[The "Alcomat" breath alcohol analyzer in a controlled drinking trial].

In controlled drinking tests according to a concrete forensic problem the residual-alcohol detector of the breath alcohol analyzer "ALCOMAT" was checked in relation to the actual blood alcohol concentration. The tests with non-alcoholic persons, rinsing the mouth with US-whiskey (40 ml, 30 seconds), showed first residual-alcohol indication and later blood alcohol levels up to 0.26%. The test groups with beforehand blood alcohol concentration showed either with mouth rinsing, or with real whiskey ingestion indicated devitations up to 2.07% according to 130% of the actual BAC. A correlation with the level of BAC was not found. The electronic precautionary fittings of the "ALCOMAT" are even under best possible controlled test conditions not sufficient reliable for forensic purpose.

Secretion of methanol-insoluble heat-stable enterotoxin (STB): energy- and secA-dependent conversion of pre-STB to an intermediate indistinguishable from the extracellular toxin.

The methanol-insoluble, heat-stable enterotoxin of Escherichia coli synthesized by clinical strains or strains that harbor the cloned gene was shown to be an extracellular polypeptide. The toxin (STB) was first detected as an 8,100-Mr precursor (pre-STB) that was converted to a transiently cell-associated 5,200-Mr form. Proteolytic conversion of pre-STB to STB was shown to be inhibited by the proton motive force uncoupler carbonyl cyanide m-chlorophenylhydrazone and did not occur in a secA background. After STB was detected as a cell-associated molecule, an extracellular form with identical electrophoretic mobility became apparent. The results suggest that there is no proteolytic processing during the mobilization of STB from the periplasm to the culture supernatant. The determined amino acid sequence of STB coincides fully with the 48 carboxy-terminal amino acids inferred from the DNA sequence. The 23 amino-terminal residues inferred from the DNA sequence were absent in the mature toxin.

[Carbon monoxide poisoning caused by medical error. Fatalities in connection with medical measures in Leipzig and Hannover].

There are significant differences in the frequency of CO intoxication and its relation to other not natural causes of death between the autopsy cases of Leipzig (DDR) and Hannover (BRD). While the percentage of all autopsies in Leipzig runs up to 7.6%, in Hannover it runs up to only 1.5%. Beside the well-known cases of not recognized CO intoxication at the autopsy with or without following intoxication of other persons, two concrete cases of not recognized CO intoxication with the survival of the victims discussed. The not noticed CO intoxication followed by the death of the victim is usually considered in medicolegal opinious as "medical malpractice". In cases of survived not noticed CO intoxications this opinion ist only supposed in special cases with longer anamnesis and a typical seasonal rythmic of symptoms.

[Alcohol risk in adolescent drivers--epidemiologic study of the Hannover region].

The traffic in the large cities requires special effort to young traffic participants and persons with just passed driving-license examination. Statistical investigations show an increasing partake of juveniles to alcohol. The results of the research of the protocols of the blood alcohol analyst's office at the Institute of Forensic Medicine of the MHH show an increasing share of drunken young traffic participants with blood alcohol concentrations below 1%. The special risk of juveniles by drinking and driving results not only of the reduction of the physical capacity because of the low alcohol tolerance, but especially of the reduction of the physical capacity and the personality in total.

[Two conspicuous findings in human tubular bones following longer exposure time].

A human skeleton was found in a forest after an aging time (laydays) of nearly three years. Surprisingly a larger number of living maggots crawled out of the medullary cavity of the humerus after sawing it longitudinally. Investigations of other long bones showed remnants of maggots, fly-pupas and maggot-excrements in similar cases. A humerus is found after an estimated aging time of over 100 years. The presence of a remaining epiphyseal gap indicated an age of about 20 years, whereas the extension of the medullary cavity showed a much higher age. After a long aging time, these structures show similar findings in other cases. Damage done by frost should be discussed.

[Visual function and professional preparation of persons with congenital glaucoma. IV. Complications of congenital glaucoma].

The authors evaluated complications which appeared in 113 eyes in 66 patients treated for congenital glaucoma. The most frequent early complication connected with the surgical procedure was an anterior chamber haemorrhage (in 22 eyes). Individual cases exhibited intravitreal haemorrhage (5 eyes) and uveitis (2 eyes). Among the late complications prevailed complicated cataract (in 30 eyes), less frequently nystagmus (11 eyes), intraocular infection (9 eyes), squint (17 eyes) and internal hemophthalmos.

[Isolated stab wound without preliminary stab wounds--suicide or homicide?].

The diagnosis suicidal or homicidal stab injury of the heart affords a very close cooperation of the police and the forensic pathologist. In the first six months of 1988 the institut of forensic medicine of the MHH was obliged to four cases of isolated stab wound of the heart without other injuries with the question of suicidal action. The differentiation between suicide and homicide in two fatal cases was possible, considering as well autopsy results, as the facts found at the place of death and surrounding investigation results.

[Animal experiment studies of free transplantation of striated muscles].

Heterotopic autologous muscle transplants of the left soleus muscle were performed in 39 Wistar rats. In analogy to the method described by Hakelius, the muscle was implanted into the bed of the contralateral superficial flexor muscle of the fingers with a varying prestretch of 0, 25 and 50% of its original length at rest. The animals were sacrificed postoperatively after 5, 10, 20, 40 and 60 days and their musculature examined. It was found that revascularisation and reneurotisation depended on the degree of prestretching applied. If prestretching amounted to 25 and 50%, complete degeneration of the connective tissue of the transplanted muscle occurred. However, even if prestretching was only low, i.e. between 0 and 5%, only one-third of the muscle mass regenerated and was interspersed with connective tissue of low cellular and vascular content with cicatrisation. These findings were also seen--to a lesser degree--if the soleus muscle was only denervated. We can conclude from this that restretching plays a decisive role even in the transplantation of striated muscle, as far as revascularisation and reneurotisation are concerned, and that even under the most favourable conditions only part of the transplanted muscle retains its full functional capacity. Furthermore, direct nerve anastomoses between the pudendal nerve and a branch of the femoral nerve supplying the transplanted semitendinous muscle, were performed in four Syrian goats. It was found that neurotisation of the muscle via microanastomosis is preferable to diffuse reneurotisation via the transplant bed. However, problems arose in respect of the blood supply of the transplanted semitendinous muscle, when using the technique after Holle and Freilinger.
