[Stage II non-seminoma cancer of the testis: combined treatment with surgery and chemotherapy].

Fifty-one patients with non-seminoma testicular cancer who underwent retroperitoneal ganglion dissection and 3 support cycles of chemotherapy with Cys-Platin, Vinblastine and Bleomycin. Four patients presented relapses (7.8%). Actuarial survival by 8 years was 87%, with a median of 38 months. The median survival was 43 months. Three of the 4 patients with relapses are currently free of disease after rescue treatment, showing in global terms a control of the disease of 98% during this clinical stage.

[Furosemide in the early diagnosis of acute renal insufficiency in the newborn infant].

The study included 13 infants under one year complaining of acute gastroenteritis and dehydration who were still in oliguria or anuria 6 hours after rehydration was initiated. They were given a single dose of furosemide at the rate of 1 mg/kg and indices of U/P of urea and osmolarity, ratio urea/plasmatic creatinine, urinary volume, natriuresis and evolution of urea plasmatic figures and of creatinine were determined. Four patients showed no response to the diuretic; all of them died and through clinical and histopathologic evaluation they were classified as having acute renal insufficiency (IRA). The nine patients showing response to the drug with an increase of 5 to 30 times the control figure for urine and natriuresis showed an index U/P of urea of 5.52 +/- 3.82, U/P of osmolarity of 1.32 +/- 09, ratio urea/plasmatic creatinine of 58.7 +/- 19.8 and the figures for urea and creatinine in blood turned normal within 2 to 4 days. This was classed as prerenal azotemia (APR). It is thus concluded that furosemide appears to be a good parameter to make an early differentiation of cases with IRA, but that this measure, the same as the rest of the indices cannot show an absolute value since there are important variations in each individual.

[Analysis of urine in the child without renal disease and with fever].

The study comprised 24 patients with ages between 5 months and 13 years and fever from different extrarenal infectious diseases. Seventy-nine samples of urine were taken to determine proteins, glucose, ketone, hemoglobine and study of the urinary sediment. Electrophoresis of proteins was practiced to samples with higher content of them. Considering a normal rate of protein the figure above 4 mg/hr/m2, 70% of the patients and 39% of samples showed abnormality with figures up to 52 mg/hr/m2, or 1,259 mg/l. All of them excreted albumin and alphaglobulins; 15, betaglobulins and 4, gammaglobulins. No relationship was found between the amount or type of protein excreted and the degree of fever, age of the patient or type of disease. Thirty-seven per cent of the patients showed ketonuria, 29% erythrocyturia, 21% hemoglobinuria, 16% glycosuria, 12% cylindruria, and 8% leukocyturia. The duration of abnormalities lasted from 1 to 4 days and the magnitude was usually mild, having obtained in all of the cases, a normal sample of urine and a negative urine culture before discharge of the patient. Findings with other studies are compared and the pathogenesis of abnormalities is discussed.

[Evaluation of nephropathies in children with rheumatoid arthritis and prolonged therapy with salicylates].

The study included 18 school-age and adolescent patients with juvenile rheumatoid arthritis who had consumed from 351 to 6393 gm. (average 2813 gm.) as total dose for the control of their disease. All of them underwent a complete physical examination, general laboratory tests and as specific tests of renal function; urinalysis, urine culture, endogenous creatinine clearance, Addis count (red and white cells), sodium and potassium urinary excretion, urinary acidity capacity, administration of ammonium chloride and capacity of urinary concentration following water restriction. Normal results in practically all parameters led to conclude that a clear evidence of nephropathy due to salicylic acid consumption, was not found in any of the patients studied.

[Children who work on the streets of Tijuana. An epidemiological profile and the prevalence of experimentation with tobacco].

OBJECTIVE: To develop an epidemiological profile of children that work in the streets of Tijuana, Mexico, and to determine the prevalence of cigarette smoking in this group, as well as the prevalence of those factors associated with tobacco experimentation. STUDY TYPE: cross-sectional. MATERIAL AND METHODS: Children aged 8 through 15, who work in the streets of Tijuana were included. Demographic, socioeconomic, and smoking related variables were analyzed. RESULTS: The study included 110 children, with a mean age of 11.43 years. Of these 91.7% live with one or both of their parents; 60% attend school and an additional 30% did so in the past. Only 6.4% of these children are the principal economic support of the household, and 51.8% reported an average daily income of $US 10 or less. Only 9.1% reported experimentation with tobacco, and 31% of them tobacco prompting (in the form of lighting up a cigarette) by an adult of the family; this is significantly more frequent on behalf of the father than of the mother (21.8% vs 4.5% p < 0.001). CONCLUSIONS: Most of the children under 15 years of age who work in the streets of Tijuana live with their parents and maintain close relationships with their family. Most of them attend school and work in the streets only to complement the family income. Their tobacco experimentation is not greater than that of other children of the same age group.

