An 80-year-old woman developed osteoarthritis of the knees as a complication of her obesity. Indomethacin treatment precipitated gastric ulceration, which led to the formation of a gastro-colic fistula.

A questionnaire was sent to the principals of 20 schools for the hearing-impaired across Canada seeking data on the types and numbers of professional support staff utilized by them. In addition the schools were asked to express opinions regarding the additional number of professionals in various disciplines that should be employed. The most commonly employed professionals were audiologists and the least common otolaryngologists. The greatest need was said to be for a vast increase in the number of speech therapists available, although among them the schools employed less than the equivalent of one such full-time person.

Plasmid pPL7065 ( approximately 4.7 x 10(6) daltons; approximately 20 copies per chromosome) determines the production of, and immunity or resistance to, a killing activity in strains of Bacillus pumilus. Plasmid pPL7065 is compatible with plasmid pPL576 ( approximately 28 x 10(6) daltons; approximately 2 copies per chromosome).
Five male subjects performed exercise at 33, 66, and 95% of their maximum power output on three occasions in random order. Each study was preceded by a 3-h period in which capsules were taken by mouth, containing either CaCO3 (control, NH4Cl (acidosis), or NaHCO3 (alkalosis) in a dose of 0.3 g/kg body wt; preexercise blood pH was 7.38 +/- 0.015, 7.21 +/- 0.033, and 7.43 +/- 0.029, respectively. Exercise was continuous and maintained for 20 min at the two lower power outputs and for as long as possible at the highest. Compared with control (270 +/- 13 s), endurance time at the highest power output was reduced in acidosis (160 +/- 22 s) and increased in alkalosis (438 +/- 120 s). No differences were observed for central cardiovascular changes in exercise (cardiac output, frequency, or stroke volume). The respiratory changes expected from changes in blood pH were observed, with a higher alveolar ventilation in acidosis. At all power outputs arterialized venous lactate was lowest in acidosis and highest in alkalosis. Plasma glycerol and free fatty acids were lowest in acidosis. Changes in blood [HCO3-] and pH were shown to have major effects on metabolism in exercise which presumably were responsible for impaired endurance.
It is hypothesized that alcoholics possess a perceptual-neurophysiological sensitivity which renders them vulnerable to the effects of information or stimulus overload: the resort to alcohol, therefore, may represent an adaptive attempt to restore physiologic homeostasis through the pharmacologic "dampening" properties of this drug. Twelve alcoholic subjects were assigned to three different levels of audio-visual sensory stimulation (no, medium, high) and three different doses of alcohol (placebo, low, high) according to a balanced incomplete block design. The results indicated trend level and significant interactions between alcohol and sensory level effects on craving intensity and systolic blood pressure, respectively, as well as a number of other intriguing findings. Should the findings of this pilot study be replicated, they will have important implications for the understanding of alcoholism.

A technique is described for estimating cell-mediated damage to the larval stages of Schistosoma mansoni that occur in the mamalian host (schistosomula), by measuring release of 51Cr from labelled organisms. This technique, although widely used for assaying cytotoxicity to single cell suspensions or monolayers, has not previously been applied to a multicellular parasite. It is more objective than microscopical assays, and allows the processing of larger numbers of samples. The new method has been used for the detection and quantification of cell-dependent cytotoxic antibodies in infected baboons. The effector cell in normal baboon peripheral blood, as in man, is associated with a neutrophil- and eosinophil-enriched fraction. Cytotoxic antibodies appear in the serum about 4 weeks after a primary infection, rising to a peak at 7-10 weeks, and then declining. In some animals, a second rise towards 30 weeks was observed. There were no consistent differences between groups of baboons exposed to 1000 cercariae, 200 cercariae, and 200 cercariae administered at each of five monthly intervals. The possible relationship between cell-dependent cytotoxic antibodies and resistance to reinfection in infected animals is discussed.
Sequential topical application of fluorouracil and 0.5% triamcinolone acetonide cream is as effective in the treatment of actinic keratoses as fluorouracil alone, but the combination obviates the unpleasent irritation caused by fluorouracil. Diluted (0.1%) triamcinolone acetonide cream preparations are ineffective in the suppression of the associated inflammation. There is no detectable difference in the number of new actinic keratoses between the combination therapy and fluorouracil alone. These findings demonstrate that the degree of success with fluorouracil therapy in actinic keratosis is not related to the degree of inflammation associated with the treatment and are consistent with a chemotherapeutic explanation of fluorouracil's effect on actinic keratosis.

The T4 ultraviolet endonuclease was previously shown to produce strand incisions (nicks) in ultraviolet-irradiated DNA on the 5' side of thymine dimers. The present studies demonstrate that the purified endonuclease creates 3'-OH and 5'-P termini at the sites of nicking. Photoreactivation of ultraviolet-sensitive sites, thereby demonstrating directly endonucleause has a molecular weight of approximately 18,000 and attacks ultraviolet-irradiated single-stranded Escherichia coli and M-13 DNA.
