[Carcinomas in situ of the breast].

In situ carcinomas of the breast represent a clinical reality of recent decades that oncologists have to face today. Their observation is rendered increasingly frequent thanks to the improvements made in radiological techniques for early diagnosis and mass screening, and better health education. Reference is made to cases of lobular and ductal in situ cancer observed at the Bologna Surgical Clinic. Recommendations are made with respect to the drawing up of a rational treatment plan.

[Therapy of spontaneous pneumothorax. Results of 20 years' experience].

256 spontaneous pneumothorax episodes in 165 patients were treated during the period 1956-1978. Continuous aspiration with drainage of the pleural cavity was employed in 156 cases, and parietal pleurectomy for 36 recurrent or undrainable cases. One patient in the latter group underwent bilateral parietal pleurectomy. The soundness of aspirative drainage on the occasion of the first episode is underscored by the low percentage of recurrences. The indications for pleurectomy are discussed. Its effectiveness is apparent from the good functional results it offers, in addition to an absence of relapses.

[50 cases of early gastric cancer].

Among 530 surgical specimens of histologically proved cancer or ulcer of the stomach during the last 5 years, 50 were classified as early gastric cancer. The increasing number of cases described in the recent years may be attributed to the more aggressive diagnostic approaches. 26 cases were confined to the mucosa and the remaining 24 invaded the submucosa. Lymph node metastasis was present in 4 patients. Surgical approach, with antrectomy, subtotal or total gastrectomy with radical lymphectomy, is pratically necessary.

Tracheobronchial ruptures due to cuffed Carlens tubes.

At our institution in the past 22 years, more than 3,000 patients have undergone chest procedures, and 2,700 of them were intubated with a cuffed Carlens endotracheal tube. In this paper we report on 5 patients with tracheobronchial ruptures caused by intubation with these tubes. We believe this hazard should be brought to the attention of physicians.

Technique for the management of anterior flail chest.

A technique for the management of anterior flail chest consisting of osteosynthesis and the positioning of two long Kirschner wires behind the sternum in the form of a St Andrew's cross is described. The procedure is easy to perform, the patient is ambulant early, and the results are good.

Ileorectal anastomosis after colectomy for benign colon disease.

The immediate, or staged reconstruction of the digestive tract after total abdominal colectomy for benign bowel disease with an ileorectal anastomosis (I.R.A.) finds adamant supporters and opposers, thus confirming the controversial aspects of this procedure. The follow-up of 27 cases operated upon between 1973 and 1985, is discussed on the basis of clinical, radiological, endoscopic, histological and functional assessments.

Toxic megacolon complicating ulcerative colitis and Crohn's disease.

The clinical, laboratory and radiological data of 17 cases of toxic megacolon (TM) complicating either ulcerative colitis (UC) or Crohn's disease (CD), referred to the Department of Clinica Chirurgica II of the University of Bologna in a twenty year-period, are reviewed. The surgical strategies and results are compared and discussed, and the mortality and morbidity considered along with the important advances in resuscitative medicine and the employment of total parenteral nutrition (TPN).