[Seroepidemiological investigation of various tissue helminthiasis, by means of indirect hemagglutination test, in Lonquimay County, Chile].

In order to have a present perspective on the prevalence on the epidemiology of some tissue helminthiasis in Lonquimay County, southern Chile, in 1993 a seroepidemiological survey, by means of an indirect hemagglutination test (IHAT) for hydatidosis, cysticercosis and trichinosis was undertaken. In 11 (1.2%), 10 (1.1%) and 6 (0.6%) out of 945 examined persons, most of them from rural areas, the corresponding IHAT resulted positive. Additionally, radiographic and ecotomographical studies for thorax and abdomen respectively were performed to the 11 individuals with serology positive for hydatidosis: whereas all the thoracic radiographies resulted negative, liver hydatic cysts were detected in five persons, who latter on were submitted to surgery. Prevalence rates of hydatidosis in Lonquimay are among the highest regional and national values.

Can garlic lower blood pressure? A pilot study.

A popular garlic preparation containing 1.3% allicin at a large dose (2400 mg) was evaluated in this open-label study in nine patients with rather severe hypertension (diastolic blood pressure > or = 115 mm Hg). Sitting blood pressure fell 7/16 (+/- 3/2 SD) mm Hg at peak effect approximately 5 hours after the dose, with a significant decrease in diastolic blood pressure (p < 0.05) from 5-14 hours after the dose. No significant side effects were reported. Our results indicate that this garlic preparation can reduce blood pressure. Further controlled studies are needed, particularly with more conventional doses (e.g., < or = 900 mg/day), in patients with mild to moderate hypertension and under placebo-controlled, double-blind conditions.

[Focal sclerosing and segmentary nephropathy in children].

The cases of 23 nephrotic children in whom percutaneous renal biopsies or postmortem studies showed focal and segmentary sclerosis were reviewed. It was more common in males and the average age at the onset was 6 years. The main symptoms were edema, hematuria and unspecific general manifestations. On admission, nephrotic syndrome was present in 91% of the cases and in a lesser proportion, edema hematuria and high blood pressure were found. The lesion was diagnosed at the first biopsy in 19 cases and in 4, subsequent biopsies or postmortem studies supported the diagnosis. Only 9 out 21 patients treated showed response to steroids and 5 of the resistant cases were given cyclophosphamide, but only one responded. They were checked at different periods of time ranging from 7 months to 10 years and by the end of the study, 11 patients were still under control showing normal renal function in six of them; 4 other cases showed mild renal insufficiency and in the remaining one, there was moderate renal insufficiency. Two died. A good correlation was evident between response to steroids and conservation of renal function. The findings are compared to those from other authors and emphasis is placed on the need to suspect the entity when a nephrotic syndrome with minimal changes is resistant to steroids, or when persistent hematuria is present; also, in cases showing initial lesions of tubular atrophy or interstitial fibrosis.

Renovascular effects of nonprescription ibuprofen in elderly hypertensive patients with mild renal impairment.

To determine the renovascular effects of nonprescription ibuprofen in the maximum labeled over-the-counter (OTC) dosage for 7 days, and to compare these effects with those of two other available OTC analgesics, aspirin and acetaminophen, we evaluated 25 elderly patients with mild thiazide-treated hypertension and mild renal insufficiency. Under double-blind conditions, patients were randomly allocated to one of three treatment groups: ibuprofen 400 mg 3 times/day, aspirin 650 mg 3 times/day, or acetaminophen 650 mg 3 times/day. Blood pressure and indexes of renal function (blood urea nitrogen, creatinine clearance, serum electrolytes) were measured over 7 days in a clinical research center. None of the treatments had a clinically significant effect on blood pressure. Renal function indexes also remained unchanged during all three treatments. We conclude that elderly patients with mild thiazide-treated hypertension and mild renal insufficiency seem not to be at risk of developing additional renal compromise or of having their hypertension control diminished by treatment with these OTC analgesics for 7 days.
