Drug use in dermatology.

Prescription of drugs was studied for 197 outpatients and 93 inpatients seeking treatment under the specialized services of the Dermatology Department of the Nehru Hospital attached to the Postgraduate Institute of Medical Education and Research, (PGIMER) Chandigarh. Disease patterns in both settings and drugs prescribed for them were analyzed from prescriptions, retrospectively. Brand drugs were prescribed to the tune of 60-65%, in both settings, some lacunae in prescription are discussed.

Drug usage in chest diseases.

A survey of prescribing trends was carried out on 70 admitted patients and 210 outpatients presenting themselves for treatment at the chest disease unit of the Nehru Hospital attached to the Postgraduate Institute of Medical Education and Research. Drug usage was found to be very rational and scientific, other findings are discussed.

Drug utilization studies in cardiology.

A study was carried out to assess the prescribing trends in the cardiology services of the Nehru Hospital attached to PGIMER, Chandigarh. Prescriptions of 45 inpatients and 62 outpatients were audited under the heads of number of drugs prescribed, generic versus trade name, choice of drug, its dosage, inter-dose intervals, duration and choice of dosage form. High scientific standards of prescribing were noted. Some minor lacunae observed, are discussed.

Drug utilization studies in internal medicine.

An appraisal of the prescribing trends in the outpatients and inpatients, was done in the Department of Internal Medicine of Nehru Hospital attached to the Postgraduate Institute of Medical Education & Research, Chandigarh. Prescriptions of 162 outpatients and 78 inpatients were audited under heads of total number of drugs prescribed, proprietary/nonproprietary drug prescribing; choice of drug, its dosage, dosage form, interdosage interval and duration of prescription. Good standards of scientific therapeutics were found in general. Some lacunae are discussed.

Tolbutamide kinetics in cigarette smokers in the Indian population.

A single oral dose of 500 mg tolbutamide was administered to 9 chronic cigarette smokers and 8 healthy matched control volunteers. Plasma tolbutamide half-life (t1/2 B) was shortened in cigarette smokers as compared to the nonsmokers, but the difference was not statistically significant (p greater than 0.05). However, the area under the plasma concentration (AUC O----x) and peak concentration (Cmax) were reduced significantly in smokers. Concern over enhanced metabolism of several drugs is probably warranted in cigarette smokers.

Isolated Dislocation of carpal scaphoid.

The functional role of the scaphoid in stabilising the wrist joint is discussed and the literature on scaphoid dislocations reviewed. A case of isolated dislocation of the scaphoid has been presented with the salient diagnostic features and preferred line of management which includes percutaneous Kirschner-wire fixation to ensure maintenance of the reduction.

Tolbutamide kinetics in a normal Indian population.

A single oral dose of 500 mg of tolbutamide was administered to 13 healthy volunteers. Plasma tolbutamide half-life was compared between male and female volunteers. The half-life in female subjects was longer as compared to male subjects, but the difference was not statistically significant (p greater than 0.05).

A study was carried out to assess prescribing trends in outpatients of the Nehru Hospital, Department of Internal Medicine, which is attached to the Postgraduate Hospital, Department of Internal Medicine, which is attached to the Postgraduate Institute of Medical Education and Research, Chandigarh. Prescriptions for 50 patients were audited under heads of generic versus trade name prescribing, the dosage, dosage form, dosage interval and duration of treatment; the prescriptions by and large were model ones - certain lacunae are discussed.

Effect of pesticide exposure on human microsomal enzyme induction.

1 The antipyrine half-life, metabolic clearance rate and the apparent volume of distribution were measured in six subjects handling malathion. These results were compared with six appropriate controls. 2 Occupational exposure of malathion in subjects reduced the antipyrine half-life from 9.18 +/- 2.16 h in controls to 4.73 +/- 1.65 h. 3 It is concluded that malathion exposure increases the rate of antipyrine elimination, possibly by inducing the hepatic microsomal enzymes.

Enhanced antipyrine metabolism in cigarette smokers in Indian population.

A single oral dose of mg/kg body weight of antipyrine was administered to 10 chronic cigarette smokers and to 9 appropriate control healthy volunteers. Plasma antipyrine half-life (t 1/2 beta) was shortened significantly (p < 0.05) in cigarette smokers as compared to the non-smokers control group. Antipyrine was cleared from the blood faster in cigarette smokers, with a mean increase in metabolic clearance rate from 0.637 ml/min/kg in nonsmokers to 0.831 ml/min/kg in cigarette smokers. Concern as to enhanced metabolism of other drugs is probably warranted in cigarette smokers.

