Interdigitating cell sarcoma (ICS). Evidence of interdigitating cell origin, immunocytochemical studies with monoclonal anti-ICS antibodies.

Three independent mouse monoclonal antibodies (mAbs) ID1 (IgG3), ID2 and ID3 (IgM) were raised against whole cells of a surgically resected human interdigitating cell sarcoma (ICS). In immunoperoxidase staining, these mAbs strongly stained the cytoplasm of ICS neoplastic cells as well as interdigitating cells in normal lymphoid tissues. These mAbs also detected monocyte/macrophages and dendritic cells, although their staining was highly variable depending on tissue distribution of the cells. Additional immuno-histological and enzyme histochemical study revealed that the neoplastic cells of ICS had cytoplasmic acid phosphatase and membranous alkaline phosphatase activity, and also possessed S100 beta protein, Ki-1 antigen. DAKO-macrophage antigen, and weak vimentin activity. Neither rearrangement of immunoglobulin heavy chain gene nor of T-cell receptor genes was detected in the DNA of ICS by Southern hybridization. These observations provide further confirmation of our previous finding (Nakamura et al. 1988, 1989) that the origin of ICS is interdigitating rather than lymphoid cell, and indicate that our mAbs could be useful as a cellular differentiation marker of interdigitating cells and for diagnosis of ICS.

Changes in cerebral hemodynamics following encephalo-duro-arterio-synangiosis (EDAS) in young patients with moyamoya disease.

To evaluate the effect of encephalo-duro-arterio-synangiosis, (EDAS), we obtained follow-up angiograms and measured regional cerebral blood flow in 21 young patients with Moyamoya disease. Carotid fork stenosis continued to progress after EDAS, although angiography demonstrated a marked increase in the number of middle cerebral artery branches via implanted arteries. Preoperative cortical blood flow was lower than normal. The post-EDAS increases in hemispheric and cortical flow were significant in patients with transient ischemic attacks, but not in patients with infarction. The increase in cortical flow at the site of EDAS was first noted 2 weeks after EDAS.

Adolescents' dropout from individual psychotherapy--is it true?

Although adolescent patients are said to have a propensity to drop out from psychotherapy, and clinical experiences seem to support this view, few studies have systematically examined this issue. However, a review of researches on adults' dropout shows that dropout is a serious problem for psychotherapy with adult patients. Moreover, available evidence on adolescents' dropout suggests that there is no significant difference in the rates of dropout from psychotherapy between adolescent patients and adult ones. In order to understand the specific quality of adolescents' dropout, the termination of psychotherapy with an adolescent patient is described and discussed. The view is put forward that it is because of the strong counter-transference feelings evoked in the therapist, when his adolescent patient leaves him prematurely, tends to maintain the impression of adolescents' proclivity to drop out from psychotherapy.

Unruptured aneurysms in patients with transient ischemic attack or reversible ischemic neurological deficit. Report of eight cases.

Thirty-five patients with unruptured aneurysms were treated between 1981 and 1987. Eight of them had either transient ischemic attacks or reversible ischemic neurological deficits as their presenting symptoms. Six of the eight patients underwent direct aneurysm surgery. All eight patients are well, and have had no recurrent attack during the follow-up period ranging from 2 to 7 years. The feasibility of surgical treatment for such aneurysms associated with reversible ischemic symptoms is discussed.

[The c protein fractions of group B streptococci: its distribution and relation to heat-labile antigens for the agglutination method].

Distribution of the c protein fractions of group B streptococci and relation to heat-labile antigens for agglutination were investigated. The results were summarized as follows: 1. Analysis of antigenicity of the c protein in group B streptococcal strains isolated revealed that 74% of the type Ia/c strains carried only alpha antigen, and 76% of the type Ib/c strains contained both alpha and beta antigens. Component of the c protein fractions in most of type Ia/c strains isolated was different from that in reference strains. 2. Of 37 Ia/c alpha strains, 45.9% were determined non-typable, 32.4% were Ia/W, 13.5% were Ia/S, and 8.1% were Ia/Q in the agglutination for heat-labile antigens. Of 8 Ia/c alpha beta strains, 87.5% were Ia/Q, and 12.5% were Ia/SW. Of 5 Ia/c beta strains, 60% were Ia/Q, and 40% were Ia/QW. Of 19 Ib/c alpha beta strains, 68.4% were Ib/-, and 31.6% were Ib/S. Of 5 Ib/c alpha strains, 60% were Ib/-, and 40% were Ib/S. One Ib/c beta strain was Ib/-. 3. Because trypsin-sensitive portions of the beta antigens were lost, preparation of antigens for agglutination by pancreatic digestion could not exactly reflect results of the precipitation method.

Dual mechanism for stimulation of glutamate transport by potassium ions in Streptococcus mutans.

