Interaction of rat ascites hepatoma cells with cultured mesothelial cell layers: a model for tumor invasion.

Interactions of rat ascites hepatoma cells with primary cultured layers of rat mesentery-derived cells were studied. The mesentery-derived cells were isolated from rat mesentery and cultured in Eagle's minimum essential medium with a 2-fold concentration of amino acids and vitamins supplemented with 10% calf serum. The primary cultured cells, consisting mainly of mesothelial cells in polygonal shape, forms a "paving stone" sheet. Upon seeding the tumor cells on the mesentery-derived cell layers, three different types of tumor cell growth were observed. Type 1 was the formation of piled-up tumor cell nests on mesothelial cell layers. Type 2 was the formation of flattened tumor cell islands underneath mesothelial cell layers. This island formation was clearly observed under a phase contrast microscope 2 days after the tumor cell seeding. Protrusion of cellular processes of the tumor cells beneath mesothelial cells was occasionally seen. Type 3 was the growth of tumor cells in suspension. These types of tumor cell growth closely resemble those in the peritoneal cavity observed after i.p. implantation of the tumor cells. When the tumor cells recovered from the blood of tumor-bearing rats were seeded, flattened tumor cell islands were formed 15 times more frequently than when the tumor cells isolated from host peritoneal cavity were seeded. Shortly after the appearance of small flattened tumor cell islands, a distinct morphological change of mesothelial cells from polygonal to spindle shape was seen preferentially at the marginal area of the cell layers (a partial retraction of cell edges). The retraction of mesothelial cells was induced not only by seeding the tumor cells but by adding the tumor ascites fluid or the medium conditioned by the tumor cell culture. The morphological change was reversed by changing the culture medium to remove the effectors. These results indicate that the system described in this study can provide a useful model to study tumor cell invasion.

Radiation therapy for primary non-Hodgkin's lymphoma of the head and neck in stage I-II.

From September 1977 through January 1983, 34 previously untreated cases with primary non-Hodgkin's lymphoma of the head and neck in stage I-II were treated at our department. 16 cases were classified into stage I and 18 into stage II. Patients were treated with 4 MV X-ray (40 to 45 Gy) which encompassed Waldyer's ring and supraclavicular region with or without chemotherapy. The survival rates at three years were 93% and 50% in stage I and II, respectively. Stage, surface markers (T- and B-cell analysis) and initial primary site and the level of lymph node involvement were important prognostic factors. Chemotherapy in pre- or post-irradiation was essential, especially for cases in stage II. It was also indicated that the adequacy of radiation portal with special reference to anatomical consideration was necessary.

The effect of N-nitrosobis(2-hydroxypropyl)amine on pulmonary neuroepithelial cells in Syrian golden hamsters.

The present study was carried out for determination of the effect of N-nitrosobis(2-hydroxyproyl)amine (BHP) on pulmonary neuroepithelial cells (NECs) in Syrian golden hamsters. This study was also carried out for elucidation of the histologic, histochemical, and ultrastructural features of NECs in the normal lungs of the hamsters. In the normal hamster lungs, NECs occurred exclusively in bronchioles and in bronchi lacking cartilage, appearing in groups as neuroepithelial bodies (NEBs). The number of NEBs was dependent on the age of the animals. Neonatal lungs contained large numbers of NEBs, but these seemed to disappear or remain as inconspicuous cell nests in adult lungs. Chronic BHP exposure (21 weeks) induced subepithelial spherical or fungating nodules composed of proliferating NECs, which projected into the bronchial lumens. The nodules appeared to arise from inconspicuous cell nests, which were rudiments of neonatal NEBs.

Argyrophil small cell carcinomas of the uterine cervix.

Thirteen cases of argyrophil small cell carcinomas of the cervix were reviewed. Seven of nine patients, who have been followed up for more than 5 years, died. These tumors were found to be aggressive in their behavior. Immunohistochemical studies to localize several peptide hormones were positive for gastrin in two of five cases.

Carcinoid tumor of the gallbladder associated with adenocarcinoma.

A case of carcinoid tumor of the gallbladder associated with adenocarcinoma in a 56-year-old man is reported and a review of the literature is made. The tumor was a polypoid mass with a size of 5.5 X 4.0 X 2.8 cm. Histologically, the tumor showed carcinoid and adenocarcinoma with areas of mucous change. Tumor cells containing argyrophil granules were observed in both carcinoidal and adenocarcinomatous areas, but no argentaffin granules were detected in either of the neoplastic areas. Some of the tumor cells had both argyrophil granules and mucin in the same cytoplasm. The electron microscopic study revealed several tumor cells containing neurosecretory granules; however, no clinical signs of hormonal activities of the tumor were observed. The patient died of generalized bone metastases 16 months after surgery. This appears to be the second case of composite tumor of the gallbladder.

Glycosaminoglycans in human lung cancer.

