Cardioprotective effects of lidoflazine in extensive aorta-coronary bypass grafting.

The cardioprotective effects of lidoflazine, a calcium entry blocker, were tested in patients undergoing multiple aorta-coronary bypass grafting (at least four grafts). Intermittent aortic cross-clamping at 25 degrees to 28 degrees C was used. Mean cross-clamp time was 11 minutes for one distal anastomosis. Patients were randomized into three groups: a control group (I), a group (II) pretreated with 0.5 mg . kg-1 lidoflazine intravenously before cardiopulmonary bypass (CPB), and a group (III) pretreated with 1 mg . kg-1 lidoflazine intravenously. The following markers of ischemia are used: (1) adenosine triphosphate (ATP), creatine phosphate (CP) and glycogen determined in transmural left ventricular biopsy specimens taken at the beginning and end of CPB; (2) ultrastructure in a similar pair of specimens; and (3) hemodynamic recovery 15 minutes after cessation of CPB. At the end of the intervention, ATP decreased to 73% in Group I but remained unchanged in Groups II (98%) and III (88%). CP decreased to 82% in Group I and remained unaltered in Groups II (100%) and III (110%). Glycogen decreased in Group I (to 44%) and in Group II (78%) but remained unchanged in Group II (138%). Ultrastructural study showed better preservation of the glycocalyx and sarcolemma in Group III than in Group I. Left ventricular stroke work index remained unaltered after CPB in Group III but decreased in Groups I and II to about 60% of its initial value. Thus lidoflazine pretreatment protects the myocardium in a dose-dependent manner against deterioration of myocardial function and structure.

Thrombolysis with human extrinsic (tissue-type) plasminogen activator in dogs with femoral vein thrombosis.

Extrinsic (tissue-type) plasminogen activator (plasminogen activator) was isolated either as a single-chain or as a two-chain molecule from the culture medium of a human melanoma cell line. The thrombolytic activity of both molecular forms of activator was investigated in beagle dogs with an experimental femoral vein thrombosis and compared with that of urokinase. The 125I-fibrinogen-labeled thrombus was formed in an isolated 4-cm segment of the vein, aged for 30 min, and the thrombolytic substances were infused over a 4-h period. The degree of thrombolysis was measured 2 h later as the difference between the injected and recovered 125I. In six control animals with a saline infusion the extent of thrombolysis was 16.3 +/- 3.8% (mean +/- SEM), in five dogs receiving 100,000 IU urokinase, 17.4 +/- 3.7% (P less than 0.4) and in four dogs with 1,000,000 IU urokinase 40.6 +/- 4.8% (P less than 0.001). Infusion of 100.000 IU single-chain plasminogen activator in five dogs resulted in 3.5 +/- 7.8% lysis (P less than 0.05) and of 100,000 IU two-chain plasminogen activator in five dogs in 60.1 +/- 10.8% (P less than 0.001). Infusion of 300,000 IU one-chain plasminogen activator yielded 57.5% lysis and of the same amount of two-chain plasminogen activator 72.9%. Significant activation of plasminogen, consumption of alpha 2-antiplasmin, and fibrinogen breakdown in plasma was only observed in animals receiving the high doses of urokinase but not in the saline, plasminogen activator, or the low-dose urokinase groups. It is thus concluded that in this thrombosis model human extrinsic plasminogen activator has a higher specific thrombolytic effect that urokinase. Plasminogen activator also appears to induce thrombolysis without systemic fibrinolytic activation and fibrinogen breakdown.

Ultrastructural correlates of left ventricular contraction abnormalities in patients with chronic ischemic heart disease: determinants of reversible segmental asynergy postrevascularization surgery.

