Blockade of 1-methyl-4-phenylpyridinium ion (MPP+) nigral toxicity in the rat by prior decortication or MK-801 treatment: a stereological estimate of neuronal loss.

1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine, (MPTP), produces a parkinsonian syndrome both in man and in experimental animals. Its toxicity is mediated by a metabolite, the 1-methyl-4-phenylpyridinium ion (MPP+). When injected into the striatum, MPP+ is accumulated by dopaminergic nerve terminals and retrogradely transported to the substantia nigra pars compacta (SNc) where it causes neuronal degeneration. MPP+ accumulates in mitochondria and blocks complex I of the electron transport chain. A proposed mechanism of neurotoxicity is excitotoxic neuronal degeneration induced by this energy depletion. We examined whether either prior decortication or administration of the N-methyl-D-aspartate (NMDA) receptor antagonist, (+)-5-methyl-10,11-dihydro-5H-dibenzo[a,d]cyclohepten-5,10-imine maleate (MK-801) could prevent or diminish the selective nigral neuronal degeneration that follows unilateral intrastriatal injection of MPP+. We quantified the extent of neuronal death in the SNc ipsilateral and contralateral to the injections on Nissl-stained sections with unbiased stereological techniques. One week after injection of MPP+, approximately 75% of the SNc neurons were lost on the side of the injection. The loss was a consequence of the reduction in both SNc volume and neuronal density. Both prior decortication or the administration of MK-801 for 2 days nearly completely prevented MPP(+)-induced neuronal loss in the ipsilateral SNc. These results are consistent with an NMDA receptor mediated excitotoxic mechanism for MPP(+)-induced nigral toxicity.

Formation and storage of 11-cis retinol in the eyes of lobster (Homarus) and crayfish (Procambarus).

Modes of storage and mechanisms of formation of 11-cis retinoids in the eyes of animals vary widely among the major phyla. We here describe evidence from two species of macruran decapod crustacea that point to different processes from those known in insects, the other group of arthropods for which there is extensive data. The eyes of the lobster (Homarus) contain about 300 pmol of retinal, somewhat less free retinol, and variable amounts (up to 1000+ pmol) of two retinyl esters, over 90% of which contain retinol in the 11-cis configuration. The major ester contains the long chain, polyunsaturated fatty acid docosahexaenoate (C22:6), but retinyl oleate (C18:1) is also present. Crayfish (Procambarus) contain the same retinyl esters, although in much smaller amounts. Homogenates of the eyes of both species are capable of isomerizing all-trans retinyl docosahexaenoate to the 11-cis configuration without using the energy of light. Crude fractionation of homogenates shows isomerase activity associated with membranes. The reaction mechanism has not been explored in detail, but on the basis of present evidence it may be similar to that found in vertebrate pigment epithelium. It is clearly different from the light-dependent processes known in insects (Hymenoptera and Diptera) and cephalopod mollusks, where isomerization takes place at the level of the aldehyde and 11-cis retinyl esters are not present as major storage reserves.

Spectral and polarization sensitivity of photocurrents of amphibian rods in the visible and ultraviolet.

Photocurrents from isolated rods of adults and sub-adults of three species of amphibians, Rana pipiens, Ambystoma tigrinum, and Xenopus laevis, were measured with suction pipette electrodes. The intensity for a half-maximal response was 0.91+/-0.48 photons microm(-2) flash(-1) (mean +/- S.D., 10-ms flashes) for Rana, 0.92+/-0.44 for Ambystoma, and 6.14+/-1.33 for Xenopus. The mean number of photoisomerizations at half-saturation was 22+/-12 for Rana, 50+/-24 for Ambystoma, and 221+/-48 for Xenopus. The photocurrent per photoisomerization is several times smaller in Xenopus rods than in the other two species. Spectral sensitivity was measured from 277-737 nm with light polarized both parallel and perpendicular to the planes of the membrane disks. Dichroism fell in the near UV and was absent in the region of absorption by tryptophan and tyrosine. Maximum sensitivity of Rana was at 503.9+/-2.6 nm (n = 86), and of Ambystoma, 505.8+/-1.8 nm (n = 24). Animals from these same batches that were sampled by HPLC had no 3-dehydroretinal (retinal2). Xenopus containing about 94% retinal2 and 6% retinal1 had lambda(max) at 519.3+/-2.7 nm (n = 11). Spectral position of the beta-band, estimated by the method of Stavenga et al. (1993), appears to be at longer wavelengths in amphibian photoreceptors than in other vertebrates. Fits of log sensitivity to a normalized-frequency template that tracks the long-wavelength tail of the alpha-band (Lamb, 1995) show that the rod pigments of Rana and Ambystoma are slightly narrower than those found in the photoreceptors of fish and mammals.