The quantitative changes of glycosaminoglycans in tumor tissue of human lung cancers (2 squamous cell carcinomas, 4 adenocarcinomas and 5 small cell carcinomas) were studied. The total amount of glycosaminoglycans in human lung cancer tissues increased 1.4 to 4 times in comparison with that in normal lung tissues. The increase in tissue content of glycosaminoglycans was accompanied by an increase in the chondroitin sulfate level in every histologic type of lung cancer, as well as by a marked increase in hyaluronic acid level in squamous cell carcinomas, and a moderate increase in its level in small cell carcinomas. The concentrations of dermatan sulfate and heparan sulfate in lung cancer tissues did not show any significant changes compared with those in normal lung tissues. The increase in total amount and changes in the composition of glycosaminoglycans in human lung cancer tissue were closely related to the histologic type of the tumor.

Diffuse leiomyomatosis of the esophagus.

A case of diffuse leiomyomatosis of the esophagus which was successfully resected is presented. The patient was a 44-year-old man who had an abnormal filling defect, which was incidentally discovered during esophagography, and was observed for two years. Although he remained asymptomatic, an operation was performed because of increase in the tumor's size. The lesion consisted of many confluent myomatous nodules throughout the thoracic portion of the esophagus, mostly seen in the circular layer of the muscularis, and some in the muscularis mucosa. Histologically, these nodules were leiomyomas. This is a very rare condition, and only a dozen similar cases have been reported so far. A brief discussion is made on the definition of diffuse leiomyomatosis of the esophagus.

[Argyrophil cell of the endometrial adenocarcinoma -light and electron microscopic observations (author's transl)].

Fifty one endometrial adenocarcinomas were used in this study. For light microscopic observations paraffin section from 43 cases were processed with H and E, PAS, and Grimelius argyophil reaction. Besides, 21 of these endometrial adenocarcinomas (25.6%) showed positive Grimelius' argyrophil reaction. With regard to the degree of histological differentiation, these 11 cases were classified as G1 or G2 and all G3 cases (2 cases) revealed negative for Grimelius argyophil reaction. Under electron microscope, in 3 cases out of 21, many small granules reminiscent of neurosecretory type granules mostly round in shape with high electron dense core so often surrounded by a limiting membrane were detected in some of the endometrial carcinoma cells having argyophil granules under light microscope. Since the distribution of these granules was assumed to be random however, the clusters of granules so often located in the groundplasm of subapical portion and subnuclear area near the epithelial basement membrane of the glandular cells. In high magnification, they consisted of an aggregation of fine granular substances with high electron density. Based on the morphological characteristics we made a comparative study between argyophil cell carcinoma of the endometrium and apudomas of the uterine cervix.

Ductal carcinoma of the pancreas. Rationales for total pancreatectomy.

In order to evaluate total pancreatectomy as a surgical procedure for ductal carcinoma of the pancreas, a histopathological analysis was made on 18 resected specimens with special regard to the pattern of cancer growth in the pancreatic tissue. In five of them there was no lymphatic involvement or extrapancreatic invasion, but cancer extended continuously to the tail along with the pancreatic ducts and reached the end of the ducts in three cases. All 11 patients treated with Whipple's procedure died of recurrence, while of four total pancreatectomized patients, one with continuously invasive cancer to the end of the pancreatic duct has been living more than eight years postoperatively. We believe that total pancreatectomy for this type of "intraductal spreading cancer" without invasion beyond the pancreas is indicated as a radical procedure.

Malignant melanoma producing serotonin.

A case of malignant melanoma, the tumor of which seems to produce serotonin, is presented. Neither hyperserotoninemia nor carcinoid syndrome was observed in the clinical course. An autopsied sample of the metastatic lung tumor contained a significantly higher concentration (1.35 microgram/g wet weight tissue) of serotonin than that of normal lung (below 0.5 microgram wet wt.). Tryptophan hydroxylase activity, the key enzyme of the biosynthesis of serotonin, was substantial in the same tumor sample when assayed by a sensitive radioisotopic method, supporting the productivity of the amine of the tumor. The ability of melanoma to produce serotonin could be attributed to its neural crest origin.

Primary adenoid cystic carcinoma of the esophagus: report of a case.

A case of primary adenoid cystic carcinoma of the esophagus is reported. A 51-year-old male patient had a tumor in the lower third of the esophagus which was incidentally found during an examination for cholelithiasis, and resected successfully. The tumor exhibited a polypoid appearance covered by normal esophageal epithelium, localized entirely in the submucosal layer, and morphologically identical to adenoid cystic carcinoma in the salivary glands. The patient is still alive and well three and one-half years after surgery. This seems to be a typical case of adenoid cystic carcinoma of the esophagus arising from the submucosal esophageal gland.

Carcinoembryonic proteins in gastric carcinoma metastatic to the liver.

Two cases of liver metastases from gastric carcinoma are described in which the simultaneous occurrences of alpha-fetoprotein (AFP), carcinoembryonic antigen (CEA) and carcinoplacental alkaline phosphatase (CPALP) were demonstrated in the sera and tumor tissues. AFP was detected not only in the tumor tissues but also in the liver tissue adjacent to the tumor, while the other 2 carcinoembryonic proteins were not detected in the non cancerous liver tissues. The characteristics of CPALP in Case 1 were almost similar to the Nagao isoenzyme, based on enzyme tests involving L-leucine, L-phenylalanine and EDTA inhibitions, heat-stability and Michaelis constant, except for electrophoretical slower moving, while that in Case 2 were identical to variant type CPALP (Warnock).
