Surgical treatment of tracheal stenosis.

During the last 3 1/2 years 8 patients underwent segmental resection of the trachea and one patient underwent resection of the carina at the Department of Surgery, University Hospital, Köln. Follow-up investigation in 6 month intervals showed good results in 6 patients. The complicated course of 1 of our patients with tumor infiltration of the trachea is discussed. In this case a large esophagotracheal fistula was covered with a vascularized esophageal segment. Suggestions for surgical indications in segmental resection of benign esophageal stenosis are outlined.

[Indications for the correction of tracheal stenosis caused by prolonged artificial ventilation of polytraumatized persons (author's transl)].

Pronounced, short-length narrowing of the trachea combined with dyspnoea at rest is an absolute indication for partial resection. For stenoses of moderate degree the indications for surgical intervention are as yet not clearly defined. They depend not only on the clinical, roentgenological and tracheobronchoscopic findings, but also on the results of lung function tests. Changes in respiratory function become noticeable at reduction of under 30% of the tracheal diameter. Experiments with artificially induced stenosis have shown an exponential rise in flow resistance when the diameter of the trachea was narrowed to below 8 millimetre. observations made in patients have proved that the experimental results also apply clinically. The indications for surgery of a tracheal stenosis should be made dependent on the results of the determinations of flow resistance, whole body plethysmography and expiratory and inspiratory volume/1 sec. Flow resistance exceeding 5 cm H2O/1 sec, equivalent to narrowing to under 8 mm in diameter, is considered to be an indication for partial resection.

[The effect of fominoben and oxyethyltheophylline in combination on pulmonary function and pulmonary hemodynamics in chronic-obstructive diseases of the airways (author's transl)].

Mechanics of breathing, gas exchange and pulmonary hemodynamics were investigated in 10 patients with chronic-obstructive lung diseases before and after treatment with oral doses of 160 mg 3'-chloro-2'-(N-methyl-N-[(morpholino-carbonyl)methyl]-aminomethyl)benzanilide hydrochloride (fominoben, Noleptan) and 300 mg oxyethyltheophylline given three times daily. The data of lung function showed a slight decrease of airway obstruction during the treatment. Hyperinflation of the lung, restriction, gas exchange were not significantly changed on the average. The main effect was an improvement of pulmonary hemodynamics: the pathologically increased mean pressures in the pulmonary artery were normalized after treatment.

[The intramural duodenal diverticulum, a topical variation of the intraluminal duodenal diverticulum (author's transl)].

The intramural duodenal diverticulum is a topical variation of the intraluminal duodenal diverticulum. It is manifested not before adult age by noncharacteristic upper abdominal symptoms. Diagnosis is made by radiologic examinations, the therapy is excision of the diverticulum. Structure and topography of the diverticulum are explained by its embryological development.

Changes of the CO-diffusing components of the lung in patients with coronary artery disease (author's transl).

In 32 patients with angiographically proved CAD the diffusion parameters of the lung (intracapillary pulmonary blood volume, Vc, and diffusion capacity of the alveolo-capillary membrane, Dm, respectively the reciprocal value membrane resistance) were measured. As reference the pre-a-LVDP was used, as it represents more adequately than the LVEDP the pressure acting on the alveolo-capillary membrane. There was a significant positive correlation between the Vc and the pre-a-LVDP as well as a highly significant positive correlation between the diffusion resistance of the alveolo-capillary membrane and the pre-a-LVDP. This corresponds to a pulmonary congestion with an increase of Vc and an interstitial edema with an increase of the diffusion resistance of the alveolo-capillary membrane. The Dco may be unchanged in spite of a marked increase of LVDP, since the decrease of the membrane component is accompanied by an increase of the blood component of diffusion. Only the differentiation of the CO-transfer in its components Vc and Dm gives an information about changes of the hemodynamics in CAD.
