induced cardiomyopathy: a report of two cases and review of the English literature.

Fatal cardiomyopathy developed in two patients receiving cyclophosphamide in preparation for bone marrow transplantation. Both patients had normal EKGs prior to receiving cyclophosphamide in total doses of 168 mg/kg (case 1) and 144 mg/kg (case 2) and subsequently developed loss of voltage and ST-T wave changes. One patient (case 1) died of CHF and hypotension while the other patient (case 2) developed tamponade. Prior to this report, the lowest total dose of cyclophosphamide reported to cause fatal cardiomyopathy was 180 mg/kg. In contrast to anthracycline congestive cardiomyopathy, the effects of cyclophosphamide appear to have an acute onset and do not appear to be the cumulative result of drug dosing. Postmortem examination in both patients revealed thickened left ventricles with intramyocardial hemorrhage.
