The Küntscher nail has proved successful in the treatment of non-unions of the median third of the femur and lower leg after opening of the medullary cavity; in fact, this method is described in literature as the method of choice. It is based on the assumption that a sufficiently dimensioned intramedullary Küntscher nail together with the biological stimulative action of the bone meal, will result in a rapid osseous bridging of the non-union. This method has been successfully used in 128 cases even in non-unions which were close to the joint. Healing occurred in 92% of the cases. In about 11 cases it became necessary to perform a secondary operation. The high rate of infection, amounting to 12.4%, is relatively low compared with the very high rate of infective relapses following the first operation to achieve healing of the bone fractures.
The "fixateur externe" made from polymer engineering materials offers the following advantages over systems made from metal: it is largely transparent to x-rays; the weight is significantly lower; shape and surface are body-compatible; manipulation is easy; and production can be effected at a reasonable price. A multitude of variants in respect to mounting can be employed to change the well-known spectrum of complications is not extended. The individual construction components are described and examples of mountings at various segments of the extremities are demonstrated.
The effect of prolonged inspiration time and PEEP on compliance, haemodynamics and gas exchange was studied on 15 patients, who were ventilated because of respiratory distress syndrome. The patients were ventilated by the Servoventilator 900 BR connected with a calculation unit. The inspiration time varied from 35 to 70%, the PEEP from 0 to 20 cm H2O. All patients were ventilated with constant tidal volumes (10-15 ml/kg b.w.). The compliance rose from 48 ml/H2O (ZEEP) to 61,5 ml/cm H2O (PEEP + 8 ml H2O). A further increase of PEEP caused a decrease of compliance to 48 ml/cm H2O (PEEP + 20 cm H2O). A prolongation of inspiration time of up to 53% showed an augmentation of compliance from 48 ml/cm H2O to 53 ml/cm H2O. Both modes of assisted ventilation caused an improvement of alveolar ventilation. The simultaneous consideration of various parameters of gas exchange, compliance and haemodynamics guaranteed the optimal mode of ventilation and minimal side effects.

With the type of external bone anchorage made from metal and usually employed in Germany, Austria and Switzerland, the considerable weight of such anchorages and their imperviousness to X-rays are major drawbacks. For this reason, the prototype of an external fixation agent was developed which does not posses these disadvantages. Fibre-reinforced polymeric materials appeared a suitable alternative to metal. Satisfactory strength of the components of the system could be expected provided the anchoring was suitably constructed. External bone anchorage with polymer materials is easy to manipulate, transparent to X-rays, sterilisable, and possesses dimenisonal stability to head. The weight reduction amounts to as much as 70%. A multitude of variants can be mounted from just four system components. Economic manufacture is possible through material-specific processing (injection moulding, machining).
