(1) Simultaneous recording of doctor/patient contacts and chemists' sales were made during a period of two weeks in a small self-contained community.(2) The conclusion of previous studies, using different methods, that non-prescribed medicines are taken twice as often as those prescribed by doctors, is confirmed.(3) The possibility that self-medication may conceal the onset of serious disease is discussed.
(1) The medical needs of patients staying at a holiday camp during the summer of 1974 have been analysed.(2) Medical advice was sought from a doctor about half as often as would have been expected from a comparable population at home, but the camp nurse saw three times as many patients as the doctor.(3) The clinical content of the consultation was analysed.(4) The outcome of the consultations and the role played by the nurse are discussed.

1. Human subjects received intravenous infusions of adrenaline preceded by saline, phentolamine (an alpha-blocker) or either pronethalol or propranalol (beta-blockers), in different experiments.2. Clinical measurements of blood pressure and pulse rate confirmed that alpha- or beta-blockade had been achieved.3. The rise in clotting factor VIII induced by adrenaline was blocked by pronethalol and propranalol but not by phentolamine. This indicated that the effect of adrenaline on factor VIII was mediated by beta-receptors.

Once an extremely rare neoplasm, primary lymphoma of the central nervous system (CNS) now ranks behind only meningiomas and low-grade astrocytomas in prevalence. Understanding of primary CNS lymphoma has increased greatly in recent years as a result of special immunohistochemical stains. Virtually all primary CNS lymphomas are composed of B cells. Although a viral cause has been suggested in some cases, the exact cause of the disease is still under investigation. Primary CNS lymphoma has a distinct affinity for perivascular extension. Although granular nodules may be seen at gross pathologic inspection, diffuse microscopic spread is always present, which accounts for the ability of this tumor to produce distant disease and local recurrences. The dense cellularity of the tumor and its predilection for the periventricular region also explains its typical hyperattenuated appearance on unenhanced computed tomographic scans and hypointensity on T2-weighted magnetic resonance images. Virtually all lesions enhance with contrast material. Although the overall prognosis for patients with primary CNS lymphoma remains poor, some advances have been made with radiation therapy and chemotherapy for this once uniformly fatal disease.
Rats developed mixed gliomas, oligodendrogliomas, and a few astrocytomas in response to transplacental ethylnitrosourea. The neoplastic cell composition of mixed gliomas must be defined; this study required a 20-80% admixture of neoplastic astrocytes and oligodendroglia for the diagnosis of mixed glioma. A battery of immunoantibodies, including Leu-7, S-100, and vimentin, were helpful in classifying rat gliomas, and the histologic features of each tumor type are described. Other brain tumor characteristics that may decide the outcome of carcinogenicity studies include incidence, multiplicity, latency, fatality, size, and malignancy. The size of tumors was determined by measuring their 3-dimensional volumes. Brain tumor volume was found to be highly correlated with malignancy and fatality. Systematic evaluation of the malignancy of brain tumors is an important but often overlooked adjunct method of measuring the effectiveness of a carcinogen. A system to estimate malignancy, one that grades 9 tumor characteristics and weights, each according to clinical outcome, was developed. It was found that mixed gliomas grew larger, had a shorter latency, and were significantly more malignant than were other gliomas.
The process of purifying and reconstituting transport membrane proteins generally involves the use of detergents, which often cannot be completely separated from the proteins. The effects of the zwitterionic detergent CHAPS on planar lipid bilayers have been measured, and it is demonstrated that CHAPS can induce microscopic electrical activity in the bilayers. Typical CHAPS-induced activity consists of large current bursts, often separated by intervals of quiescent activity, with no definable conductance levels. The size of the current bursts is generally increased by higher CHAPS concentration or by millimolar ATP and usually reduced by millimolar Mg2+ and micromolar ruthenium red. The response of the CHAPS-induced currents to these agents is compared to that of the ligand-gated Ca2+ release channel of muscle sarcoplasmic reticulum.
