Although first identified in 1976, penicillinase-producing Neisseria gonorrhoeae (PPNG) began to have significant epidemiologic impact in the United States only after 1980. The epidemic curve has been marked by a series of successive sigmoidal increases, the result of staggered epidemic activity in Los Angeles, California, New York, New York, and Miami. The most recent increase and current plateau has led to a provisional total of 16,608 cases for 1986, with the proportion of PPNG isolates as high as 30% in some communities. The intensity of PPNG transmission appears to parallel that of gonorrhea in general, with high attack rates in central city areas (i.e., among core groups) and with a dimmishing gradient outward from the center. The endemicity of PPNG is related in a general way to the size of the inoculum. A county experiencing seven cases in a month at the time of introduction, for example, has less than a 50% probability of reverting to zero cases during the next 12 months. It would appear that the inoculation of PPNG organisms into a community leads to a "take" when high-level transmitters of gonorrhea are affected. Though current projections suggest as many as 35,000-40,000 cases of PPNG infections by 1991, control programs that focus resources on core-group transmitters of gonorrhea may alter that course.
In dry etching processes--one of the sources of potential exposure to toxic wastes in the semiconductor industry--complex mixtures of inorganic and organic compounds arise from reactions between feed stock gases (BCl3/Cl2), top layers (aluminium photoresist), and the carrier gas (N2). Two different fractions of the complex mixture--one an ethanolic solution (ES) and the other an insoluble liquid residue (LR)--were examined for acute oral toxicity in rats. Analytical data showed that the ethanol soluble fraction contained mainly inorganic compounds, whereas the residue contained various halogenated hydrocarbons. Neither death nor behavioral changes occurred after oral administration and observation up to 23 days. ES caused a lower mean arterial blood pressure in both sexes, increased P-R-intervals in male rats, and caused some mild biochemical and hematological alterations and changes in relative organ weights compared to the control groups. Exposure to LR influenced food and water intake, and caused a significant decrease in body weights, signs of polyurie, as well as changes in various relative organ weights and biochemical and hematological parameters. The blood pressure of the male animals fell and the heart rates of both sexes decreased.

In this study 55 women (38 pregnant women without renal complications and 17 patients suffering from renal symptoms) are prospectively examined between the 16th week of pregnancy and 2 weeks after delivery using nephrosonography. Our results underline the value of the nephrosonographic investigation of the maternal kidneys during pregnancy and make clear the "physiologic" changes of renal parameters.
Examining serum level values and urinary concentrations taken from nonpregnant control persons and pregnant women suffering from urinary tract infections (UTI) during the third trimenon, the pharmacokinetic dates after single application of 1 g cefotaxime have been evaluated. We used a computer program based on the two-compartment model. It is shown that during pregnancy complicated by UTI the elimination half-life will be prolonged, the distribution volumes and clearance values will be increased. The results discussed with the dates given by literature allow us to state that it is not necessary to change dosage and application interval of cefotaxime during pregnancy.
Examining blood level values and urinary concentrations taken from pregnant women during the third trimester and nonpregnant female volunteers, the pharmacokinetic dates of cefotiam (a second generation's cephalosporin) have been evaluated. We used a computer program of an open two-compartment model for intravenous injection. We found an increase of some pharmacokinetic parameters (elimination half-life, distribution volume, clearance) during pregnancy. Because the serum levels were yet effective and nearly equal 6 hours after intravenous injection we state that it is not necessary to change the dosage and the application interval of cefotiam during pregnancy.
Examining blood level values and urinary concentrations taken from pregnant and nonpregnant women, the pharmacokinetic dates of azlocillin have been evaluated. We used a computer program based on the two-compartment model. The evaluation of azlocillin concentrations in serum and urine has been realized with the two following methods: polarography and agar diffusion test. We found nearly equal values of serum concentrations when using both methods. There are no statistical differences between pharmacokinetic dates of pregnant and nonpregnant women. Therefore we state: It is not necessary to change the dosage and the application interval of azlocillin during pregnancy.
