Affinity chromatography of antiserum to a gram negative organism.

An immunoadsorbent method is described for the purification of antibody directed against gram negative bacteria. Two anion exchange materials were compared for their ability to immobilize whole cells of Veillonella alcalescens, a gram negative oral bacterium. Purified antibody preparations were applied to the columns and subsequently eluted with various combinations of desorbing buffers. The quantity of recovered antibody was measured and its activity assayed, using a microagglutination technique. The highest levels of protein and specific antibody activity were recovered from Dowex-1-acetate columns desorbed by a combination of borate buffer pH 10 and 3 M KSCN pH 6, followed by levels of specific antibody activity obtained from a DEAE cellulose column desorbed by glycine--HC1, PBH 2.3.

Influence of coronary artery anatomy on survival following resection of left ventricular aneurysms and chronic infarcts.

Left ventricular (LV) aneurysms were resected in 27 patients for treatment of congestive heart failure and in five patients for treatment of ventricular arrhythmia. There were eight early (25 per cent) and five late (15 per cent) deaths. Preoperative hemodynamics including analysis of left ventriculograms in the right anterior oblique position did not consistently predict survival. In contrast, coronary artery anatomy appeared to influence the mortality rate strikingly in patients with congestive heart failure. The hospital mortality rate was 6 per cent and the late mortality rate was 13 per cent for 16 patients with one-or two-vessel coronary artery disease, but with unobstructed LV lateral wall blood supply. All but one of the survivors obtained a good late result. In nine patients with two-and three-vessel coronary artery disease and obstructed LV lateral wall blood supply there were six hospital deaths and two late deaths. It is concluded that the presence or absence of occlusive disease in the arteries supplying the LV lateral wall is an important determinant of the mortality rate associated with resection of anterior apical LV aneurysms in patients with severe congestive heart failure.

Licensure, competence, and manpower distribution. A follow-up study of foreign medical graduates.

Medical statistics report United States medical graduates licensed at higher rates than foreign medical graduates. This difference is often interpreted to show greater medical competence of United States graduates. This study questions this interpretation by analyzing 1971 licensure rates for both groups who had been interns and residents in 1963. We found that factors unrelated to competence--namely, visa-citizenship status and state of examination--are associated with holding a license. Moreover, quality of medical education is not an accurate predictor of licensure. It follows that the use of licensure rates as measures of medical competence distorts understanding of the quality of medical care in the United States. More probably, the difficulties in obtaining medical licensure experienced by foreign graduates result from the use of such graduates to relieve specific medical-manpower shortages.

The internal structure of the Eating Disorder Inventory.

The Eating Disorder Inventory (EDI; Garner, Olmsted, & Polivy, 1983) is used as a screening instrument for identifying potential anorexia nervosa and bulimia nervosa. It comprises eight subscales: Bulimia, Drive for Thinness, Body Dissatisfaction, Maturity Fears, Introceptive Awareness, Perfectionism, Interpersonal Distrust, and Ineffectiveness. We examined the internal structure of the EDI using a principle components factor analysis, for a sample of women with no known eating disorder. The results of the factor analyses showed differences between the extracted factors and Garner et al.'s subscales. Seven factors were extracted, accounting for 60.8% of the variance. The first factor accounted for 33.5% of the variance. The analysis indicated that items factored along positive-negative and factual-emotional lines. We conclude that at present there is not enough evidence to support the efficacy of the subscale structure with women who are not known to have eating disorders.

When your clients are in jail.

Nursing in jails has been present for a long time and yet is still an emerging practice field. Often nurses are isolated in their practice arena and may be confused by the culture of the setting. The author examines the culture of correctional health care in an attempt to assist jail nurses in developing a framework for practice. Elements of a correctional culture are examined and contrasted with a nursing culture.