Dermatomycoses among industrial workers in Cross River State, Nigeria.

A survey of dermatomycoses was carried out amongst industrial workers in three different factories during the period 1987-1988. A total of 194 workers were screened, out of which 54 proved to be mycologically positive by microscopy and/or culture. Incidence was apparently highest amongst workers in a cement factory (Calcemco, 33.3%) followed by those in a wood factory (Seromwood, 30.8%) and a rubber factory (CREL, 26.2%). Pityriasis versicolor was the predominant clinical type of dermatomycosis, followed by tinea pedis. A total of 51 fungal organisms were identified. Malassezia furfur was the most prevalent causative agent (74.4%) followed by Trichophyton soudanense (5.8%), T. rubrum (3.9%) and Epidermophyton floccosum (3.9%). Other species identified were one isolate each of T. tonsurans, T. mentagrophytes, T. verrucosum, Candida tropicalis, Candida spec. and Geotrichum candidum.

Isoconazole nitrate in the treatment of tropical dermatomycoses.

A total of 54 patients with culturally proven tropical dermatomycoses, comprising 23 with various types of dermatophytoses, one with foot infection due to Trichosporon beigelii and one with foot infection due to Geotrichum candidum, two with candidosis of the groin and 27 with pityriasis versicolor, were included in a clinical trial of efficacy of 1% isoconazole cream (Travogen, Schering, Berlin, Germany). Five patients were not evaluable. A clinical and mycological cure was achieved in 29 cases in 3-4 weeks. In 15 (31%) of the remaining patients treatment was required for 5-6 weeks, while another three patients required treatment for 8 weeks. In two patients the disease proved to be resistant to treatment with the drug.

Pediatric hospitalists in Canada and the United States: a survey of pediatric academic department chairs.

OBJECTIVES: To document the prevalence and practice patterns of pediatric hospitalists in academic centers in Canada and the United States; to characterize academic pediatric department chairs' definition of the term hospitalist; and to characterize pediatric department chairs' views of the training requirements for pediatric hospitalists. METHODS: A 14-item questionnaire was sent to all 145 pediatric department chairs from Canada and the United States during the fall of 1998. We defined hospitalists as physicians spending at least 25% of their time in inpatient care. RESULTS: Of the 145 eligible pediatric chairs, 128 (89%) responded (United States, 111/126; Canada, 14/16; Puerto Rico, 3/3). Ninety-nine (77%) of 128 pediatric chairs either have (64/128) or are planning to have (35/128) hospitalists in their institutions. Within academic programs with hospitalists, 82% of hospitalists currently work on general pediatric wards. Two thirds of hospitalists teach, 50% provide outpatient care, 50% have administrative duties, and 44% conduct research. One hundred eight (84%) of 128 believe that hospitalists should spend at least 50% of their time in inpatient care. Less than one third (30%) of pediatric chairs believe that hospitalists require training not currently provided in residency. CONCLUSIONS: A large proportion of academic pediatric centers either employed or planned to employ hospitalists in 1998. Pediatric academic department chairs do not see a need for training beyond residency for hospitalists. Further studies should address how pediatric hospitalists affect quality of care, cost, and patient satisfaction.

Effect of vitamin "A" on free radical cascade in pregnancy induced hypertension.

The role of vitamin A therapy in pregnancy induced hypertension (P.I.H.) on free radical cascade was studied in P.I.H. patients of third trimester. It was found that vitamin A therapy causes statistically significant decrease in lipid peroxidation. However it has no effect on superoxide dismutase and catalase. This indicates that vitamin A acts as chain breaking antioxidant. The present study opens a future prospective of giving vitamin A for the prophylaxis of P.I.H.

Differential efficacy evaluation of formulations of Bacillus thuringiensis Var israelensis against mosquitoes: a laboratory investigation.

