Hair dye use and risk of leukemia and lymphoma.

Data from a population-based case-control study of incident leukemia and non-Hodgkin's lymphoma among adult men in Iowa and Minnesota were used to evaluate risk associated with hair dye use. The relative risk for ever using hair dyes was 1.8 (95% confidence interval [CI] = 1.1-2.7) among leukemia patients, and 2.0 (CI = 1.3-3.0) among cases with non-Hodgkin's lymphoma. There was a suggestion of increased risk with extent of hair dye use. Given the widespread use of hair coloring products, these observations deserve more detailed evaluation in populations where the exposure is relatively common.

First-pass radionuclide angiography using a multiwire gamma camera and tantalum-178.

A portable multiwire gamma camera (MWGC) with enhanced imaging characteristics relative to conventional sodium iodide camera has been evaluated with 178Ta, a short-lived, generator-produced radioisotope (half-life 9.3 min). First-pass radionuclide angiography (FPRA) was performed and results were compared to those obtained with FPRA using a multicrystal camera (MCC) and 99mTc in 38 patients. The overall left ventricular count sensitivity (counts/mCi/sec/millisteradians [msr]) was significantly higher with MWGC/178Ta (176 +/- 132 versus 108 +/- 49, p less than 0.001) yielding images of higher statistics with higher resolution collimation (31 versus 63 msr). Left ventricular ejection fraction was 0.54 +/- 0.18 by MWGC and 0.54 +/- 0.18 by MCC with an excellent correlation between the two techniques (r = 0.94, s.e.e. = 0.06). The detection of wall motion abnormality was virtually identical with the two techniques. Intra- and interobserver reproducibility by MWGC was excellent (r = 0.99 and 0.99, respectively). Thus, this new technology provides first-pass studies of higher statistical quality and improved resolution, affording more precise assessment of left ventricular performance and likelihood of further substantial improvement by use of even higher doses of 178Ta.

Atopic eczema unresponsive to evening primrose oil (linoleic and gamma-linolenic acids).

This study was designed to look at the effect of evening primrose oil (linoleic and gamma-linolenic acids) as an oral supplement for patients with atopic eczema. We used a double-blind, blocked crossover design with random assignment of patients to treatment groups. We used Wilcoxon's signed-ranks method of comparing changes during the trial. We observed no significant effect on erythema, scale, excoriation, lichenification, or overall severity in 123 patients with atopic eczema of average severity while they took oral doses of evening primrose oil (2 or 4 gm in children, 6 or 8 gm in adults).

Tri-state toxic-state syndrome study. I. Epidemiologic findings.

Eighty women who had onset of toxic-shock syndrome (TSS) between October 1, 1979, and September 19, 1980, and 160 age- and sex-matched neighborhood controls participated in this study to evaluate risk factors associated with TSS in women. Of the 80 women, 76 had onset of illness during their menstrual periods. The odds ratio for developing menses-associated TSS with any use of tampons compared with no use of tampons was 18.01 (P less than 0.001). The odds ratios ranged from 5.29 to 27.5 for individual brand use compared with no use. When exclusive use of a particular tampon brand was compared with exclusive use of all other brands. Rely (Procter and Gamble, Cincinnati, Ohio) was the only brand with a significantly increased odds ratio (2.49; P = 0.005). By multiple logistic regression analysis, the risk of TSS was more closely associated with tampon fluid capacity (absorbency) than with the use of all tampon brands.

Toxic shock syndrome: relation to catamenial products, personal health and hygiene, and sexual practices.

In 1980, the discovery of an association between vaginal tampon use and toxic shock syndrome affected the lifestyles of menstruating women and the catamenial products industry. It made both the general public and the medical community more aware of all aspects of menstruation. The relation between developing toxic shock syndrome and tampon use is unclear; tampon fluid capacity (absorbency) remains the best predictive measure of that risk. No unique aspect of tampon use other than absorbency seems to increase the risk of developing toxic shock syndrome, and numerous hygiene and medical history factors do not seem to play a role in the pathogenesis of the disease. Studies in Minnesota, Wisconsin, and Iowa indicate that 70% to 75% of women between the ages of 15 and 24, the group with the highest risk of developing menstrual toxic shock syndrome, continued to use tampons after news media attention in 1980 on the association of the syndrome with tampon use. This rate of use is higher than the rate found for the general population by recent tampon market research.

Coffee consumption and mortality from ischemic heart disease and other causes: results from the Lutheran Brotherhood study, 1966-1978.

In this cohort study, 16,911 men who had completed a mailed dietary questionnaire were followed for 11 1/2 years with 721 deaths reported from ischemic heart disease (IHD) (excluding those reporting on the questionnaire a prior history of angina or other heart conditions). Although no association was found between coffee consumption and mortality from IHD, a negative association between coffee consumption and mortality from diseases other than IHD was found. This negative association, found exclusively in the first four years of follow-up, was observed in deaths from digestive diseases, other than malignancies, and paralysis agitans, which made the greatest contribution to this observed negative association. The negative association appeared to reflect a reduction in coffee consumption related to the disorders in question and not to a protective effect of coffee. It has been suggested that the positive association between coffee consumption and IHD reported in some case-control studies may reflect a decreased consumption among controls rather than an unusually high consumption among cases.

Private psychiatric hospitals: "excellence is their watchword".

Throughout their history private psychiatric hospitals have been a major force in psychiatry, and their humanistic concern for the mentally ill has resulted in numerous innovations in treatment. Over the past two decades the sharp increase in psychiatric patient care episodes has resulted in a greater reliance on private psychiatric hospitals, and they have reponded by modifying their role, broadening the spectrum of patients served, and developing alternatives to inpatient treatment. In addition to continuing to provide high-quality inpatient care, private psychiatric hospitals in the future should expand various alternatives to such care, explore opportunities for collaboration with other health care systems, and provide more patient services to the underserved.
