Classification and assessment of cerebral bioavailability of lopirazepam (D-12524) by quantitative EEG and psychometric analysis.

In this double-blind, placebo-controlled study the CNS efficacy and pharmacodynamic properties of 3-hydroxy-5-(o-chlorophenyl)-7-chloro-1,2-dihydro-3H-pyrido[3,2-e]-1,4-diazepin-2-one (lopirazepam, D-12524) was investigated in a group of 10 normal volunteers. Quantitative EEG and psychometric analyses and clinical evaluations were done before as well as 2, 4, 6 and 8 h after oral administration of single doses of placebo, 3 mg, 5 mg and 10 mg lopirazepam and 10 mg prazepam. EEG digital computer period analysis demonstrated dose-dependent changes which are typical of the class of anxiolytics and which started in the 2nd hour, peaked in the 4th hour and lasted after 3 mg, 5 mg and 10 mg up to the 4th, 6th and 8th hour, respectively, 10 mg D-12524 produced an additional augmentation of slow waves indicating sleep-inducing qualities in this dosage. The equipotent dosage to 10 mg prazepam seems to be 5 mg. Psychometric analyses demonstrated an improvement in attention, psychomotor performance, mood and affectivity after 3 mg, opposite changes after 10 mg D-12524. Flickerlight fusion, reaction time, after-image and EAS score were determined as well. Data concerning side effects, pulse rate and blood pressure suggested a good tolerance of the drug.

Regional myocardial blood flow after hypothermic arrest and cardioplegia.

Malperfusion due to increased coronary vascular resistance is presumably one of the factors responsible for incomplete functional recovery of the heart after aortic cross-clamping. Myocaridal blood flow (MBF, radioactive microspheres) was measured before and after 60 min of hypothermic ischemia in 16 dogs on cardiopulmonary bypass. After ischemia the hearts were reperfused for 30 min. MBF was measured in the empty beating heart and in the isovolumetrically contracting ventricle loaded with enddiastolic volumes (EDV) of 10, 20 and 30 ml (intraventricular latex balloon).

[Maturation and aging of elastic fibers (authors transl)].

The non-sunexposed skin of 12 healthy individuals was investigated under the electron microscope and the fine structure of normal elastic fibers was compared by different methods in 3 groups of age: 1--10 years, 30--50 years, and 60 years. 1. The elastic fibers of human skin may be semiselectively stained with silver tetraphenylporphyrine sulfate (Ag-TPPS), uranyl acetate, lead citrate and tannic uranyl acetate. They are composed of an amorphous matrix which is interwoven and surrounded by microfilaments. Elastic fibers are fully developed in early periods of age, first showing numerous microfilaments as structural glycoproteins, and then the deposition of the amorphous proelastin to be transformed into elastin. 2. The period of maturation is followed continuously by processes of physiological aging: a) decreasing number of microfilaments, b) appearing of electron-dense inclusions into the elastin matrix and c) fragmentation and disintegration of the fiber. 3. These changes indicate that aging of the fiber includes diminished synthesis of structural glycoproteins (microfilaments) and increasing content of osmiophilic groups, such as polar aminoacids, fatty acids and calcium salts. The fiber becomes thus more susceptible to the activity of elastase, and may totally disintegrate in older age.

[Effects of the new brondilatator reproterol on the cardiac function during treatment of bronchial asthma and chronic obstructive bronchitis (author's transl)].

The new beta-adrenergic phenylethy-aminoalkyl-xanthine derivative 7-(3-[2-(3,5-dihydroxyphenyl)-2-hydroxy-ethylamino]-propyl)-theophylline (reproterol, Bronchospasmin) was tested in two patient groups afflicted with bronchial asthma or chronic obstructive bronchitis. Neither single i.v. administration nor single and 4-week long-term application of blood pressure or ECG tracings. Dosages of i.v. 60 and 90 mug, single peroral administration of 20 mg or long-term therapy with 60 mg in either 3 or 6 daily doses, definitely and significantly improved disturbances in the pulmonary ventilation (Rt, EGV, FEV1). This new preparation represents a predominantly beta2-mimetic broncholysis as well as the absence of cardiovascular side effects.

Treatment of cutaneous T cell lymphoma.

Primary cutaneous T cell lymphomas encompass a wide variety of lymphomas that are characterized by a distinct clinical presentation. Advanced biological techniques have allowed a more precise classification in recent years. Stage-adapted therapy is at present the best approach to treat cutaneous T cell lymphoma.

Surgical treatment of thrombotic occlusion of pelvic veins.

During a three-year period, 20 patients were operated on for acute thrombosis of the pelvic veins. Seventeen of them underwent a thrombectomy and the remaining three had a femorofemoral bypass after Palma. As regards long-term results in 18 patients, between one and four years postoperatively, phlebography in the thrombectomized group revealed ten open and five occluded vessels. Two patients died. In the group with bypass, two grafts were open. The relatively good results in spite of delayed intervention (in some cases eight or nine days after occlusion) suggest that even if performed after the generally accepted six day deadline, it is worthwhile to attempt an operation. The importance of an early diagnosis is stressed; among the many characteristic symptoms "heaviness" of the affected limb is an early complaint which aids diagnosis.