An ATP-driven primary transport system operative for L-glutamate or L-aspartate in Streptococcus mutans is, through the entire pH range from 5.5 to 8.5, specifically stimulated by extracellular potassium ions. The stimulation by potassium ions observed in the low pH range between 5.5 and 7 has been interpreted to be due to potassium ion-dependent regulation of the intracellular pH (the first mechanism). In the high pH range from 7 to 8.5, on the other hand, the present study demonstrates that potassium stimulation is essentially not associated with such intracellular pH regulation. This conclusion is based on our observation that potassium stimulation in the high pH range is insensitive to a proton conductor, carbonyl cyanide-p-trifluoromethoxy-phenyl-hydrazone. Since none of the other monovalent cations, including sodium, rubidium, ammonium, and Tris ions, could replace potassium ions in significantly stimulating glutamate transport, it is most likely that the influx of potassium ions specifically cancels the membrane potential derived by movement of glutamate with the net negative charges across a membrane and thus facilitates transport (the second mechanism). The second mechanism appears to be operative even in a low pH range, in addition to the first mechanism.

An appropriate ingestion volume of oral sulfa drug suspension in pigs.

The influence of ingested volume of a sulfa drug suspension, sodium sulfamonomethoxine (SMMNa), on the oral pharmacokinetics was studied in pigs, with regard to bioavailability and gastric emptying. Eighteen pigs, weighing 30-70 kg, were used. Phenol red solution was used for the evaluation of gastric emptying study. SMMNa suspension was used for pharmacokinetic study. Both of these fluids were administered by natural swallowing. Three experimental groups were constructed: G-I; 5 ml/kg of the test fluids to starved animals, G-II; 5 ml/kg of the test fluids to fed animals and G-III; 20 ml/kg of the fluids to fed animals. The glucose glycine electrolyte solution (GGES) was used as the vehicle for both the compounds. Six pigs, having duodenal cannula, were used for the study of gastric emptying. The gastric emptying rate was rapid in G-I, relatively rapid in G-III, and slow and variable in G-II. In agreement with the result of gastric emptying study, the values of Cmax and tmax were high and rapid in G-I, relatively high and rapid in G-III, and low and slow in G-II. Accordingly, the voluminous ingestion of drug suspension can facilitate the gastric emptying, in turn may make the oral absorption of the drug rapid-and-uniform. The 20 ml/kg volume of sulfa drug suspension may practically be recommended for the oral administration in pigs.

[Clinical study on the prognosis of complete denture wearers covering 10 years. 4. Masticatory efficiency].

The purpose of this study was to estimate masticatory efficiency of complete denture wearers. The subjects were 72 patients, treated before 5-10 years at students' clinic of Tsurumi University, School of Dental Medicine. The masticatory efficiencies with peanuts and Kamaboko were calculated. Results 1. The mean masticatory efficiency with peanuts for 63 patients was 35.8%, and with Kamaboko (steamed fishpast) for 64 patients was 94.5%, in comparison with natural dentition. 2. The mean masticatory efficiency with peanuts was 124.3%, and with Kamaboko was 99.6%, for complete denture wearers. 3. Comparing the measured value of masticatory efficiency with 5 to 10 years wearing period, there was significant difference in the masticatory efficiency between peanuts and Kamaboko. 4. There was significant difference in the masticatory efficiency with peanuts between the group needed new denture and the group not needed but there was not significant difference with Kamaboko.

[Study on prosthodontic procedure in patients with unilateral cleft lip and palate--effects of various extensions of splint on tooth-borne ability of teeth adjacent to the cleft].

It is mandatory that the prosthodontic devices for the cleft palate patients not only prevent the relapse of the corrected arch and teeth by orthodontic and/or surgical intervention but also equilibrate the tooth-borne ability between the upper and the lower jaw. The purpose of this paper is to set up a criterion for the extension of the splint from the point of the tooth-borne ability. Four patients with unilateral cleft lip and palate were examined for the maximal biting force at the tooth adjacent to the cleft in each alveolar segment in the case of various extensions of the splints. The findings were as follows: 1. The maximal biting force increased significantly in every splint, compared with the non-splint. 2. The maximal biting force increased significantly in the splints where the neighbouring tooth in the same segment was involved, compared with that in the splints where only the tooth adjacent to the cleft was involved, even if the tooth in another segment increased in number for splinting. 3. The maximal biting force did not increase significantly in most splints where three teeth in the same segment were involved, compared with that in the splints where two teeth in that segment were involved.

[Trigeminal-related peptides and mammalian intraocular muscles].

