The idiopathic hypertrophic subaortic stenosis (IHSS) is thought to be infrequently complicated by infective endocarditis. Because IHSS is a disorder of the myocardium and not endocardium is not generally considered to have increased risk of infective endocarditis. Some Authors found, however, that approximately 5% of patients with IHSS develop bacterial endocarditis, because hemodynamically IHSS is like to valvular diseases. Therefore the incidence of endocarditis in IHSS is the same of valvular diseases. The vegetations can be found on anterior mitral valve leaflet, aortic cusps and subaortic endocardium. Some patients have dental surgery prior to the onset of endocarditis and others are infected with oral streptococci. For these reasons prophylactic antibiotics should be administered to patients with IHSS undergoing dental manipulation and in other settings where the risk of developing bacteremia is high. We describe one case of IHSS complicated with streptococcus viridans endocarditis. Vegetations, determined with echocardiography, were present on anterior mitral valve leaflet, aortic right coronary cusp and interventricular septum. In a review of literature we are been able to find twenty-seven cases of IHSS complicated with endocarditis.
The national data provided by ANMCO in 1978 show a total of 2.5 m heart cases in Italy, including 1.5 m instances of ischaemic cardiopathy, and an annual incidence of 105,000 new cases of myocardial infarct. An assessment was made of the 1976 and 1977 pattern of "cardiological emergency cases" (ISTAT codes 402, 410-14, 427-29, 441, 450, 519, 997) in the province of Florence and two of its social and health consortia: 39 (municipalities of Dicomano, Londa, Pelago, Pontassieve, Rufina, S. Godenzo), and 51 (municipalities of Borgata Ripoli, Greve, Impruneta, S. Casciano, Tavarnelle). The records of the S. Maria Nuova, Florence Hospital were used for in-patient data, since this is the only hospital in the province with automatic filing by discharge diagnosis according to the ISTAT code. Mortality in emergency cases in the two consortia m and the province fell from 14.68% and 17.17% to 11.20% and 18.11% respectively in the two years. In both populations, the highest incidence was between 70 and 74 yr of age in 1976, and between 65 and 69 yr in 1977. In the case of infarct, mortality moved from 18.75 to 7.58% (consortia) and from 22.40% to 20.93% (province), with maximum incidences in 1976 between 70 and 74 yr (consortia and province), and in 1977 between 65 and 69 yr (province) and between 55 and 59 yr (consortia). Admissions were greatest in number on Mondays (peak hours: 11 a.m. to 4 p.m.) whereas no difference was noted between one month and another. The Hospital Resuscitation Centre, which serves the two consortia, received 45 emergency cases in 1977, including 25 infarct patients. Total mortality was 13.33% (infarct 20%). The slight numeric discrepancy between in the latter data and those held in the computer suggests that the ISTAT code may not be easy to use and not always employed by physicians in a rational manner.

The development of a false aneurysm of the ascending aorta following arteriotomy and cannulation of the aorta performed during closure of a ventricular septal defect is described. The evidence suggest that the sutures of the arteriotomy led to rupture of the aortic wall and secondarily late formation of a false aneurysm.
Infrahepatic interruption of the inferior vena cava with azygos continuation has been demonstrated by angiography in three patients; in a fourth case an anomalous venous channel connected the patent azygos vein and the inferior vena cava at the renal level. Cardiac defects were present in all cases.

A case of electrical T wave alternans without change of P wave and QRS complex is reported here. Literature on this subject is reviewed. It is emphasized that the alternation of T wave always exists when important morphological anomalies of QT tract and prolongation of QT are present.
Two cases of left ventricular aneurysm occurring in young patients (23 and 29 years respectively) are reported. Coronary arteriograms were normal. Pertinent literature was reviewed. About 70 cases of left ventricular aneurysm in patients under 35 years of age have been reported: approximately half of them are congenital. They include both diverticuli of the apex, as a part of a syndrome of congenital defects, and those, much rarer, following myocardial infarction in children with anomalous origin of coronary arteries. A few cases may be attributed to trauma or myocarditis. In 26 patients, including the two reported here, etiology was uncertain (idiopathic aneurysm of the left ventricle) in as much as malformation or an infectious disease might have been the underlying cause.
Two cases of acute accidental quinidine poisioning in two children are presented. Both cases are characterized by the same ECG abnormalities consisting of rhythm disorders and AV and intraventricular conduction defects. It is suggested that there is a causal relationship between the electrophysiologic cardiac effects of quinidine and the ECG abnormalities observed. Both patients were successfully treated by continuous intravenous infusion of a hypertonic sodium chloride solution and lidocaine.
