[Osteomyelography and pseudarthrosis of the leg].

The authors have performed osteomedullography in 25 cases of tibial non-union. They conclude that a complete block at the fracture level is in favour of an established non-union. This technique can make it possible to differentiate between delayed bone union and non-union. It cannot be used in cases treated by intramedullary nailing.

[Tympanicum jugulare paraganglioma in an uncommon site].

The study is concerned with cases of tympanicum jugulare paragangliomas of special interest due to their localisation in unusual positions. These atypical positions have been established by the analysis of 5 cases operated on in our department. The surgical and analytical procedures are described in detail and a study of the nature of these T.J.P.s is provided.

[Idiopathic spontaneous pneumothorax - a current problem].

134 patients suffering from spontaneous pneumothorax were seen at the Surgical Department of Granada-University. Most of the patients were of young age (average age 23 years), with men prevailing. Until today its aetiology is unknown. Conservative treatment will do for cases with only partial pnth. In all other cases a suction drainage (Bülau-type) is installed. Surgery is only performed in those cases where no complete re-expansion of the lung can be achieved by suction drainage.

[Delayed traumatic apoplexy (author's transl)].

The yearly average of patients suffering from severe cranio-encephalic injuries who are admitted to our department is 492. From 1972 to 1980 we have been able to operate on 11 delayed traumatic intracerebral hematomas. Bollinger, in 1981, was the first to describe delayed traumatic apoplexy. Since the treatment is usually surgical evacuation of the hematoma. Results of treatment and conclusions are dealt with.

Prevention of stress ulcer by cimetidine and somatostatin in rats.

In experiments on live animals we have examined to what extent prophylactic applications of cimetidine and somatostatin are able to prevent stress-induced ulcer in the rat. 130 male Wistar strain rats were exposed to a combined immobilisation-immersion stress, cimetidine was given subcutaneously and somatostatin as a continuous intraperitoneal infusion. Results show that cimetidine produces no change in the average ulceration index (U.I.), nor in the transmural potential (P.D.). However, somatostatin significantly lowers the incidence rate of stress-induced gastric ulcers in rats.

[Alteration of serum enzyme activity after trauma and/or bacterial infection (author's transl)].

Our clinical and experimental study is based on the evaluation of the changes in enzymatic activity produced by surgical, traumatic and infectious agression. To this end we have evaluated the activity of the transaminase (GOT-GPT), malic dehydrogenase (MDH), lactic dehydrogenase (LDH) and creatine phosphokinase (CPK). The details of our study are based on a total of 175 patients and 82 animals (dogs). Our results show that the highest rate of enzymatic activity was in proportion to the seriousness of the wound (lesion) produced. The clinical course-index developed from the activity of CPK and MDH allows us to evaluate and predetermine the course which our patients and our animals were to follow.

Simultaneous treatment of portal hypertension and hepatic cirrhosis by splenohepatoplasty.

A study was done on the action of splenohepatoplasty upon intrahepatic and prehepatic portal hypertension. Portal hypertension was induced Wistar rats following the establishment of cirrhotic conditions. Hepatic cirrhosis was obtained in these rats by an oral, daily dose of tioacetamide during a four and five month period. Prehepatic hypertension was produced in dogs by the installation of a rubber cylinder, completely covered by cellophane paper, around the trunk of the vena porta. This procedure brought about a progressive compression of the vena porta. The application of splenoheptoplasty is a highly valuable treatment for portal hypertension as a derivative system and for intrahepatic portal hypertension as a derivative regenerative system.

Exogenous growth hormone: effects on the catabolic response to surgically produced acute stress and on postoperative immune function.

The first purpose of the present prospective randomized study was to determine if the protein catabolic response after operation could be restrained by administration of hypocaloric parenteral nutrition (HPN) plus human growth hormone (hGH). Our second aim was to determine if the administration of hGH could improve the systemic host defenses, thereby reducing the risk of infection. We performed a placebo-controlled randomized double-blind trial in 180 patients after elective cholecystectomy with or without choledochoduodenostomy (placebo = control group, n = 93; hGH-treated group, n = 87). The results obtained in this study show that positive nitrogen balance can be achieved during the postoperative period from the first 24 hours onward, with a combination of HPN support (1.0-1.5 g protein/kg/day and 900 kcal/day) together with the administration of small doses (8 IU) of hGH. The potassium balance tended to follow the same positive balance as the nitrogen balance. Our study shows that in the control group, treated only with HPN, a significant decrease in serum levels of some acute-phase proteins (retinol-binding protein, transferrin, prealbumin), albumin, total proteins, and immunoglobulins occurs by the 5th postoperative day. In the GH group the values of all the above-mentioned proteins do not change or tend to rise if compared to preoperative levels. In the GH-treated group liver function appeared to improve at the start of the study. GH and somatomedin C levels were similar in the two groups and had not changed significantly in the control group by the 5th postoperative day.(ABSTRACT TRUNCATED AT 250 WORDS)
