Human nasopharyngeal carcinomas positive for Epstein-Barr virus DNA in North America.

Nasopharyngeal carcinomas (NPC) from 2 black patients and 1 Caucasian patient were positive for Epstein-Barr virus (EBV) DNA. Of the tumors, 2 were lymphoepitheliomas (undifferentiated NPC) and 1 was a moderately differentiated NPC. All 3 patients had high IgG titers against EBV early antigen and high IgG and IgA titers against virus capsid antigen (VCA). In one patient, the levels of anti-VCA IgA were different than those of anti-VCA IgG over the course of the disease. Our data support the association of EBV and NPC in North America.

Metastases to parotid nodes.

The differential diagnosis of masses in the preauricular and retromandibular regions includes a number of diseases in addition to primary tumors of the parotid. The lesions most commonly misdiagnosed as parotid tumors are intraparotid lymph nodes involved with inflammatory or neoplastic disease. Metastatic tumors in parotid nodes are unusual but must be considered. We present 12 patients with isolated metastases to parotid lymph nodes. Nine of the patients had primary tumors in the local afferent lymphatic bed. Three patients had metastases from unknown or distant sites. The majority of tumors that metastasize to the parotid are of cutaneous origin. Six of the patients had squamous cell carcinomas, three had adenocarcinomas, two melanomas and one a small cell carcinoma. The treatment of parotid metastases from local tumors is surgical removal of the parotid and associated regional nodes with postoperative irradiation therapy in certain instances. Management of the facial nerve should follow those principles appropriate for primary parotid tumors.

Zenker's diverticulum- carcinoma of the esophagus?

Malignant change may occur in a pharyngo-esophageal diverticulum and although it is rare, a small number of cases has been reported. A similar case is reported here to contribute to the awareness of the existence of such an entity.

Frontometaphyseal Dysplasia. Evidence for dominant inheritance.

A 10-year-old boy with mixed bilateral hearing loss and unusual facies was found to have frontometaphyseal dysplasia. He has prominent supraorbital ridges, height-span disproportion, dental abnormalities, thick clavicles, pectus excavatum, winged scapulae, joint contractures, and generalized muscular underdevelopment. Roentgenograms show supraorbital hyperostosis, antegonial notching of the mandible, flared ilia, contraction of the midpelvis, flattened vertebrae, deformities of the ribs posteriorly, flared metaphyses of the long tubular bones, and greatly widened and elongated metacarpals, metatarsals, and phalanges. His mother has prominent supraorbital ridges, distinct scoliosis, contractures of the fifth fingers, and conductive hearing loss. She also has many of the roentgenographic features of frontometaphyseal dysplasia. Frontometaphyseal dysplasia in a mother and son strongly suggests a dominant mode of inheritance.

Multiple head and neck malignancies.

Case histories of 47 patients with multiple head and neck neoplasms were reviewed at the Henry Ford Hospital. Data were obtained on the site of the malignancy and treatment program. Follow-up data have been good, utilizing clinic records. Several patients have had more than two malignancies of the head and neck, and one patient has just completed treatment for his fifth epidermoid carcinoma. The incidence of carcinoma of the esophagus is high in this series (16 of 47 patients). The literature is reviewed in relation to the present findings. Follow-up care is described at the Henry Ford Hospital.