The efficacy evaluation of three formulations; wettable powder (W.P.) floating pellet and beads of Bacillus thuringiensis Var israelensis (Bti), revealed a greater susceptibility of the early larval instars of mosquitoes to Bti, sensitivity of anophelines to floating pellet, culicines to bead and equal efficacy and faster kill of W.P. to all the mosquitoes tested. A greater persistence of the slow release formulations, floating pellet and beads for 49 and 28 days against anophelines and culicines respectively was observed in contrast to a maximum persistence for 21 days in case of W.P. formulation.

Radiotherapy for splenomegaly.

Radiotherapy for massive, symptomatic splenomegaly has been used in a palliative setting since the early 1990's. Massive splenomegaly may be seen in CML, CLL, hairy cell leukemia and splenic marginal zone lymphomas, prolymphocytic leukemia, myeloproliferative disorders such as polycythaemia rubra, polycythaemia vera or essential thrombocytosis or myelofibrosis. Splenic radiation therapy has been shown to be effective in palliation of the signs and symptoms due to massive splenomegaly. We present here one such case of myelofibrosis where the patient was treated with radiotherapy to the spleen for symptomatic relief. The patient achieved excellent response to the treatment.

Brachytherapy↗

Curative interstitial brachytherapy for early stage carcinoma lip.

Brachytherapy is potentially useful in the treatment of head-and-neck cancers, because most tumor sites, such as the lip, tongue, floor of mouth, tonsil, pharynx, nasopharynx, sinuses, and neck, are accessible for the placement of afterloading applicators and catheters. It has the advantage of delivering a higher radiation dose to the tumor while sparing surrounding normal tissue from radiation. Furthermore, the overall treatment duration is shorter, and the dose distribution confirms to tumor shape. Brachytherapy is used as "monotherapy" for the treatment of small primary tumors or recurrent disease after external beam radiation therapy (EBRT). Small cancers of the lip (less than 2 cm) are treated equally well with surgery or radiation therapy with excellent cosmetic and functional results seen in radiation therapy. We treated an 18-year old male diagnosed as stage I carcinoma lip with curative interstitial brachytherapy. He was treated to a dose of 3 Gy per fraction, two fractions per day at interval of 6 hours between the two fractions for 6 days. He therefore received a total dose of 36 Gy with High Dose Rate (HDR) Brachytherapy, which is equivalent to 58.36 Gy conventional radiotherapy dose. At two months follow, the patient is clinically disease free and has no complaints.

Breast cancer detection practices of south Asian women: knowledge, attitudes, and beliefs.

PURPOSE/OBJECTIVES: To explore knowledge, attitudes, beliefs, and practices regarding breast cancer detection practices among South Asian women. DESIGN: Descriptive exploratory design. SETTING: Toronto, Ontario, Canada. SAMPLE: 57 South Asian women, age 40 and over, who are first generation immigrants from India and Pakistan and speak one of the four languages identified for the study--Hindi, Punjabi, Gujarati, or Urdu. METHODS: An interview guide was designed specifically for this study. It contained questions regarding knowledge, attitudes, beliefs, and practices about breast self-examination (BSE), clinical breast examination (CBE), and mammogram. In addition, questions assessing the variables of the Health Belief Model and health motivations also were included. The data were obtained during face-to-face interviews in the primary language of the participating woman. The interviews were transcribed and translated into English. FINDINGS: 12% of the participants practiced BSE monthly, 49% had undergone at least one CBE during their lives, and 47% had never had a mammogram. The majority (54%) said they did not know very much about breast cancer. While 21% of the women said detecting cancer early was important, only 5% reported that cancer could be cured. Age, education, or mother tongue showed no statistically significant relationship with the breast health practice scores. However, proficiency with the English language (p = 0.009) and number of years in Canada (p = 0.009) had a significant relationship with the breast health practice scores. The significant explanatory factor for the variable breast health practices was a cue to action (p = 0.009). CONCLUSIONS: South Asian women with minimal knowledge of breast cancer did not engage in breast cancer detection practices. IMPLICATIONS FOR NURSING PRACTICE: This segment of the population of immigrant women needs to be better informed about breast cancer and the benefits of breast cancer detection practices.