Effects of substance P (SP) and vasoactive intestinal peptide (VIP) on bovine intraocular muscle tissues were studied, using isolated strips. While SP generally contracted the iris sphincter, exogenous application of SP did relax the iris sphincter muscle when its tone was highly developed. SP had no significant influence on the bovine dilator, and VIP lowered the tone of the iris muscles. The ciliary muscle tone was not altered by these peptides, but the electrically evoked response of the ciliary muscle was enhanced in the presence of VIP. These results suggest possible different roles of these trigeminal nerve-related peptides (SP, SK, VIP) on intraocular muscle tissues.

[A case of S 100 protein-positive large cell carcinoma--its pathological study].

We have experienced a case of S 100 protein positive large cell carcinoma of the lung which metastasized to the jejunum. The present case had a somewhat morphological resemblance to a Ki-1 lymphoma and/or an interdigitating cell sarcoma, and its differential diagnosis was problematic. This type of case, to our knowledge, is rare in the literature. The morphologic, immunohistologic, and electron microscopical findings of the neoplastic cells are described in this study.

[Ultrasonic detectability of multiple small hepatic nodules].

We analyzed the ultrasonic detectability of multiple daughter nodules accompanied by 18 hepatocellular carcinomas and multiple small nodules in 11 metastatic liver tumors identified by infusion hepatic angiography. In the cases of hepatocellular carcinoma, ultrasonic detectability of daughter nodules was remarkably limited. If any daughter nodules are detected by US, there is the great possibility that many other daughter nodules are undetected. However, US is effective in detecting small nodules in the metastatic liver tumor.

[Application of HPLC to measurement of plasma histamine in bronchial asthma].

We measured the plasma histamine concentration in patients with bronchial asthma by the post-column derivatization method developed by Yamatodani. The mean fasting plasma concentrations of histamine determined for bronchial asthma and control groups (male and female) were 1.93 +/- 0.29 (male) and 1.37 +/- 0.15 (female) pmol/ml and 4.05 +/- 0.69 (male) and 5.19 +/- 0.35 (female) pmol/ml, respectively. The histamine concentrations in the bronchial asthma group were significantly lower than those in the control group, although no significant sex-related difference was found. On the other hand, the histamine concentrations in the patients with bronchial asthma after provocation with histamine were significantly higher than when the patients were in a quiescent stable. In this study we can get the behavior of the slight changes in bronchial asthma by considering the finding for the patients, sampling trials and selection of the determination method.

An unusual case of bronchial carcinoid predominantly consisting of large cells.

A 58-year-old woman presented an unusual variant of bronchial carcinoid. A tumor measuring 20 x 25 mm was recognized upon gross examination in the upper lobe of the right lung. Microscopically, the tumor consisted of large polyhedral cells with a pseudoglandular arrangement similar to pheochromocytoma cells. Immunohistochemically, the tumor cells contained serotonin, S-100 protein and neuron-specific enolase. Thus, we consider this neoplasm to be a large cell variant of bronchial carcinoid which, to our knowledge, has not been described in the literature.

[Increased neutrophils in bronchoalveolar lavage fluid from a patient with developing adult respiratory distress syndrome].

A 16-year-old, 28-week pregnant woman was admitted to our hospital with multiple bone fractures caused by a traffic accident. She had massive blood transfusion because of anemia in her laboratory findings and ritodrine hydrochloride was administered because of the fear of threatened abortion. She developed a cough with bloody sputum on the 4th day after admission, and developed pulmonary insufficiency with PaO2 41.0 torr and presented bilateral diffuse infiltrates on chest roentgenograms on the next day. Swan-Ganz catheterization revealed normal pulmonary capillary wedge pressure and analysis of the lavage fluid from the patient showed an increase in the percentage of neutrophils (40.0%) and the existence of leukotriene B4 which is known to be the most potent chemokinetic and chemotactic agent for neutrophils. Her condition was considered to be permeability edema developing adult respiratory distress syndrome (ARDS) and 1 g/day of methylprednisolone was administered intravenously for 3 days, which brought about remarkable improvement of her respiratory failure. This report suggests that analysis of the lavage fluid may provide useful information for the early diagnosis of ARDS and the indications of corticosteroid treatment.

[Treated case of aortic valve regurgitation associated with Behçet's disease].

Aortic valve replacement was performed to the incompetence of valve cusps with the Behçet's disease. Artificial valve was fixed from the outside of the aortic wall using teflon strips. Satisfactory results were obtained. There are 18 cases of aortic valve regurgitation accompanied with Behçet's disease including this reported case. Not only the aortic valve but also surrounding aortic walls are involved by the inflammatory lesion in Behçet's disease, so that such fixation technique of the prosthetic valve as demonstrated above might be useful. Since Behçet's disease is systemic, progressive and chronic disease, anti-inflammatory treatment with the adrenal steroids is required prior to and after the operation, and there is no distinct method of treatment for Behçet's disease, strict observation of the patient and systemic controls should be recommended continuously.