Use of cold medications for upper respiratory tract infections in children.

Prescribing practices in the treatment of upper respiratory tract infections (URTI) in children were studied, with particular emphasis on overuse of antibiotics, combination cold medications, and off-label use of medicines in children. Prescriptions with the diagnosis of URTI were studied over a period of 5 months in a general pediatric clinic. Out of 224 prescriptions scrutinized, a cold medication was prescribed in 88.4% of cases and 50.9% prescriptions contained a combination preparation. Antihistamines (50.9%), alpha-adrenoceptor agonist decongestants (50.0%) and opioid anti-tussives (24.5%) were the common ingredients of cold medications. Antihistamines (35.7%) were also used alone. Of the children 18.2% were receiving a drug contraindicated or not recommended in their age group. Antibiotics were prescribed in only 8.5% of cases, and for appropriate indications, reflecting a more restrained use of antibiotics.

Adverse drug reactions among inpatients in a north Indian referral hospital.

BACKGROUND: Monitoring spontaneous adverse drug reactions is one of the epidemiological methods for assessing the safety of drugs in a hospital setting. METHODS: Data on adverse drug reactions (ADRs) were collected over a 3-year period among inpatients in a north Indian referral hospital using the spontaneous ADR monitoring system. RESULTS: A total of 317 ADRs were reported. Cutaneous reactions (38.8%) and gastrointestinal disturbances (28.4%) made up a large proportion of reported ADRs. Antimicrobial agents, including those used for antituberculosis therapy, were responsible for 47.3% of the events. Radiocontrast dyes, and antineoplastic and psychoactive agents were the other important drugs causing ADRs. No age- or sex-related differences were found in the overall rate of ADRs, though toxic epidermal necrolysis was more frequent in the elderly. CONCLUSION: The pattern of ADRs and drugs involved were largely similar to those reported in the western literature.

Load-independent analysis of a pulsatile right ventricular assist device.

BACKGROUND: Right ventricular assist devices are becoming increasingly used as both a bridge to heart transplantation and as a means of temporary support after cardiopulmonary bypass. There has also been a resurgence of interest in pulsatile devices fueled by anecdotal, clinical reports. However, a load-independent analysis of biventricular function after right ventricular assistance comparing a pulsatile versus a continuous-flow right ventricular assist device has not been performed, and we hypothesize that a pulsatile device is less detrimental to cardiac function than a conventional, nonpulsatile pump. METHODS: Sixteen dogs (20 to 25 kg) were instrumented through a median sternotomy for placement of left ventricular and right ventricular epicardial dimension transducers in the major, minor, and septal-free wall axes. Intracavitary micromanometers were placed in both ventricles as well. Baseline pressure-dimension data were collected, and the right atrium and pulmonary artery were cannulated. Right ventricular bypass with the use of a pneumatically driven pulsatile right ventricular assist device (SV = 60 ml; n = 7) or a conventional continuous-flow centrifugal right ventricular assist device (n = 9) was instituted for a 4-hour duration. Animals were then weaned from right ventricular support and decannulated. After bypass, biventricular function data were then collected. The load-insensitive stroke work-end diastolic volume relationship known as preload recruitable stroke work was derived and expressed as a fraction of baseline function along with conventional hemodynamic indexes, cardiac output, and pulmonary vascular resistance. RESULTS: Results of this analysis show no significant benefit to either right ventricular or left ventricular function (right ventricular preload recruitable stroke work index: 0.863 +/- 0.3 [pulsatile] versus 0.849 +/- 0.2 [continuous], left ventricular preload recruitable stroke work index: 0.880 +/- 0.4 [pulsatile] versus 0.821 +/- 0.3 [continuous] after pulsatile right ventricular support. Likewise, cardiac output (1.4 +/- 0.1 [pulsatile] versus 1.5 +/- 0.2 [continuous] L/min) and pulmonary vascular resistance (4.8 +/- 1.0 [pulsatile] versus 3.2 +/- 1.1 [continuous] Wood Units) were not significantly different in either study group. CONCLUSIONS: We conclude from these data that pneumatically driven pulsatile right ventricular assist devices provide no additional benefit to myocardial performance beyond that of conventional, nonpulsatile pumps. Further studies investigating a speculative benefit from pulsatile circulatory support are necessary to further define a potential role for these novel devices.
