[Normal values in the Aulhorn flicker test].

In this paper the results of the Aulhorn flicker test in 105 healthy subjects are described. With decreasing flicker frequencies there was an increasing sensitivity to brightness below the individual flicker fusion frequency. These results contrast with those obtained in patients with acute optic neuritis. In these patients the sensitivity to brightness decreased at low flicker frequencies. The range of frequency intervals between 3 and 50 Hz in which the maximum increase in brightness sensitivity was be observed was also investigated. The results have been compiled in a diagram for clinical use.

Pressure compliance of the optic nerve head in low tension glaucoma.

Twenty eyes of 10 healthy subjects, 11 eyes of seven patients with low tension glaucoma, and three eyes of three patients with ischaemic optic neuropathy were investigated. Visual evoked responses were recorded under stepwise artificially increased intraocular pressures. The results of the visual evoked response recording (pressure compliance test) allow a clear distinction to be made between healthy subjects, patients with low tension glaucoma, and patients with ischaemic optic neuropathy. In the groups investigated a lack of autoregulation of the optic nerve head circulation was found in patients with low tension glaucoma only. Patients with anterior ischaemic optic neuropathy showed the same pressure compliance behaviour as healthy subjects. The methods used here seem to provide a practicable clinical tool in the differential diagnosis of low tension glaucoma.

[A test of pressure tolerance of the optic nerve head in ocular hypertension].

VER was recorded in 7 patients with ocular hypertension while intraocular pressure was artificially elevated. Four of the patients manifested signs of vascular autoregulation, while in 3 no such signs were observed. This absence of autoregulation was also observed in primary open-angle glaucoma patients. On the basis of these first results it may be expected that the pressure compliance test will identify patients with ocular hypertension in whom damage to the optic disk is likely to occur at a later date.

The 'flicker test' according to Aulhorn in the diagnosis of acute optic neuritis.

The flicker test, introduced by Aulhorn is a new simple method to detect acute optic neuritis. We examined 24 patients suffering from acute optic neuritis. 23 patients showed a pathological result. In 179 subjects (patients with other ophthalmological diseases and healthy persons) the flicker test always showed normal results. After restitution of acute optic neuritis the results of the flicker test return to normal while the visual evoked responses remain pathological in many cases. Thus the flicker test can be an efficient tool in the diagnosis and differential diagnosis of acute optic neuritis.

Effect of timolol on optic nerve head autoregulation.

Healthy subjects show, in a special pressure compliance test of the optic nerve head, signs of autoregulation of optic nerve head circulation. In this test visual evoked response (VER) amplitudes were recorded during stepwise increased intraocular pressures. In the resulting VER amplitude/pressure curves glaucoma patients did not show the sign of autoregulation of optic nerve head circulation which is seen in healthy subjects. It is known that topically applied timolol maleate may affect optic nerve head circulation. Therefore the pressure compliance test was performed in 20 healthy eyes before and after 3 days of topically applied timolol maleate 0.5%. The aim of the study was to test the effect of topically applied timolol on optic nerve head autoregulation. In addition we determined the ocular perfusion pressures, heart rate and mean arterial blood pressure. While the ocular perfusion pressures show a slight, but statistically not significant, decrease under timolol, the systemic parameters and the autoregulation behavior did not change.

[Comparative study of the behavior of intraocular pressure in anesthesia induction by diazepam and midazolam].

20 patients scheduled for cataract extraction were randomly assigned to two groups. Before anaesthesia was induced, group A patients received 0.3 mg/kg body weight diazepam, whereas group B was given 0.15 mg/kg body weight midazolam. The ocular pressure, blood pressure, pulse and respiratory rate were determined and a capillary blood gas analysis was performed prior to the administration of the tranquilizer (time I), after the patient fell asleep (time II) as well as 5 min. after intubation (time III). Before intubation the patients received fentanyl and succinylcholin. The reduction in ocular pressure is initially somewhat less in the midazolam group, but is identical to that in the diazepam group at time III where it fell to 57% of the preinduction value. In combination with fentanyl, these two drugs are suitable for anaesthesia in intraocular surgery.

Autoregulation of ocular blood flow during changes in intraocular pressure. Preliminary results.

Visual evoked potentials were recorded in six normal subjects and in three patients with primary open-angle glaucoma, while intraocular pressure was artificially elevated in stepwise increases. The resulting perfusion-pressure amplitude curves in our controls showed a kink, which can be interpreted as a sign of vascular autoregulation. No kink was observed in the curves of our glaucoma patients, which we interpret as indicating a lack of vascular autoregulation in primary open-angle glaucoma.

[Ulrich oculo-oscillodynamography: initial results in carotid stenoses].

Oculo-oscillodynamography (OODG) after Ulrich was performed in 11 patients suffering from a monolateral, hemodynamically relevant carotid stenosis identified by Doppler sonography. All of the pressure parameters which can be measured by OODG--systolic retinal, systolic ciliary, and diastolic ocular perfusion pressure--were low on the side concerned. In contrast to the already-established methods of measuring ocular perfusion--ophthalmodynamometry and ophthalmodynamography after Hager--the advantages of OODG are that it not only enables the systolic retinal pressure to be differentiated from the systolic ciliary or diastolic ocular perfusion pressure, but also permits the results of the investigation to be illustrated graphically, so that objective evaluation is possible.

[Ocular hemodynamics in central and branch artery occlusions].

Ocular oscillodynamograms (OODG) were made in 9 patients with an occlusion of the central artery and 11 patients with an occlusion of one branch artery. While there were no pathologic findings in patients with branch artery occlusions, it was not possible to record any systolic-retinal oscillations of the pulse in patients with an occlusion of the central artery. In three cases the systolic ciliary perfusion pressure was also low owing to carotid stenosis.

Flicker fusion frequency and organic syndrome in alcoholics.

Flicker fusion frequencies of 48 healthy controls and of 35 alcoholics were measured to detect possible associations between flicker fusion and signs of nonspecific brain damage, the aim of the study. FFF was statistically significantly lower in patients addicted to alcohol than in healthy subjects. Lower flicker fusion frequencies were associated with the severeness of organic psychosis as rated psychopathologically. The hypothesis that addiction to alcohol is a sign of diffuse, nonspecific, organic brain damage was discussed.

[Influence of a barrier filter on tear film breakup time].

In 30 healthy volunteers the tear-film break-up times of both eyes were determined, comparing one eye with a cut-off filter (customarily used in fluorescein angiography (Schott OG 530, thickness 3 mm] with the common method, not using this filter, in the other eye. By using the filter the variance of observed break-up times is decreased, and the time elapsing after the administration of the dye no longer influences the break-up time significantly. The filter described here is therefore recommended for determining the tear-film break-up time.