The relationships between structural alterations and left ventricular (LV) contraction abnormalities were studied in patients with coronary artery disease (CAD). Transmural biopsies of the LV anterior free wall were taken during aortocoronary bypass surgery (CABG) in 62 patients. When preoperative anterior wall motion (AWM) was reduced, significant myocardial cell degeneration was found in patients with as well as without previous anterior infarction (MI). The amount of myocardial fibrosis was increased only in patients with ECG evidence of previous anterior MI (p less than 0.001). In a second series of 139 CAD patients, cineventriculograms performed before and 8 months after CABG were examined. In patients with patent grafts to the LV anterior wall not previously infarcted, reduced AWM became normal. In patients with previous anterior MI the outcome of AWM was unpredictable (usually unimproved). Thus the histologic correlate of reduced AWM in segments not previously infarcted was progressive loss of contractile material in otherwise viable myocardial cells. Some reversibility was suggested by restoration of resting function after CABG. Unpredictable results in segments associated with pathologic Q waves appear related to the fibrous component of these previously infarcted areas.

Aortofemoral grafting: factors influencing late results.

During a 15-year period, the aortofemoral Dacron graft was used for revascularization of 615 limbs in 352 patients, with an overall operative mortality rate of 5.1% and a cumulative 10-year patency rate of 62%. The late complication rate was 27%, with late thrombosis being by far the most frequent complication (24%). Among several factors responsible for late occlusion, the presence of trifurcation disease and the site of the femoral anastomosis are by far the most important. Our procedure of choice for a thrombosed graft limb is thrombectomy and profundaplasty, which give a long-term patency rate of 68%.

Clinical trial of brinase and anticoagulants as a method of treatment for advanced limb ischemia.

A randomized, single-blind trial of repeated intravenous infusion of Brinase was carried out in 70 petients with severe chronic limb ischemia, who were candidates for lumbar sympathectomy or amputation. The enzyme caused s significant increase in calf and ankle pressure index. At six months follow-up, the clinical results were statistically significant in favour of Brinase when all patients were considered, but not if 10 patients with Buerger's disease were omitted from the analysis. Patients treated with a combination of Brinase and coumarins had a better clinical outcome than patients receiving either treatment on its own.

Initial experience with coronary artery bypass graft surgery. A follow-up study.

The preoperative, postoperative and late follow-up data of an initial series of two hundred patients with coronary artery disease, who underwent direct coronary artery surgery, were analyzed. With increasing experience operative mortality, postoperative complications, graft closure and perioperative myocardial infarction showed a definite decrease. Patency rate was definitely related to symptomatic relief, return to work, postoperative exercise tolerance, and the absence of perioperative myocardial infarction.

[Aortic arch syndrome. Stenosis and obstruction of the sub-clavian artery in women (author's transl)].

Takayasu's disease evolves in two stages: The pre-obstructive state: frequently it gives rise to infectious diseases which often resemble rheumatic fever. Often there exist fever and joint pains. Laboratory examinations show the presence of an inflammatory process: increased ESR hypoalbuminuria and increased alpha-2 and gammaglobulins. Then one reaches the obstructive stage. Here the diagnosis of the disease is often suspected or confirmed. Often there are eye signs with the characteristic appearance of arteriovenous anastomoses in the fundus oculi. These signs were absent. The etiology is unknown. The authors propose the following definition: that this is a form of juvenile arteritis occurring, above all, in women and involving one or other of the main aortic trunks, but which may be diffuse. The disease is often inflammatory. The etiology is unknown.

[The tetralogy of Fallot: surgical treatment. 145 cases (author's transl)].

1. The overall operative risk for the palliative procedures for Tetralogy was 3,2% (93 cases). The Blalock-Taussig shunt remains the intervention of choice in older children. The Waterston shunt gives optimal results in the infants under six months of age but on long term the hasard of pulmonary hypertension still exists. 2. Before 1965, the mortality rate for the total repair was 43,7% (16 cases). During the last 8 years, the mortality rate decreased to 5,7% (36 cases). One operative death after total correction with a Potts shunt is not included in this figure. This type of shunt was replaced by the Waterston shunt after 1966, because of the high operative risk total correction and the high frequency of pulmonary hypertension on long term. 3. The long-term results were excellent in 85% of the survivors (mean follow-up of 4,1 years). A pulmonary insufficiency was found on clinical examination in 35% of the cases. This insufficiency was well tolerated by all patients. The long-term consequences, however, are uncertain. 4. It seams rational to proceed to an earlier total correction in these cases when well defined criteria are fullfilled, as the mortality figures of the palliative and corrective procedures have a tendency to reach each other: (3,2 versus 5,7%).
