On the homogeneity of personality disorder clusters.

This study was conducted using as subjects persons seeking psychiatric care in a public intake facility. Patients who received a diagnosis of a personality disorder (PD) on axis II constituted the study sample. The study is unique in that it attempts to analyze in a treatment population the relationships between all of the personality disorders (PD) stipulated in DSM-III with emphasis on their relationships to the associated features recorded in the remaining axes of DSM-III. The specific aim of the study was to determine the variation that exists with respect to clinical and demographic variables across the PD clusters categorized in DSM-III and across the PD types of each of the DSM-III PD clusters. In general, considerable inhomogeneity was shown within and across clusters in both demographic and clinical variables, although differences among clusters were noted as well. Cluster B differs prominently from its cohorts with respect to demographic and clinical characteristics. Some of the limitations of these results are emphasized.

Sociocultural and clinical characteristics of patients with comorbid depressions: a comparison of substance abuse and non-substance abuse diagnoses.

Patients with three varieties of major depression (MD) were compared with respect to sociocultural and clinical characteristics. The patients sought psychiatric evaluation in an intake setting during a 6-year interval. The groups that were compared included MD (sole axis I diagnosis; N = 3,913); MD with a comorbid non-substance abuse ([MD-nonSA] N = 594); and MD with a comorbid substance abuse ([MD-SA] N = 690). Prominent demographic differences were found in the three groups, with males, lower social class status, younger age, and African American ethnicity being more prominent in MD-SA. The four demographic variables, axis I status, axis II diagnosis, and axis III diagnosis constituted the independent variable in an analysis of variance (ANOVA) main-effects comparison of social function (during last year and currently in three areas) and type of depression symptoms (somatic and psychological). Of the demographic variables, age and social class proved to have significant effects on social function and psychological symptoms. Each of the three "syndromic" axes (Axis I, Axis II, and Axis III) had a significant impact on social function and psychological symptoms. Only Axis III produced differences in somatic symptoms. The social and cultural implications of these results are discussed.

Chimaeric HBV core particles carrying a defined segment of Puumala hantavirus nucleocapsid protein evoke protective immunity in an animal model.

Hantaviruses are rodent-born agents which are pathogenic in humans causing haemorrhagic fever with renal syndrome or hantavirus pulmonary syndrome. To induce a protective immunity against a European hantavirus (Puumala) we constructed chimaeric hepatitis B virus (HBV) core particles carrying defined fragments of the Puumala virus nucleocapsid protein. After immunisation of bank voles, the natural host of Puumala virus, with core particles possessing an insertion of the N-terminal part of Puumala virus nucleocapsid protein, four of five animals were protected against subsequent virus challenge. The results show that the major protective region of the nucleocapsid protein is located between amino acids 1 and 45 and that chimaeric HBV core-like particles are useful carriers of foreign protective epitopes.

[Spontaneous hematoma of the abdominal wall: diagnostic error of abruptio placentae].

An emergency cesarean section was performed in the 38th week of pregnancy in a patient with acute abdominal pain in the left upper quadrant. The ultrasound picture was interpreted as a retroplacentar hematoma. Cardiotocography demonstrated fetal tachycardia which was interpreted as asphyxia. A diagnosis of abruptio placentae was made, but was finally ruled out when the cesarean section was performed. The intraabdominal exploration revealed a large hematoma of the rectus sheath with about 800 ml of blood. After evacuation and drainage the postoperative recovery was uneventful. Sudden rupture of a deep epigastric vessel with formation of an abdominal wall hematoma also mimics intraabdominal conditions such as appendicitis, diverticulitis, cholecystitis, tumours and visceral injuries.

[Congenital fistula of the raphe penis].

One case of a congenital fistula of the raphe penis with gonorrhoea is reported. In this case spectinomycin seemed to fail in therapy, because pus was still remaining in the fistula, but not in the urethra. As the antibiotic resistogram did not prove our assumption, we tried the double dose and succeeded in killing the bacteria.

Detection times of natural teeth and endosseous implants revealed by the method of reaction time.

Reaction times were employed to assess whether implants have longer detection times than natural teeth. A soft push was applied to an endosseous implant or to the corresponding natural tooth. Patients were asked for a speeded manual response upon the detection of the push. The temporal interval from the onset of the push to the onset of the manual response denoted the reaction time. This measure was only slightly longer for endosseous implants than for natural teeth, indicating that an implant's ability to detect rapid load changes is almost unimpaired.
